About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in New Mexico for use in the Asset Management System for Fiber Optic Infrastructure solicitation. The form requires offerors and bidders to certify their eligibility and responsibility status prior to proposal submission. Bidders must confirm that neither they nor their principals are currently debarred, suspended, proposed for debarment, or voluntarily excluded from covered transactions by any federal, state, or local department or agency. The certification also requires disclosure of any convictions, civil judgments, indictments, or charges related to fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, falsification of records, or other offenses indicating lack of business integrity that occurred within three years preceding the certification date.
Offerors must further certify that they have not violated contract provisions through deliberate failure to perform in accordance with specifications or timelines, have not demonstrated preventable failure or unsatisfactory performance on prior contracts, have not violated ethical standards set forth in the County Procurement Code, and have not had any public contracts terminated for cause or default within the three-year period preceding certification. Additionally, bidders must confirm they are neither current nor former County employees and are not immediate family members of County employees or public officials. The form permits offerors unable to certify any statements to attach written explanations, though certification failures do not necessarily preclude consideration for award. Falsification of any statement in the form constitutes grounds for rejection of the proposal or rescission of any contract award.
