EXISTING CONDITIONS NOTES:

1. The original site survey was prepared by Triad Engineering Inc., dated March 16, 2017 .

2. Supplemental site survey field work was completed by Triad Engineering Inc. in July and August

2017. Existing conditions shown hereon reflect expanded survey area prepared by Triad

Engineering Inc., and last updated August 4, 2017.

3. Existing above-grade and below-grade utilities have been shown in accordance with the best available information. It shall be the Contractor's responsibility to verify exact locations of existing utilities prior to commencing Work. Notify the COR and the appropriate utility provider immediately should discrepancies be found in mapping of underground utilities be encountered during earthwork operations. Record all existing utility locations verified in the field.

DEMOLITION PLAN NOTES:

1. The exact location and depth of existing utilities must be field-verified by the

Contractor prior to construction of proposed improvements, including, but not limited to, the location of all buried electrical lines.

2. Refer to Sheet C001 for Site Symbols and Abbreviations.

3. Refer to Electrical Drawings for power utility demolition or relocation work and exterior site lighting demolition, Sheet EP101.

4. All trees to be removed shall be tagged in the field prior to the start of construction; no construction work of any kind shall take place until the COR confirms, in writing, that the proper trees have been tagged.

5. Plant material to remain shall be protected from injury to their roots, trunks, and branches; maintain protection during the entire construction period.

6. Install tree protection fencing beyond drip line of existing trees to remain; stake fencing securely in place with posts set at 10'-0” max. Erect fencing prior to commencing work.

7. No material shall be stored or piled with 25'-0” of the trunks of any tree.

8. No gasoline, fuel oil, harmful chemical or material shall be stored within 150' of any tree to be preserved.

9. No vehicles shall be permitted in the tree protection area.

10. Repair all injuries to bark, trunk, limbs, and roots of remaining plants by properly dressing, cutting, tracing and painting, using approved arboricultural practices.

11. The General Contractor shall replace any tree larger than 6” dbh that dies due to construction activities during the period from start of construction to eighteen (18) months after the issuance of the certificate of occupancy for the building.

12. Remove all existing traffic signage within limits of work; store and reinstall or remove from site as directed by Owner, unless noted otherwise. Provide new signage as indicated.

13. Approximate limits of tree protection fence are also indicated on the site plan. The final location of the tree protection fence shall be coordinated in the field and approved by the COR.

14. The exact location and depth of existing utilities must be field-verified by the contractor prior to construction of proposed improvements.

15. The contractor shall be responsible for locating/verifying all buried utilities.

16. Coordinate all saw cutting of pavement and pavement removal with the proposed work. See Sheet C110 for approximate limits of new pavement.

17. Mill existing pavement to provide key between new and existing pavements.

18. Deliver all salvaged items to the Owner, unless intended to be re-installed in kind. Coordinate staging and material storage areas with the COR, and with the Erosion and Sedimentation Control Plan, Sheet C150.

19. Clear and grub only as needed. Perform no work that will cause damage to existing trees and their root systems to remain. Note that tree root systems generally mirror the tree canopy in diameter.

20. Remove existing lawn and strip topsoil only as required to perform the work.

Coordinate any removal or stripping work under canopy of existing trees to remain with professional care.

21. Haul and properly dispose of all extra soil in accordance with the construction waste disposal notes on the erosion and sedimentation control plans.

22. Do not store topsoil beneath the drop line of existing trees to remain. If additional topsoil storage space is required, coordinate with the owner and with the local conservation district.

23. Contractor to salvage any miscellaneous sign faces within limits of work that may interfere with the proposed work and deliver to the Owner. Contractor to remove and dispose of any related posts and foundations.

2. Contours shall be used as grade points where no spot elevations are shown.

3. Identify and clearly mark benchmarks. Verify and record benchmark elevations prior to commencing work. Protect benchmarks and survey control points from damage or displacement. Reestablish or relocate benchmarks disturbed by site work operations at the Contractor's expense.

4. “Benchmark” - See Sheet C100. All elevations shown are based on benchmark.

5. Existing above-grade and below-grade utilities have been shown in accordance with the best available information. It shall be the Contractor's responsibility to verify exact locations of all existing utilities prior to commencing work. Notify the COR and the appropriate utility provider immediately should discrepancies be found in mapping of underground utilities be encountered during earthwork operations. Record all existing utility locations verified in the field.

6. Refer to Sheet C510 for Erosion and Sedimentation Control Plan and permitted Limit of Disturbance.

7. The Contractor shall strip topsoil from all areas to be regraded and/or disturbed. Coordinate limits of disturbance and topsoil stockpile locations with all other prime contractors.

8. Verify suitability of subsoil materials. All fill materials are subject to testing and inspection.

9. Provide positive drainage away from all buildings and structures. Avoid the development of isolated low spots to prevent freestanding surface water.

