About this file

This is an Insurance Requirements form issued by Collier County Board of County Commissioners, Florida, for the Biosolids Hauling Services contract (Solicitation 25-8403R). The document outlines mandatory insurance and bonding specifications that all proposers must maintain throughout the contract term. Key insurance requirements include Worker's Compensation coverage in accordance with Florida Statutes Chapter 440 and federal requirements, Commercial General Liability with minimum limits of $1,000,000 per occurrence and $2,000,000 aggregate, and Automobile Liability coverage of $1,000,000 per occurrence. Additional coverage types are specified as applicable to the work, including Watercraft, Maritime (Jones Act), Aircraft Liability ($5,000,000 minimum if applicable), and Umbrella/Excess coverage. Proposers must provide evidence of insurability within five days of award, and coverage must remain in force for a minimum of six months from the award date. All subcontractors must comply with identical insurance requirements.

Proposers must submit a Bid Bond equal to 5% of their cost proposal with their bid response. For contracts exceeding $200,000, Performance and Payment Bonds written for 100% of the contract award amount are required upon contract execution, with costs borne by the awarded bidder. Surety companies must be authorized to conduct business in Florida and rated "A-" or better by A.M. Best Company. Collier County must be named as an Additional Insured on the Commercial General Liability certificate, with the county's coverage being primary and non-contributory. All certificates of insurance must list Collier County Board of County Commissioners at 3295 Tamiami Trail East, Naples, FL 34112, include a 30-day cancellation notice clause, and reference the contract or project number. The county historically has spent approximately $3,700,000 annually for biosolids hauling services, though this may not reflect future purchasing patterns. The county will procure and maintain Builders Risk Insurance on construction projects where necessary, with premiums billed to the project rather than included in contractor proposals.
