B.3 PERFORMANCE WORK STATEMENT
Asbestos Survey Services
1. Introduction:
This is a non-personal service contract for the procurement of asbestos survey services. The Government shall not exercise any supervision or control over the contractor service providers performing the services herein. Such service providers shall be accountable solely to the contractor, who is responsible to the Government. The contractor shall provide all the labor, materials, equipment, and transportation necessary to conduct the survey, collect samples, perform lab analysis, and produce the required deliverables.
The Orlando VA Healthcare System (OVAHCS) Safety Department, located at 13800 Veterans Way, Orlando, FL 32827, is responsible for the Health, Safety, and Well-Being of the Staff, Patients, and Veterans at the OVAHCS hospitals and Outpatient Clinics.
2. Background:
Federal regulations for asbestos control have been established by the Occupational Safety and Health Administration (OSHA) and the Environmental Protection Agency (EPA). These regulations are thorough in that they inspect the most accessible functional spaces and observe bulk samples taken of suspect materials. The last Baseline Assessment survey report (See Attachment D.1 and D.2) for the Orlando VA Medical Center located at Lake Baldwin was conducted in 2013. VHA Directive 7714 of 6 April 2017, Asbestos Management Program, calls for a survey following requirements of the Asbestos Hazard Emergency Response Act (AHERA), known as a baseline survey (previously called triennial survey).
The Baseline AHERA Survey is unobtrusive in that samples are not taken where doing so would result in objectionable surface damage or where institutional barriers preclude access. The Asbestos Baseline Survey and Facility Asbestos O&M Plan will be updated when the VISN Occupational Safety and Health Manager or VISN Industrial Hygienist determines that abatement/ removal updates to the baseline have been inadequate during Annual Workplace Evaluations (AWE) (formerly triennial).
3. Objective:
The principal objective is to survey all buildings listed herein. Collecting representative samples of suspect asbestos-containing materials (ACMs), assessing their condition, and quantifying the materials and hazards present throughout the buildings. Provide a Re-Baseline Asbestos Assessment Survey Report and an Operations and Maintenance (O&M) plan for the place of performance herein.
4. Place of Performance:
a. Orlando VA Medical Center, Lake Baldwin VA Clinic. 5201 Raymond St, Orlando, FL 32083.
b. Orlando VA Medical Center, Lakemont Campus. 2500 S Lakemont Ave, Orlando, FL 32814.