10.All walks shall have a cross pitch of 2% in the direction of the slope or to the curb.

11.High and low points on walks and curbs shall be rounded off to a distance of at least five feet to show no abrupt change of grade.

12.The tops and bottoms of all slopes shall be rounded off to smooth curves five feet long.

13.Make new grade changes gradual; blend slope into level areas. Provide smooth transitions between new and existing grades.

14.Finish grade behind curb shall be flush with the top of the curb, unless otherwise noted.

15.Where mulched planting areas abut curb, finish soil grade behind curb shall be maintained three-inches below top of curb to allow for placement of mulch.

16.Remove stockpile areas, leave area in clean and neat condition; grade site surface to meet the proposed grades and to prevent freestanding surface water.

17.The Contractor shall preserve existing vegetation where possible and/or as noted on the drawings.

18.The Contractor is responsible for the removal and disposal of all rubbish, trash, debris and organic material in a lawful manner.

19.Verify required spot elevations/grading in the vicinity of the building with the architectural plans.

20. Where new construction abuts existing improvements, verify the elevations and grades of the existing improvements. Notify the Architect of discrepancies between the survey and existing field conditions prior to proceeding with new work.

21. Where utility trenching occurs, restore finished grades to match existing conditions unless otherwise indicated.

22. See lighting plan for locations of proposed site lighting and related electric utility service.

23. If a sinkhole is encountered during construction, contact a Geotechnical Engineer licensed in the State of West Virginia for a Repair Plan.

CLEAN FILL AND ENVIRONMENTAL DUE-DILIGENCE

This erosion and sedimentation pollution control plan covers the “moving, depositing, stockpiling, or storing of soil rock or earth materials”. If this project will need fill imported from an off site location, the responsibility for performing environmental due diligence and the determination of clean fill will reside with the Contractor. Environmental due diligence is defined as investigative techniques, including, but not limited to visual property inspections, electronic data base searches, review of property ownership, review of property use history, Sanborn maps, environmental questionnaires, transaction screens, analytical testing, environmental assessments or audits. Analytical testing is not a required part of due diligence unless visual inspection and/or review of the past land use of the property indicates that the fill may have been subjected to a spill or release of regulated substance. If the fill may have been affected by a spill or release of a regulated substance, it must be tested to determine if it qualifies as clean fill. Testing should be performed in accordance with WV DEP's standards. Clean fill is defined as uncontaminated, non-water soluble, non-decomposable, inter, solid material. The term includes soil, rock, stone, dredged material, used asphalt, use brick, block or concrete from construction and demolition activities that is separate from other waste and is recognizable as such. The term does not include materials placed in or on the Waters of the State unless otherwise authorized. (The term “used asphalt” does not include milled asphalt or asphalt that has been processed for re-use.) Fill material that does not qualify as clean fill will be considered regulated fill. Regulated fill is waste and must be managed in accordance with the department's Solid Waste Program. If applicable, fill determination is not required by the contractor unless there is evidence that a spill or release of a regulated substance has occurred.

CONSTRUCTION WASTE DISPOSAL

Individual responsible for earth disturbance activities must ensure that proper mechanisms are in place to control waste materials. Construction wastes include, but are not limited to, excess soil materials, building materials, concrete wash water, sanitary wastes, etc. that could adversely impact water quality. Measures should be planned and implemented for housekeeping material management, and litter control. Bulk construction waste must be disposed of off-site, at a permitted location (I.E. Landfill). Excess fill material must be disposed of in an area that has an approved E&S Plan associated with

it. Concrete wash water and sanitary wastes may not be discharged to a storm sewer or permitted to flow overland to receiving waters. Wherever possible, recycling of excess materials is preferred, over disposal.

EROSION & SEDIMENT CONTROL NOTES:

1. In accordance with chapter 24-C underground facility damage prevention act, as amended, the Contractor must call the following phone number and notify all affected utility companies of the intention to excavate no less than 48 hours prior to excavation activities West Virginia 811, INC (WV 811) one call center: 1-800-245-4848 or 8911.

2. The exact location and depth of existing utilities must be field-verified by the contractor prior to construction of proposed improvements.

3. Provide transition from 6” curb height to flush condition in a 6'-0” horizontal length.

4. See lighting plan for locations of proposed site lighting and related electric utility service.

5. See Sheet C550 for additional notes and details.'

6. If a sinkhole is encountered during construction, contact a Geotechnical Engineer licensed in the State of West Virginia for a Repair Plan.

NPDES PERMIT:

The contractor is responsible for obtaining a General WV/NPDES Water Pollution Control Permit for stormwater associated with construction activities prior to commencing any earth disturbances.
