About this file
The document is a Subconsultant/Subcontractor Utilization Worksheet issued by the City of St. Petersburg for the I-375 Surface Parking Lot Project (Project Number 20077-112). The worksheet provides detailed instructions for prime contractors regarding subcontractor participation, with up to 50% of the contract allowed to be subcontracted. The form requires comprehensive information about potential subconsultants, including their legal name, business address, contact information, trade classifications, and contract amounts.
The worksheet includes specific provisions for changing subconsultants, with seven defined scenarios permitting substitution, such as subcontractor failure to execute contracts, licensing issues, bankruptcy, or performance problems. The city has a particular focus on Small Business Enterprise (SBE) participation, requiring that any SBE subconsultants replaced must be substituted with another SBE subcontractor unless the prime contractor can demonstrate alternative SBE requirement fulfillment. The form is designed to ensure transparency, accountability, and compliance in subcontractor selection and utilization for municipal projects.
