About this file

This document is an Amendment (0004) to Solicitation 36C25225Q0555 for the Tomah VA Medical Center Chapel AV Upgrade project. The amendment extends the offer submission deadline to August 13, 2025, at 13:00 CDT and revises two key service requirements in the Statement of Work: (1) on-site physical response time is modified from 24 hours to "one business day" to avoid weekend responses, and (2) loaner equipment provision is adjusted to be available within 24 hours after an on-site response fails to identify a solution.

The underlying contract opportunity involves upgrading the Tomah VA Medical Center Chaplain Service's audio and video sound system. The contractor will be responsible for comprehensive services including consulting, design, project management, equipment procurement, delivery, installation, licensing, programming, sound system equalization, low voltage wiring, and hardware/cabling installation. The project is being administered by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 12, located in Milwaukee, Wisconsin.
