About this file
The document is a Certification of Legal Work Status form from the City of St. George, Utah, specifically addressing legal employment verification requirements for contractors. The form requires contractors to certify that they will not knowingly employ workers in violation of 8 USC § 1324a, and mandates that all subcontractors must also sign similar certifications before performing any work on the project.
The document outlines specific compliance requirements, including the use of E-Verify to verify employment eligibility of all employees. Contractors who violate these requirements may face serious consequences, such as unilateral contract cancellation and potential suspension from future city projects for one year. Additionally, the contractor would be liable for all costs associated with contract termination, including damages and attorney fees. The form includes spaces for the contractor's name, address, E-Verification number, signature, and requires notarization to validate the certification.
