About this file

This is an Insurance Requirements document for the City and County of San Francisco, Department of Public Health, establishing mandatory coverage standards for contractors providing Community Outreach and Media Services under RFQ 0000011970. The contractor must maintain multiple insurance policies throughout the full contract term, including Commercial General Liability with limits of $1,000,000 each occurrence with Abuse and Molestation coverage, Commercial Automobile Liability at $1,000,000 combined single limit, Workers' Compensation at statutory amounts with Employers' Liability of $1,000,000 each accident, Professional Liability at $1,000,000 per claim, Technology Errors and Omissions Liability at $5,000,000 per claim, Cyber and Privacy Insurance at $20,000,000 per claim, and Pollution Liability Insurance at limits to be determined during risk assessment. The Commercial General Liability and Commercial Automobile Liability policies must name the City and County of San Francisco, its officers, agents, and employees as Additional Insureds with primary insurance provisions. All insurers must hold ratings of A-, VIII or higher and be authorized to conduct business in California. The contractor must provide certificates of insurance and endorsements prior to commencing work and must notify the City within thirty days (or ten days for non-payment) of any policy cancellation, non-renewal, or coverage reduction to the specified email address.

For claims-made policies, coverage must be maintained continuously throughout the contract term and for three years thereafter to ensure claims arising during the contract period are covered even if submitted after expiration. If any required insurance lapses, payment requests will not be processed until satisfactory evidence of reinstated coverage effective as of the lapse date is provided to the City, or the City may terminate the Agreement at its sole discretion. The Workers' Compensation policy must include a waiver of subrogation in favor of the City, and if subcontractors are utilized, they must obtain equivalent insurance and name both the City and the prime contractor as Additional Insureds. General annual aggregate limits must be double the occurrence limits where applicable. Approval of insurance by the City does not relieve or diminish the contractor's liability obligations under the Agreement.
