For each project selected, Offerors shall summarize the following information, not to exceed two (2) pages for each project (total maximum page limit – 10 pages):
· Describe the project team composition and your firm’s role (AE Firm, General Contractor, and key sub-contractors involved).

· Describe the cost and schedule performance including explanations for any cost and/or time growth experienced.
· Include the project’s safety record (i.e., number of lost time accidents).
· Provide the Owner's name, and POC information including a current phone number.
Past Performance Questionnaires (no page limit):
For each of the three (3) projects identified above, if CPARS evaluations are available, the Offeror shall provide a summary listing (table format) of interim and final past performance evaluations for those projects. Using CPARS, the Government will download the identified CPARS assessments for review and analysis.
All offerors must follow the process outlined below for all recent and relevant projects identified above that do not have CPARS evaluations available:
The Offeror shall initiate Past Performance Questionnaires, or PPQs, by completing blocks 1 through 7 on the attached form. The Offeror shall request that the owner’s representative most knowledgeable of the project complete block 8 on the form and email the PPQs directly to the Contracting Officer identified on the form no later than the proposal due date/time identified on Page 1 of form SF 1442.
In case PPQs are not received by the Contracting Officer prior to the proposal submission due date, the Offeror shall provide a listing of all owner references to whom PPQs were sent including name, email address, and phone number. Offerors are responsible for ensuring that the contact information provided for the owner’s representative indicated for each relevant project is accurate and that the representative is aware that the Contracting Officer may be contacting them regarding the questionnaire and the Offeror’s past performance. If completed PPQs are not received timely by the Contracting Officer from the owner’s representative, the Government reserves the right to contact the references directly or obtain the requested information from any other sources.
Note: Multi-division or regionalized firms should submit projects performed by the division/business segment that would, if selected, perform the construction on this contract. Projects performed by other divisions/business segments may be submitted but may be considered less relevant than projects performed by the local division/business segment proposing on this project.
Evaluation Methodology:
Past performance evaluation results in an assessment of the offeror’s probability of meeting the solicitation requirements. It considers each offeror’s demonstrated recent and relevant record of performance in supplying products and services that meet the contract requirements. One performance confidence assessment rating and one performance relevancy rating will be assigned for each offeror after evaluating the offeror's recent past performance, focusing on performance that is relevant to the contract requirements. (see FAR 15.305). There are three aspects to the past performance evaluation:
Recency: Each contract will be assigned a rating of Recent or Not Recent.
Relevancy: Each project submitted for evaluation will be assigned a Relevancy Rating factor from the Past Performance Relevancy Rating Chart.
Quality: “Quality” refers to how well the contractor performed on the recent and relevant past contracts. This process gathers and evaluates information from customers on how well the offeror performed those past contracts. The TEB shall identify the summary performance rating for each contract and identify, in narrative format, all past performance information that would bolster or diminish the Government’s confidence in the offeror’s ability to perform work on this contract.
After evaluating the Recency, Relevancy, and Quality of each past performance contract identified above, the TEB will assign one overall, adjectival rating for the Past Performance factor for each Offeror. In the case of an Offeror without a record of past performance or for whom information on past performance is not available, the Offeror may not be evaluated favorably or unfavorably on past performance and will receive a rating of neutral. (Total page limit – 10, not including PPQs)
FACTOR 2 – RELEVANT EXPERIENCE
Sub-Factor 2.1, Relevant Team Experience Sub-factor 2.1 evaluates the relevant project experience of the Offeror, core team members, and major sub-contractors. Offerors should complete and submit summaries, a minimum of three and a maximum of 5 of recent and relevant projects. Offerors without projects meeting the recency and relevancy definitions described above may submit the requested information for their most recent and relevant projects regardless of value or size.
Multi-division or regionalized firms should submit projects performed by the division/business segment that would, if selected, perform the construction of this National Cemetery project. Projects performed by other divisions/business segments may be submitted but may be considered less relevant than projects performed by the local division/business segment proposing on this project.
· Describe the project in detail and highlight relevance of the project to the Cape Canaveral National Cemetery Phase 2 project.
· Describe the project team composition (construction firms involved).
· Describe the pricing and schedule performance including explanations for any price and/or time growth experienced.
· Include the project’s safety record (i.e., number of lost time accidents/days).
· Describe how sustainable design principles were incorporated into the project; indicate to what extent environmental program goals (e.g., Energy Star, Leadership in Energy and Environmental Design) were achieved.
· Provide project's Point of Contact (name, title, email, and phone number).
· Offerors shall summarize the above information not to exceed two (2) pages for each project. (Total page limit – 10) Sub-Factor 2.2, Key Personnel Sub-factor 2.2 evaluates the relevant experience of the Offeror's key personnel.
