About this file
This document contains questions and answers regarding a federal solicitation for installing miscellaneous high-tech, high-cost equipment in a cardiac catheterization lab at the Memphis VA Medical Center. The work includes general construction, alterations, mechanical, and electrical work to renovate approximately 1,560 square feet of space on the second floor over two phases. A site visit was held on June 17, 2022 and another is scheduled for August 17, 2022 to allow inspection of restricted areas. AutoCAD files and radiation shielding reports are available. Equipment, including UPS systems, will be provided by Philips as identified in drawings from July 2019. Past performance questionnaires may be submitted in lieu of the standard attachment. The estimated construction cost ranges from $1-2 million. Proposals are due in May 2022 in response to solicitation number 36C24922R0058 issued by the Department of Veterans Affairs for this 100% Service-Disabled Veteran-Owned Small Business set-aside project.

QUESTIONS AND ANSWERS
1. Will another site visit that allows access and inspection to the project limits be scheduled prior to the Phase 1 due date?
ANSWER – Access was provided for inspection on the 6/17/2022 pre-bid site visit. The only exception was for the Control Room and the G224 rooms due to an active case that was underway. Another site visit has been scheduled for 8/17/2022. The time has been coordinated so that the Control Room can be entered without an active case taking place. As
G224 is a sterile procedure room, it will only be viewed from the Control Room and not entered to prevent risking contamination. This is true as well immediately after a case and a terminal cleaning hasn’t taken place. Note the time for entering the Control Room is contingent on when the last case is completed. An early entering time is projected to be 3:00 p.m. Cases can go beyond this by 30 or more minutes. Meet at Building One (1) Room AEG22 by 2:45 p.m.
a. Considering the significate work required above the ceiling and below the floor of the project limits for this type of equipment, will access to the floor spaces above and below the project limits be allowed for inspection.
ANSWER – Access to the 1st floor spaces below the project 2nd floor was provided for inspection on the 6/17/2022 pre-bid site visit. There was no identified reason or request made to inspect the 3rd floor above. As the 3rd floor space above is currently an active ICU, access would have been denied.
2. Are site specific final equipment drawings or preliminary layouts available for BOTH Cath Lab Equipment Packages purchased by the VA for the two affected Cath Labs?
ANSWER – Per the Statement of Work, there is the Philips drawing package dated 7/12/2019 for the G221 rooms. There is no Cath Lab Equipment Package for the G224 rooms as the work in those rooms is limited to what is detailed.
a. Will partial or full Cath Lab equipment UPS systems be purchase with the equipment (or otherwise required for the new project) to protect the Cath Labs during emergency power switchover or poor Memphis or facility power quality that does not meet OEM power quality specifications?
ANSWER – Per the Philips drawing package dated 7/12/2019, the Equipment List identifies
UPS and battery cabinet as furnished by Philips.
3. Are scaled .pdf floor plans (i.e., Life Safety) available of;
a. The project floors
b. Floor above project floor
c. Floor below project floor
d. Are AutoCAD files of these floor plans available?
ANSWER – AutoCAD files are available for the Life Safety floor plans.
4. Are Radiological Physicist Radiation Shield Designs or Reports available for the affected Cath Lab Suites?
ANSWER – The radiation shielding placard for G221 states:
SURFACES OF THIS ROOM HAVE BEEN INSULATED
WITH SHEET LEAD OF THE FOLLOWING THICKNESSES
TO A HEIGHT OF 7 FEET ABOVE FLOOR SLAB
ROOM G221
NORTH WALL 3/64” LEAD TO 7’-0”
SOUTH WALL 1/32” LEAD TO 7’-0”
EAST WALL 1/32” LEAD TO 7’-0”
WEST WALL 22 FT OF 3/64” LEAD TO 7’-0”
5. Will arc flash studies be required if a panel is altered in any way?
ANSWER – Yes.
6. Past performance questionnaire, page 1 of 6, states “The Offeror must provide this entire document to each of its assessors.” If CPARS is not available, are Offerors permitted to submit previously completed PPQ-0 forms instead of Attachment 22 – Past performance?
ANSWER - Yes previously completed PPQ forms are acceptable.
