About this file
This document is Amendment A00002 to a Request for Proposal (RFP) for installing an analog phone line to Building 6 at VA Northern Indiana Health Care, Fort Wayne Campus (Project Number 610A4-26-510). The solicitation is issued by the Department of Veterans Affairs, Network Contracting Office 10, Indianapolis, Indiana.
The amendment extends the offer receipt deadline to September 10, 2026, at 1:30 PM EDT and provides answers to nine clarification questions submitted by potential offerors. Key technical requirements include installation of Category 3 (Cat 3) 25-pair copper twisted-pair wiring from the 100-pair Building Entrance Protector to the 66-block, with higher categories acceptable if copper twisted-pair is used. Cable installation must be field-certified to meet TIA 568-C.2 performance category requirements. The scope covers work in maintenance holes MH #6, MH #7, and MH #8, which contain existing fiber optic cables; MH #7 and MH #8 are configured as double vaults with one side dedicated to 480V power and the other to communications. Termination may be made directly onto the existing 66-block without requiring new building entrance protector or surge-protection modules. The contractor must comply with all Generic Division 27 requirements, including three comparable installations, OEM authorization, material-conformance certifications, cable samples, and test-report formats. For confined-space rescue operations, the contractor may designate FWFD Special Ops at 260-427-1478 rather than providing a dedicated onsite team. Firestop inspection will be conducted by the Contracting Officer's Representative using 3M Fire Barrier Moldable Putty to replace any putty removed during installation. The solicitation is structured as an all-or-nothing proposal; partial proposals will not be considered, and all offerors must perform all aspects of the required work.

QUESTION AND ANSWER (RFI)
CORRECTION AND CLARIFICATION
Disclaimer: Questions that have not received a response were either unclear or lacked sufficient specificity for an answer. All questions must be submitted by 11:30 am on August 28, 2026, to be recorded.
Question 10: What systems are currently installed in the maintenance holes? Is high voltage present?
Answer 10: Fiber optic cables are present.
Question 11: Please provide available information regarding MH #8, MH #6 and MH #7, including dimensions, depth, access conditions, water infiltration, existing cable racks, existing cable quantities, conduit occupancy and known hazards
Answer 11: MH#6 measures 6 feet by 6 feet by 7 feet 6 inches. The exact dimensions of MH#7 and MH#8 are unknown; however, visual inspection indicates they have a smaller footprint.
MH#7 and MH#8 are configured as double vaults, with one side dedicated to 480V power and the other two communications. All work will occur on the communications side. All maintenance holes are accessible from the grounds. Recent work in MH#7 and MH#8 required water removal from the vaults. Each vault is expected to contain four fiber runs.
Question 12: Please confirm whether a new building entrance protector and surge-protection modules are required at Building 6 or whether termination directly onto the existing 66-block is acceptable.
Answer 12: Termination may be made directly onto the existing 66-block.
Question 13: Please confirm whether the contractor must provide a dedicated onsite confined-space rescue team or whether an evaluated third-party emergency rescue service may be designated. Please also identify any.
VANIHCS-specific confined-space orientation, permit approval, rescue coordination, or equipment requirements beyond 29 CFR 1926 Subpart AA and the provided VA CSEP form?
Answer 13: Utilization of FWFD SPECIAL OPS at 260-427-1478 for rescue operations is acceptable.
No additional requirements are specified beyond 29 CFR 1926 Subpart AA and the VA CSEP.
Question 14: Please confirm the required copper-pair acceptance tests and reporting format for all 25 pairs. Please also confirm whether the final functional test consists of restoring and verifying the blue call-pole analog communication circuit and identify who will witness and approve the test.
Answer 14: Following installation, each cable shall be field-certified to meet the performance category requirements in accordance with TIA 568-C.2.
Restoration and verification of the blue call-poles is not required. The scope of work extends from the 100-pair Building Entrance Protector to the 66-block.
Question 15: Please confirm whether the generic Division 27 requirements for three comparable installations, OEM authorization, OEM material-conformance certifications, cable samples and proposed test-report formats apply in full to this small cable-replacement project?
Answer 15: Yes, the Generic Division 27 requirements apply in full to this project.
Question 16: Please identify the fire-resistance rating and construction of the existing Building 3 OIT telecommunications-room penetration. Please provide the existing listed firestop-system number, if available, or confirm that the contractor may select a compatible listed system after verifying field conditions.
Answer 16: A 3M Fire Barrier Putty Sleeve Kit is currently installed and will remain in place. Any putty removed during cable installation must be replaced with 3M Fire Barrier Moldable Putty.
Installation must be repacked with 3M Fire Barrier Moldable Putty.
Question 17: Please confirm whether an independent third-party firestop inspector is required for the single telecommunications-room penetration. If required, please identify the required inspector qualifications, inspection report format, and required advance.
notice.
Answer 17: Inspection will be conducted by the Contracting Officer’s Representative (COR). An independent third-party firestop inspector is not required.
Question 9: Can Cat5 25-pair wire be used instead of Cat3 25-pair wire?
Answer 9: Category 3 (Cat 3) is the minimum acceptable standard. Higher categories are also acceptable, provided the wiring is copper twisted-pair rather than fiber optic or other types.
NOTE: This solicitation is an all-or-nothing request for proposal. Partial proposals will not be considered. All offerors must perform all aspects of the required work.
