Re: Orlando Veterans Administration - Lakemont Campus 3rd Floor Asbestos Inspection &
Assessment Report. Air Analytics Project No. 10-10124.
Dear Mrs. Escobales:
Air Analytics (AA) is pleased to submit this asbestos survey report conducted at the Orlando Veterans
Administration Lakemont Campus, located at 2500 S. Lakemont Ave., Winter Park, Florida 32814.
Edward A. Nunez, a Florida Licensed Asbestos Consultant (LAC AX0000048) and Asbestos Inspector
(Certification number AA101409IR04), completed the field investigation conducted on May 13, 2010, encompassing the physical assessment, sampling and analysis for suspect Asbestos Containing Building
Materials (ACBM) at the subject facility.
Twenty samples were collected from nine distinct homogenous sampling areas. These samples yielded twenty-six distinct materials, when each layer was identified by the laboratory, in accordance with EPA requirements. Laboratory results determined that none of the materials were found to be asbestos-containing. Therefore, no action is required.
All of the information collected as a result of this survey will be treated with the utmost confidentiality and will not be released to any unauthorized person(s) for any reason without your prior written consent.
Air Analytics appreciates the opportunity to be of service to you on this project and looks forward to our continued relationship. If you should have any questions concerning the content of this report, please do not hesitate to contact us.
Very truly yours, Edward A. Nuñez, CIH, LAC

1.0 INTRODUCTION & BACKGROUND
An asbestos-containing building materials inspection & physical assessment survey was conducted at the Orlando Veterans Administration Lakemont Campus (third floor only), located at 2500 S. Lakemont Ave., Winter Park, Florida 32814. The evaluation was performed on May
13, 2010. Air Analytics’ Edward A. Nunez, a Florida Licensed Asbestos Consultant (LAC
AX0000048) and Asbestos Inspector (Certification number AA101409IR04), conducted the subject evaluation.
Briefly, the services included:
• Inspection and sample collection for suspected asbestos-containing building materials, by an
EPA-accredited asbestos surveyor according to federal guidelines, to provide information on material condition, approximate locations, and estimated quantities of ACBM, according to a generally representative sampling protocol.
• Analysis of all bulk samples utilizing Polarized Light Microscopy and Dispersion Staining
Techniques performed in accordance with EPA Bulk Analysis Method EPA 600/R-93/116, incorporating visual estimates of identified material percentages.
• Compilation of a report with collected samples, assessment forms, and photographic documentation.
• Data Evaluation And Interpretation
• Final Report
2.0 METHODOLOGY
The asbestos inspection was limited to the third floor of the Orlando Veterans Administration -
Lakemont Campus facility. The asbestos facility inspection was conducted in general accordance with AHERA guidelines, by identifying Homogeneous Sampling Areas (HSA), and collecting a minimum number of samples required by the standard. A homogeneous sampling area can be described as any suspect asbestos material that is similar in appearance and texture, having the same installation date, use, and function. Sample collection depends on the category that the
HSA falls into and the professional judgment of the field inspector/surveyor.
If the analytical results indicated that all the samples collected per HSA did not contain asbestos, then the HSA (material) would be considered a non-ACM. However, if the analytical results indicate one or more of the samples collected per HSA contains asbestos in quantities of greater than 1 percent asbestos by weight (as defined by EPA), all of the HSA (material) shall be treated as an ACM Material.
Friable materials and Category I and II non-friable materials, which have become friable are classified by EPA NESHAPs (40 CFR 61 M) as Regulated Asbestos Containing Materials
Orlando Veterans Administration - Lakemont Campus Page 2 of 4
(RACM), and must be removed by a State of Florida-licensed asbestos abatement contractor prior to building demolition. Category I non-friable ACMs shall not be subjected to abrasion, grinding, sanding or any other processes during demolition, which will render these non-friable materials friable. A ten-day notification must be submitted to the FDEP prior to the initiation of the abatement of more than 160 square feet or 260 linear feet of an RACM. Air monitoring to ensure that other areas of the building are not being contaminated with asbestos fibers should be performed by a licensed asbestos business organization during the abatement project. Final clearance air sampling should be performed to document that the consensus clean air standard of
0.01 fibers per cubic centimeter of air has been met.
3.0 RESULTS
A thorough walkthrough of the facility’s third floor was conducted, to identify suspect ACBM materials prior to sampling. Nine homogeneous sampling areas were identified, and twenty samples were collected. These samples yielded twenty-six distinct materials, when each layer was identified by the laboratory, in accordance with EPA requirements.

NAD – No Asbestos Detected
Laboratory results determined that none of the materials were found to be asbestos-containing.
Therefore, no action is required.
All ACM physical assessments were documented in the “Asbestos Building Survey Form” found in Appendix A of this report. Photographic documentation is included in Appendix B.
This report is designed to aid the building owner, construction manager, general contractors, and potential asbestos abatement contractors in identifying and locating ACM. Under NO circumstances is this report to be utilized as a bidding document or as a project specification document, as it does not have any of the components required to serve as an Asbestos Project
Design document or an Abatement Work plan.
