Attachment D - Frame Kit VISN 10_12 and 20 _ Rev1.1.xlsx

This is a Frame Kit usage attachment for a federal prescription eyeglasses manufacturing contract covering Veterans Integrated Service Networks (VISN) 10, 12, and 20. The document provides detailed historical usage data for eyeglass frames across fiscal years 2024 and 2025, organized by product category (Men's Metal, Men's Zyle, Unisex Metal, Unisex Zyle, Women's Metal, and Women's Zyle) and further subdivided by manufacturer, model/style, color, and size. The data includes specific usage quantities for established frames as well as newly introduced frames in FY25, enabling the government to establish realistic demand forecasts and pricing baselines for the contract period.

The frame inventory spans numerous manufacturers and styles, with usage quantities ranging from single-digit orders for new or specialty items to over 5,000 units for high-demand products such as the Men's Metal Capri Modern Bo in Matte Gun Metal (4,626 FY25 usage). The attachment serves as critical sourcing data for vendors responding to solicitation 36C24126R0001, a VA-wide prescription eyeglasses manufacturing contract set-aside for service-disabled veteran-owned small businesses (SDVOSBs), allowing offerors to structure proposals and pricing based on documented historical consumption patterns across multiple VA sites and their catchment areas.
