About this file
This is a Quality Assurance Surveillance Plan (QASP) for YCV-16 Maintenance and Repair under Department of the Navy Solicitation N6264926RA031. The QASP establishes a comprehensive framework for Government surveillance of all work performed on the vessel, ensuring full compliance with technical, safety, administrative, scheduling, environmental, and security requirements as defined in the Statement of Work (SOW). Surveillance activities cover towing, docking/undocking, maintenance, abrasive blasting, painting, cleaning, ventilation, welding/hot work, dry docking with certification and weight control, scaffolding, and installation of threaded fasteners. The Government will also monitor administrative functions including modification proposals, progress scheduling, integrated logistic support submittals, and hazardous waste management.
Government oversight is provided by the Contracting Officer, Contracting Officer's Representative/Program Manager, Marine Maintenance Officer, Government Docking Observer, and additional experts such as shipyard competent persons, environmental specialists, and security officers. Performance standards require 100% compliance with the contractor's quality assurance plan, greater than 95% of deliverables submitted on time without rework, and an overall Satisfactory rating or higher in the final evaluation. The Government will employ on-site inspections, customer feedback, daily/weekly spot checks, monthly formal progress meetings, key milestone inspections, and final project completion reviews. Contractor performance is rated as Satisfactory (requirements fully met or exceeded), Marginal (minor deficiencies with corrective action taken), or Unsatisfactory (major deficiencies adversely affecting contract execution). Performance documentation will be recorded in the Contractor Performance Assessment Report System (CPARS) and may support future contractual actions.

DEPARTMENT OF THE NAVY
Quality Assurance Surveillance Plan
(QASP)
For YCV-16 Maintenance and Repair
Solicitation: N6264926RA031 Attachment (VI)
1. Purpose
This QASP establishes a comprehensive framework for Government surveillance of all aspects of work performed on YCV-16. Its purposes are to:
Ensure full compliance with all technical, safety, administrative, scheduling, environmental, and security requirements as set forth in the work specifications defined in the Statement of Work (SOW).
Monitor and verify that technical operations—including towing, docking/undocking, maintenance, abrasive blasting, painting, cleaning, ventilation, welding/hot work, dry docking (with certification and weight control), scaffolding, and installation of threaded fasteners—are executed per the approved plans.
Confirm that administrative functions (such as modification proposals, progress scheduling, authorized correspondence, integrated logistic support submittals, and hazardous waste management) are maintained and reported as required.
Record and document performance and initiate corrective action as needed when proper notification to the requiring activity and contracting officer, when deficiencies are discovered.
Provide the Government with a consistent method to evaluate overall contract performance and ensure that all deliverables are submitted in a timely, quality manner.
2. Government Roles and Responsibilities
Contracting Officer (CO): Holds overall authority for contract administration. Ensures that all contractual requirements (technical, safety, schedule, insurance, and environmental) are met.
Contracting Officer’s Representative (COR)/Program Manager (PM): Conducts on-site inspections and document reviews. Collects and analyzes documentation (e.g., inspection checklists, photographs, test reports). Monitors progress schedule updates and initiates Corrective Action Requests (CARs) when necessary. Acts as the primary Government liaison.
Marine Maintenance Officer (MMO) & Government Docking Observer: Provide technical oversight during docking, undocking, and maintenance operations. Ensure vessel safety and protection of Navy interests during operations. Perform daily inspections, spot checks, and audits to verify work quality and compliance. Document all findings and assist in the CFR process.
Additional Experts (Shipyard Competent Persons, Environmental Specialists, and Security Officers): Provide specialized inspections and services in areas such as gas-free certification, confined space entry, hazardous waste management, and physical security.
3. Performance Standards
The performance and acceptable quality level standards are listed in Table 1. The Government shall use these standards to assess contractor performance.

4. Methods of Surveillance
The Government will monitor performance using the following methods:
On-site Inspections: The ACO and/or PM/COR will provide on-site oversight to assure the Contractor is adequately performing to the contract specifications
Customer Feedback: Customer feedback may be obtained either from the results of formal customer satisfaction surveys or from customer complaints. Customer complaints, to be considered valid, must set forth clearly and in writing the detailed nature of the complaint, must be signed and must be forwarded to the contractor. The contractor shall maintain a summary log of all formally received customer complaints as well as a copy of each complaint in a documentation file.
5. Evaluation Ratings
Contractor performance for each element will be rated as follows:
Satisfactory (S): Requirements are fully met or exceeded.
Marginal (M): Minor deficiencies exist; prompt corrective action has been taken.
Unsatisfactory (U): Major deficiencies exist; performance adversely affects overall contract execution.
Overall contract performance must achieve a Satisfactory rating (or higher) in the PM’s final report.
6. Documenting Performance
The Government will document positive and/or negative performance. Any report may become a part of the supporting documentation for any contractual action and preparing past performance information using the Contractor Performance Assessment Report System (CPARS).
7. Frequency of Measurement
Performance will be measured at the following intervals:
Daily/Weekly: Spot checks and on-site observations during active work phases.
Monthly: Formal progress meetings and safety/security briefings.
At Key Milestones: Inspections at specific work phase completions (e.g., after surface preparation, prior to dry docking, and at final inspection).
At Project Completion: Overall performance will be reviewed in the final evaluation.
As Needed: Additional assessments will occur when corrective actions or modifications are required.
NOTE: ANY ADDITIONAL QUALITY ASSURANCE OR QASP REQUIREMENTS AND RELATED
DETAILS ARE PROVIDED IN THE STATEMENT OF WORK (SOW), IN ACCORDANCE WITH THE
SOW. ANY SUPPLEMENTAL INFORMATION OR UPDATES ARE AVAILABLE THERE.
