About this file

This is a Drug Free Workplace Certification form required by the Town of Palm Beach, Florida for the Bee Hives - Town Wide contract opportunity. The certification outlines mandatory compliance requirements that all grantees and their employees must meet to participate in Town contracts. The contractor must publish a workplace statement prohibiting the unlawful manufacture, distribution, dispensing, possession, or use of controlled substances and must establish a drug-free awareness program educating employees about the dangers of drug abuse, the company's drug-free policy, available counseling and rehabilitation resources, and applicable penalties. All employees engaged in grant performance must receive a copy of the workplace prohibition statement and be notified that compliance is a condition of employment. Employees are required to notify their employer of any criminal drug statute convictions related to the workplace within five days of conviction, and employers must notify the Town within ten days of receiving such notice from employees or through other means.

Within thirty days of receiving notice of an employee conviction, the Town contractor must take appropriate personnel action, up to and including termination, or require the employee to participate in an approved drug abuse assistance or rehabilitation program. The contractor, through an authorized agent, must certify full compliance with all seven requirements and maintain a good faith effort to continue operating a drug-free workplace. The certification form requires the firm name, authorized agent signature, date, title, email, and phone number. This documentation is a mandatory prerequisite for bid submission and contract award for the five-year beehives installation, maintenance, and honey harvesting project at the Par 3 Golf Course.
