About this file
This file is a Form W-9 (Request for Taxpayer Identification Number and Certification) issued by the Internal Revenue Service, accompanied by supplementary documentation from the City Colleges of Chicago regarding procurement requirements. The document serves as a tax identification form required from vendors and contractors doing business with the City Colleges of Chicago system in Illinois for the Thrive Strategy Evaluation contract opportunity. The form requires vendors to provide their correct taxpayer identification number (either a Social Security Number for individuals or an Employer Identification Number for entities) and certify their U.S. person status for federal tax reporting purposes. No specific project timeline, response dates, or contract award dates are detailed in this particular document.
The supplementary materials included with the W-9 form address the City Colleges of Chicago's Minority Business Enterprise (MBE) and Women Business Enterprise (WBE) participation requirements, ethics compliance obligations, and economic disclosure statement requirements that vendors must complete as part of their bid or proposal submission. Vendors are required to submit an executed W-9 form to the Procurement Office, comply with the district's ethics policy and annual ethics training requirements, and provide detailed implementation plans for MBE/WBE participation including Schedules A through D as applicable. All vendor and contract workers must sign an ethics acknowledgment and return it to the Procurement Office prior to contract execution. The document does not specify pricing terms, contract amounts, funding sources, or renewal options for the Thrive Strategy Evaluation opportunity.
