The purpose of this modification is to:

1. Corrected PWS associated with the requirement in results to questions posed by Vendors to address Certificate of Public Necessity (COPCN) requirement on page 10 section XIV,

14.1 and 14.4. The COPCN is not required for this requirement; However, Hillsborough

County certification and all vehicle permits in accordance with the Hillsborough County-

Vehicle for Hire, Handicab services is required with proposal submission.

2. Correct the evaluating Factors in RFR 36C24821R0088 page 106, 52.212-2 Evaluation and 108.

3. Extend Solicitation response time from 07/30/2021 1:00PM Eastern Standard Time to

08/04/2020 1:00PM Eastern Standard Time.

4. All Clauses, Provisions, required documents remains unchanged.

E.7 52.212-2 EVALUATION—COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the

Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

(d) The following factors will be used to evaluate offers (in descending order of importance):

1. Factor 1- Technical Capability: This factor shall be used to assess the Offeror’s ability to provide SMT services on short notice, and if unable to provide these services in-house have a list of subcontractors that can provide these services. Sub-factors shall include:

Government has in an offeror’s ability to meet the requirements of the solicitation, based on a demonstrated record of performance.

3. Factor 3 – Price: The offeror’s price proposal will be evaluated for award purposes by adding the total price for the base year and all option years.

Paragraph C.12, of this RFP. Please complete attached Limitation on Subcontracting and submit it with your proposal.

(g) Due to the critical nature of these services, the Offeror’s Technical Capability and Past

Performance, when combined, are considered significantly more important than getting the lowest price for the services.

E.12 BASIS FOR AWARD

1. Factor 1: Technical Capability: This factor shall be used to assess the Offeror’s ability to provide SMT services in accordance with the PWS, and if unable to provide these services in-house have a list of subcontractors that can provide these services. The contractor is required to provide safe and timely transportation of veterans to their appointments at MVAHS Medical facilities while meeting the VHA ICARE standard of providing outstanding customer service.

Offerors will be evaluated to the extent to which it provides the technical capability in the following sub-factors:

1) Sub-Factor 1 – Customer Services Performance.

i. Submit information to assess the type and style of Customer Service that the

Offeror is currently providing with their internal and external customers.

ii. Submit information how the Offeror shows how they would incorporate the

VHA ICARE “Integrity, Commitment, Advocate, Respect, Excellence” standards of adequate Customer Service to their service model of care.

2) Sub-Factor 2 – Certificate of Public Necessity (COPCN)

i. Submit copy of current Certification from the Department of Transportation

(DOT) COPCN for SMT services to our Veterans.

3) Sub-Factor 3 – Transition Plan.

i. Submit information that shows the Offeror’s ability to transition smoothly from the current contractor without any disruption in SMT services to our

Veterans. The Contractor shall, upon the Contracting Officer’s written notice,

(1) furnish phase-in, phase-out services for up to 90 days after this contract expires and (2) negotiate in good faith a plan with a successor to determine the nature and extent of phase-in, phase-out services required. The plan shall specify a training program and a date for transferring responsibilities for each division of work described in the plan and shall be subject to the Contracting

Officer’s approval. The Contractor shall provide sufficient experienced personnel during the phase-in, phase-out period to ensure that the services called for by this contract are maintained at the required level of proficiency.

4) Sub-Factor 4 – Performance Plan

i. Submit a performance plan that reflects an understanding of the scope of work and describe how that work will be accomplished.

ii. Submit information regarding the ability to manage grounds transportation contracts of similar size and scope.

iii. Describe your ability to respond to changes and increases in workload and the potential for simultaneous taskings across the coverage region.

iv. Describe contingency plan when schedules are disrupted altered and/or increased or decreased during the life of the contract.

5) Sub-Factor 5 – Quality Control Plan (QCP): Contractor will be required to provide data on how they will be able to meet and maintain VA standards during the duration of this contract.

i. Describe how your Quality Control Plan (QCP) will meet the Performance Work

Statement’s (PWS) requirements. Provide a copy of your company’s QCP as it relates to Grounds Transportation services. The QCP should address staff training plans, ability to meet the workload, especially the scheduling changes, methodologies for identifying and correcting changes to staff level and training, ability to provide adequate supervision, and ability to have appropriate and qualified staff.

ii. Describe how the QCP would address the verification of patient pickup and delivery information to ensure timely arrival for appointments, and the adequacy of invoicing in accordance with the Price Schedule.

iii. Describe how your vehicle inspection system is designed to reduce errors such as breakdowns, equipment failure, etc.

iv. Describe the company’s contingency plan in the event that the Contractor owned vehicles, equipment, etc. fails. The plan shall address temporary solutions to interruptions as well as methods to complete trips due to vehicle breakdowns.

6) Sub-Factor 5 – Vehicles and Equipment

i. Submit evidence of availability of vehicles and equipment specified in the

PWS, specifically the vehicles, equipment, supplies, and any other resources necessary to complete the task.

ii. Submit evidence of insurance in accordance with the PWS.

iii. Describe the specific vehicles and equipment to be used in transporting passengers with varying needs.

iv. Describe the plan for vehicle maintenance.

v. Provide copy of DOT inspected vehicle documentation to reflect date inspected and exportation dates of each inspected vehicle.

7) Sub-Factor 7 – Staffing: Contractor is responsible for ensuring all staff are properly trained, certifications are current and remain in good standing, and that staffing levels are maintained to provide VA standard of care throughout the duration of contract.

i. Provide current resumes of Offeror’s Key Personnel, including, but not limited to, Owner/Chief Executive Officer (CEO), Chief Financial Officer (CFO), Chief Operating Officer (COO), Transportation Coordinator, Primary

Administrator of this contract and the Supervisors that will be assigned to this project. Resumes should include staff’s skills, levels of knowledge in providing transportation services, experience in driving the various types of vehicles, customer service history, and experience in handling different types of patients, to include any de-escalation training.

2. Factor 3 – Past Performance: Past performance information will be obtained from references provided by the offeror and from other sources known to the Government or learned during the course of the evaluation. Each offeror will be evaluated on its performance for similar services.

The government will focus on information that demonstrates quality of performance relative to the size and complexity of the procurement.

1) In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance, in accordance with FAR 15.305 (a)(2)(iv).

Therefore, the offeror shall be determined to have unknown past performance.

2) In conducting the past performance evaluation, the Government reserves the right to use both the information provided by the offeror, and information obtained from other sources, such as the Past Performance Information Retrieval System (PPIRS) or similar systems and commercial sources.

3) Offerors shall provide completed Past Performance Surveys of at least 3 and not more than 5 recent (within the past 3 years) and relevant references (See Paragraph D.2).

Relevant means similar in size, scope, magnitude of effort and complexity to this requirement, previous history and completion of similar scale (size and scope) projects within the same time and cost estimates. The Contracting Officer will evaluate past performance to rate accordingly and may contact sources provided in the survey as necessary.

3. Factor 3 – Price: The offeror’s price proposal will be evaluated for award purposes by adding the total price for the base year and all option years. In evaluating the offeror’s proposed price for this requirement, the Government determination includes whether:

1) Proposed price is reasonable in comparison with industry standards and the

Government’s history of procurement.

2) Proposed price is reasonable in comparison with prices with the independent Government estimate.
