1. CHANGES TO THE SOLICITATION
· General Requirements 01 00 01, page 7, Section G(1)(a)(i) is hereby changed to read as follows:
ETO 1: To be completed within 300 days of Notice to Proceed. New Burial Sections 33 and 34 (360 and 600 burial sites respectively) each containing in ground cremains. This work is to construct and make ready for use by the Cemetery operations the in ground cremain fields, and adjacent sidewalk with all appurtenances.

Have any changes been made to the plans and specs between the time Amendment 3 was released and the present time other than the items detailed on page 1 of the Amendment 4 write- up? If yes, please summarize the changes if possible.
No changes have been made to the drawings of specifications since May of 2019.
RFI #2
We are interested in quoting the 13 chain link aluminum slide gates that were listed on the project to various fence contractors in the area. The drawing indicates that the slide gates are for 22’ openings and 8’ tall, powder coated, black. The legend on the plans says to look for “11-DT503”. I have been thru all of the site plans several times and have not been able to locate that particular number. Please advise as to where those gates are located and confirm the quantities.
11-DT503 is located in the upper right-hand corner of sheet DT503 and the proposed gate locations are shown on CS108.

As it was stated in the meeting prior to the site visit on 9-19-19 the selection of the prime contractor may not be made until late December 2019 or early January 2020. This being the case, with the ETO having a duration of 120 days, is it to be assumed that the VA will extend the duration of the ETO being that in all likelihood the NTP and project commencement will not take place prior to early spring?
This question was already answered. See Solicitation Amendment 0003 (attached), dated July 1, 2019, VA Response to RFI #1. Also see Section 1 of this Amendment 0005.
RFI #4
Regarding Area 8A Maintenance Complex, the asphalt path leading up to the area from the south, the parking lot to the Northeast, and a small piece of the horseshoe drive to the East are hashed out like they are to be Milled and Overlayed, but there isn’t a designation of that work taking place. Will we be milling and repaving these two areas?
The same can be said about the roadway piece shown adjacent to the new asphalt at Area 8B Soil Stockpile/Storage. Will that be milled and overlayed as well?
The westerly limits of Areas #1-#5 are as they are shown on the plans, correct?
It looks as though a large portion of the roadway leading up to Areas 8A and 8B will not be repaved. Is this correct? I only ask because the limits of disturbance lines make it seem as though the work will continue further than the site plans show.
There is a note in the top right hand corner that says to mill all on site existing roadways/parking lots. Is this only in the eight areas on the site plan or is it every roadway/parking lot in both the eight areas and outside of the eight areas?
The Mill and overlay limits are clearly shown on sheet CS100, which includes all existing asphalt on-site except within the fenced maintenance yard. The “eight areas” shown on the larger scale site plan sheets do not encompass all mill & overlay areas of the site, and contractor should refer to the hatch shown on CS100. A portion of the turnaround area within “Area 8B” on the site plan will be removed and replaced with full-depth asphalt as hatched on the site plan.

We would like to request that our MEL-ROL please be reviewed as an rolled, self-adhering waterproofing membrane material here. Please see the attached data sheets to assist with review, if you can please let me know if it’s acceptable it would be greatly appreciated.
Materials provided must meet the requirements of the design specifications; pre-approval of materials will not be provided.

Does the 12,000cy existing stockpile available for this contract in the back of the property meet the gradation and LL/PI requirements of this contract for “Fill”?
The stockpiled material available on site is suitable for use as fill. VA has not verified the volume of the stockpiled material pile but assumes that between 10,000 & 12,000 cy of material exists. Contractors to verify volume of stockpile and are responsible to provide any additional fill material required to complete the contract.

If we had Past Performance Questionnaires submitted prior to the proposal extension, will we need to have our references resubmit the questionnaires or can they be used from the first submission even though we are revising our initial proposal? (projects and POCs will remain the same)
Any Past Performance Questionnaires (PPQs) received by the Contracting Officer prior to September 3, 2019, will be considered when evaluating proposals received on or before the new proposal deadline. However, to clarify, proposal revisions will not be accepted. If you notified the Contracting Officer in writing of your desire to withdraw your proposal submitted on or before July 19, 2019, 1) it will be destroyed, 2) you are eligible to submit a new proposal. The new proposal must be a complete proposal, in accordance with the solicitation requirements. See Amendment 0004 language, issued September 3, 2019 & excerpted below:
a. Potential Offerors are advised that the deadline for receipt of proposals for this solicitation has been extended to October 18, 2019 at noon CT.
b. Offerors who submitted proposals on or before July 19, 2019:
i. Your proposal will be considered for award unless you notify the Contracting Officer of your intent withdraw your proposal from consideration for award, as follows:
ii. Notify the Contracting Officer, in writing (email jeffrey.gerbensky@va.gov is acceptable), if you wish to withdraw the proposal from consideration for award. If you withdraw the proposal, it will be destroyed. Offerors who withdraw their proposals are eligible to submit a new proposal on or before the proposal due date. You must notify the Contracting Officer of your intent to withdraw your proposal from consideration for award prior to the deadline for receipt of proposals. Only one proposal from each offeror may be considered for award.

Considering the various permits that will likely have to be pulled for this project, what sort or permits are foreseen to be necessary for the execution of this contract? (development, construction with local government agencies, etc.)
The project Architect of Record has already obtained the following permits which will be shared with the eventual awardee of this contract:
1. Michigan DEQ Part 303 Wetlands permit and Notice of Completion form
2. Oakland County Soil and Erosion permit It is our understanding that no additional permits are necessary, however it is the contractor’s responsibility to adhere to all of the requirements of the attached permits and any applicable laws and regulations. Contractor shall refer to the specifications and plans for any additional permitting requirements.

Regarding the end piers on the columbarium units, in lieu of having them be formed and poured concrete could there be consideration for the pier construction to be core filled, reinforced CMU?
This proposed, alternative, approach to constructing the columbaria end piers will not be considered at this time.

Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).
The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of ""identifiers"" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).
Union Rate Identifiers
A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers
Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.
Survey wage rates are not updated and remain in effect until a new survey is conducted.
Union Average Rate Identifiers
Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.
A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.
WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can be:
* an existing published wage determination
* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on a wage determination matter
* a conformance (additional classification and rate) ruling
On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. The classifications are listed in alphabetical order of ""identifiers"" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).
Union Rate Identifiers
A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers
Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.
A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.
