About this file

This document is an Attachment containing Request for Information (RFI) Responses related to a VA Healthcare System construction project. The RFI responses address two specific questions: (1) permit details for the project, including NPDES Permit from KYDEP and Construction Permits from LFUCG, with associated fees estimated to be less than $1,000 each, and (2) clarification about Add Alternate 4 for Full Depth Pavement Replacement, with references to specific drawing sheets (CD104, CU304) and asphalt detail (CU501) for implementation. The document includes contact information for Terry Humphries from the KY Division of Water and provides technical guidance for project documentation and construction specifications.

80. Do you know how much this permit will cost or the name of the permit and who it is issued by?

Answer: NPDES Permit from KYDEP, no fees associated with obtaining coverage. Construction Permits for water and sewer from LFUCG. Link to fee schedule Wastewater Collection and Treatment Plant Construction Permits - Kentucky Energy and Environment Cabinet. Below is a contact at LFUCG to confirm the fees but they appear to be less than $1,000 each.

Terry Humphries, P.E.

Supervisor, Engineering Section Water Infrastructure Branch KY Division of Water 300 Sower Blvd, 3rd Floor Frankfort, KY 40601 (502)782-6983

81. Add Alternate 4 is for Full Depth Pavement Replacement. I could not find this anywhere in the drawings or the details. Can you please point this out to me?

Answer: Full depth pavement section is depicted on sheet CD104 and the new full depth pavement is on CU304. The asphalt detail 4 on CU501 should be used.
