About this file

The document is a two-page procedural guide for the REACH VET (Recovery Engagement And Coordination for Health - Veterans Enhanced Treatment) program, detailing a six-step process for healthcare providers serving high-risk veterans. The steps include receiving notification about a veteran, optionally accessing a clinical dashboard, re-evaluating the veteran's care comprehensively, considering treatment enhancement strategies, conducting outreach to the veteran using specific talking points, and documenting interactions in the CPRS (Computerized Patient Record System).

The guide provides detailed instructions for each step, emphasizing comprehensive care review, enhanced communication strategies, safety planning, and collaborative treatment modifications. Providers are required to complete all tasks by the fourth Wednesday of the month and must initiate outreach to identified veterans within one week of notification. The document is an attachment (D.18) to Request for Proposal (RFP) 36C24225R0047 for the Samuel S. Stratton VA Medical Center in Albany, NY, which is seeking Community Based Outpatient Clinic (CBOC) services in Kingston, NY.
