About this file
This is a Reference Questionnaire form used by Collier County, Florida's Procurement Services Division to collect performance information on firms and key personnel bidding on the County Signage project issued by the Parks & Recreation Division. The form functions as an evaluation tool for assessing contractor qualifications and past performance. Proposers are required to submit one completed questionnaire for each reference, with responses collected from previous clients who have engaged the bidding firm on similar work. The questionnaire must be fully completed and returned with the bid or proposal submittal to be considered in the selection process.
The reference evaluation employs a standardized 10-item scoring rubric on a scale of 1 to 10, with 10 indicating very satisfied and likely to rehire and 1 indicating very unsatisfied and would never rehire. Evaluated criteria include ability to manage project costs and maintain schedule, quality of work and consultative advice, professionalism and personnel management, project administration and documentation, communication skills, risk management, contract compliance, and overall customer satisfaction. References must provide project description, completion date, project budget, and project duration. Any blank responses are automatically scored as zero, incentivizing complete evaluations. The total score across all items is calculated to inform the County's contractor selection decision.
