About this file

This document is an Amendment (0002) to a Department of Veterans Affairs (VA) solicitation (36C24225R0047) for Community Based Outpatient Clinic (CBOC) services in Kingston, NY. The amendment provides responses to 18 vendor questions covering various aspects of the procurement, including key clarifications about proposal submission, facility requirements, and service expectations. Significant details include: the incumbent contractor is Valor Healthcare, Inc. (contract 36C24220D0028), proposers must submit separate documents for technical, past performance, SDVOSB/VOSB status, and price proposals, and the contingency plan should be included in Part III. The proposal due date has been extended to September 25, 2025 by 16:30 ET.

Key service requirements include providing a facility with a Kingston, NY address, submitting a facility floor plan and Primary Care Provider licenses with the proposal, and providing a Clinical Pharmacy Provider (CPP) as Key Personnel. The VA clarified that the contractor is responsible for Clinical Pharmacy Services, previously unclear in the Performance Work Statement. The solicitation emphasizes opportunities for Service-Disabled Veteran-Owned Small Businesses (SDVOSB) and Veteran-Owned Small Businesses (VOSB), though specific evaluation scoring methods were not disclosed. Additional noteworthy points include VA handling Suboxone injections, with no storage requirements for the contractor, and currently no expected patient volume for Substance Use Disorder (SUD) nurse services.

Question Number

Question/Answer

1 Question: The answer to question 25 stated, yes, a contingency plan should be included with the proposal and submitted in Part IV, Miscellaneous. Question 29 asked, will the VA please confirm if Part I - Technical Proposal, Part II -Past Performance, Part III SDVOSB/VOSB Status & Miscellaneous and Part IV - Price Proposal should all be in separate documents/files for a total of four separate documents? And the answer was yes; each part should be in their own separate file.

It is not clear which volume the Miscellaneous information should be located. Should the Miscellaneous information be provided in Part III SDVOSB/VOSB Status, or in Part IV Price Proposal, or in a separate volume and add Part V Miscellaneous?

Also, can the Government confirm that they are only asking for a Contingency Plan for computer downtime and not additional items?

Answer: This is a mistake. The Contingency Plan should be included in Part III, not Part IV.

2 Question: Also, can the Government confirm that they are only asking for a Contingency Plan for computer downtime and not additional items?

Answer: The contingency plan referenced (above) in Question 1, which is to be submitted in Part III, specifically refers to a computer downtime plan.

3 Question: With there being no suitable locations in the city limits of Kingston, will the VA consider expanding the area to include sites with a Kingston, NY address?

Answer: Yes, sites with a Kingston, NY address will be considered.

4 Question: In Attachment D.5, when the Government asks for the number of hours available per week for Key Personnel, does the Government want actual scheduled hours or the full availability of Key Personnel?

Answer: In Attachment D.5, the Government is looking for the FTE level (number of hours available per week) of the proposed Key Personnel. For example, if the Key Personnel is being proposed as a 0.5 FTE, they would work 20 hours per week.

5 Question: Is it acceptable to provide the facility floorplan within 15-21 days post award as well as the PCP license etc.

Answer: No, the facility floor plan must be submitted with the proposal and will be part of the technical evaluation. The Primary Care Provider licenses should be also submitted with the proposal.

6 Question: Who is the current provider of these services and their contract number?

Answer: The incumbent contractor is Valor Healthcare, Inc. The contract number is 36C24220D0028.

7 Question: Are multiple Price Schedules acceptable based on a. new build out or b. Upgrades to existing facility?

Answer: Each alternate proposal should be submitted independently and fully address the solicitation's requirements. Multiple Price Schedules relating to different geographic locations should not be submitted within the same proposal, they should be submitted independently as their own proposal.

Each alternate proposal will be independently evaluated by the source selection evaluation board as a stand-alone proposal.

8 Question: Please can you tell me the things needed for outpatient care center for the contract

Answer: Please refer to the solicitation and the attachments for detailed specifications and information about what is necessary. VA is unable to address your question without more detail.

9 Question: Is the contractor responsible for providing anticoagulation (anti-coag) pharmacy services?

-If so, what is the required FTE (Full-Time Equivalent) ratio per number of veterans served for anticoagulation services?

