About this file

This is a URS Post-Retired Verification Form for Independent Contractors (1099 Employees) administered by Jordan School District in Utah. The form requires contractors to disclose whether they have retired from the Utah Retirement Systems (URS) and, if so, to provide their signature and date of completion. The form must be submitted to the Accounting Department along with a W-9. This verification requirement applies to any contractor who will be engaged by Jordan School District to provide train-the-trainer services for the Science of Reading Professional Development Program.

Contractors who are URS retirees receiving monthly benefits face restrictions on re-employment, which includes work performed as either a paid employee or independent contractor for a participating URS employer. Contractors must contact URS's Retirement Benefits Department at 801-366-7770 before beginning work with Jordan School District to complete Form RTRT-27A (Post-Retirement Employment form). Failure to report re-employment to URS in compliance with Utah Code section 49-11-1207(2) will result in the retiree's liability to repay URS any overpayments resulting from the violation.

(business).

If it is discovered that a URS retiree failed to report reemployment to URS on its Post-Retirement

Employment form (Form RTRT-27A) in violation of Title 49, the retiree will be liable to repay URS the amount of any overpayment resulting from the violation to report. See Utah code section 49-

11-1207(2).

You are directed to contact URS's Retirement Benefits Department at 801-366-7770 before beginning work with Jordan District. You will be asked to complete and file a
