About this file
This document is a Task Order Closeout & Quality Certification template specifically designed for VA Medical Center (VAMC) electrical projects. The template provides a comprehensive checklist to ensure rigorous quality control and regulatory compliance for electrical work, with specific focus on meeting NFPA 99 (Health Care Facilities Code) and NFPA 70 (NEC) standards. The document covers critical areas such as task completion summary, safety and environmental reporting, and final contractor certification, requiring detailed verification of electrical system installation, testing, and compliance with healthcare facility electrical requirements. The checklist includes specific compliance checks like grounding and bonding resistance, receptacle testing, labeling of electrical panels and breakers, lockout/tagout procedures, panel clearance, GFCI testing, and firestopping of electrical penetrations. The template is intended to be used as a comprehensive quality assurance tool to document and verify the completion of electrical work in VA medical facilities before final payment.

Instructions for the Quality Assurance (QA) Walkthrough
To ensure a smooth hand-off to the VA Engineering Department, the contractor should have the following ready for the Government inspector:
1. Verification of Energized Work: If an Energized Electrical Work Permit (EEWP) was used, ensure the "Reason for Energized Work" section is signed off as closed.
2. ILSM Sign-off: If smoke detectors were capped or egress paths altered, provide proof that the Interim Life Safety Measures have been decommissioned and the area is returned to standard "Code" operation.
3. Digital Submission: If required by the VAMC, ensure a PDF copy of these test results is uploaded to the project folder (e.g., eCMS or Procore).