-Additionally, could you please provide the estimated number of veterans who will be receiving anticoagulation services under this contract?

Answer: No. The contractor is not responsible for providing anticoagulation pharmacy services.

10 Question: Please confirm that a certified Service-Disabled Veteran-Owned

Small Business (SDVOSB) will receive more evaluation credit than a large business that proposes to subcontract with an SDVOSB or Veteran-Owned Small Business (VOSB).

Answer: The CO cannot disclose detailed internal scoring methods or weights related to the evaluation ranking system. As a certified SDVOSB or VOSB prime contractor, the inherent commitment to Veterans is recognized and valued in the evaluation.

11 Question: What percentage of the estimated overall contract spend must a large business subcontract to an SDVOSB or VOSB to receive evaluation credit? Is the credit tiered based on the percentage of SDVOSB/VOSB subcontracting spend?

Answer: The CO cannot disclose detailed internal scoring methods or weights related to the evaluation ranking system. While specific percentages of subcontracting to SDVOSBs or VOSBs may not be explicitly stated in the evaluation factors of the solicitation, it is essential to understand that the VA places a high priority on subcontracting with these categories of small businesses. The intent is to maximize opportunities for SDVOSBs and VOSBs while fostering their growth and participation in federal contracting.

Contractors should refer to FAR 52.219-9 and FAR 52.219-8 for more information about subcontracting plan requirements. Subcontracting plans are reviewed by Small Business Liaisons to confirm that the plan meets VA and regulatory requirements.

12 Question: Please describe the process the VA uses to ensure that, once awarded, a large business follows through on its proposed subcontracting commitments to SDVOSBs and VOSBs.

Answer: VA relies on established reporting systems, such as the Electronic Subcontracting Reporting System (eSRS), for contractors to submit required subcontracting performance data. Throughout the contract performance period, the large business must submit periodic subcontracting reports.

These reports provide details on the subcontracting activities and the extent to which the commitments to SDVOSBs and VOSBs are being met.

13 Question: Injection Administration

- Will the VA be providing the injections for the SUD Nurse?

- Please confirm that these injections will not remain on site at the clinic.

Answer: VA provides injections for the VA SUD Nurse. The medication will not be stored at the clinic.

14 Question: Patient Enrollment and Volume

- Are SUD Nurse patients considered enrollees at the clinic?

- How many patients will the SUD Nurse see while in clinic?

- How many veterans are expected to come to the clinic for injections on a monthly basis?

Answer: SUD Injections at the Kingston CBOC would be arranged based on patient needs, this is not something provided regularly at any of the CBOCs in the Albany Stratton VAMC region. Patients seen by the SUD Nurse would be members of the clinic, unless special arrangements are made by VA to accommodate a Veteran that is not a Kingston CBOC enrollee. The current estimated number of patients expected would be 0.

15 Question: Suboxone Logistics and Security

- Will the VA bring the Suboxone with them for injection?

- Will there be any requirement to receive or store Suboxone on site?

- Will VA personnel bring VA security during administration times?

Answer: Yes, VA will bring Suboxone when it is required. There is no requirement for the contractor to receive or store Suboxone on site. VA personnel would not bring VA security with them to administer the medication, unless required based on the situation.

16 Question: Administration Schedule

- Please verify the number of days and hours Suboxone will be administered at the clinic.

Answer: Currently, there is not a schedule. A schedule would be developed if patient need arises.

17 Question: Page 54 of the revised PWS states, " Clinical Pharmacy Services:

Clinical Pharmacy Services will be provided by the Samuel S. Stratton VAMC via telehealth. The Contractor shall provide scheduling support services." We believe this is in conflict with the response to Question 17 of the QA responses. It is our assumption that this was overlooked during the revision process and that the Contractor is responsible for Clinical Pharmacy services provided by a CPP.

Please advise if it is correct to assume that the Contractor must provide the Key Personnel position CPP.

As the addition of a CPP impacts cost, a response prior to proposal due date would be greatly appreciated.

Answer: The contractor must provide the CPP Key Personnel. The revision to the PWS was to change the responsibility from the VA to the Contractor.

The section referenced on Page 54 was erroneously missed.

18 Question: Will the government extend the due date by two weeks?

Answer: The due date will be extended as a result of this Amendment.
