About this file
This document is a Department of the Army permit verification issued by the U.S. Army Corps of Engineers, Sacramento District, to the City of St. George, Utah, for the George Washington Boulevard Bridge project located on the Virgin River near 1670 South Crosby Way in Washington County, Utah. The project involves construction of a new bridge crossing and road alignment across approximately 27 acres, requiring discharge of dredged and/or fill material into 3.9 acres of waters of the United States under Section 404 of the Clean Water Act. Specific authorized activities include construction of bridge abutments, support bents, riprap erosion control, temporary construction access, and temporary diversion channels. The project will result in permanent effects to 0.45 acre of perennial river and 0.05 acre of wetlands, with temporary effects to approximately 3.4 acres of perennial river and 0.53 acre of wetlands. The permit was issued on December 30, 2025, under Nationwide Permit Number 14 and is valid until March 14, 2026. The permittee must complete all construction activities within 12 months from the date the NWP is modified, reissued, or revoked. A compliance certification must be submitted within 30 days after completion of authorized work.
The permit includes mandatory compliance requirements with the U.S. Fish and Wildlife Service Biological Opinion (Number 2025-0033207) regarding incidental take of three endangered species: Southwestern willow flycatcher, Virgin River Chub, and Woundfin. The permittee must also comply with all terms and conditions of the December 8, 2025 Section 401 Water Quality Certification issued by the State of Utah. Special conditions require the permittee to clearly identify disturbance limits in the field with visible markers and maintain identification until construction completion and soil stabilization. The permit does not authorize take of endangered species without separate authorization under the Endangered Species Act. All contractors and workers must be made aware of and adhere to permit terms and conditions, with a copy of the verification and associated drawings required at the project site. Compliance inspections by U.S. Army Corps of Engineers representatives are subject to occur, with potential suspension, modification, or revocation of authorization for non-compliance.

DEPARTMENT OF THE ARMY
U.S. ARMY CORPS OF ENGINEERS, SACRAMENTO DISTRICT
1325 J STREET
SACRAMENTO CA 95814-2922
December 30, 2025
Regulatory Division (SPK-2023-00569)
City of St George Attn: Mr. Jay Sandberg 175 East 200 North St George, Utah 84770-3795 jay.sandberg@sgcity.org
Dear Mr. Sandberg:
We are responding to your May 27, 2025, pre-construction notification for a Department of the Army (DA) permit for the George Washington Boulevard Bridge project. The approximately 27-acre project site is located on the Virgin River, near 1670 South Crosby Way, Latitude 37.079176°, Longitude -113.575507°, St. George, Washington County, Utah (Enclosure 1).
Based on the information you provided to this office, the George Washington
Boulevard Bridge project involves the discharge of dredged and/or fill material into
3.9 acres of waters of the U.S. for the construction of a new bridge crossing and road alignment, subject to Section 404 of the Clean Water Act. The specific activities that require DA authorization are the construction of bridge abutments, support bents, riprap erosion control, temporary construction access, and temporary diversion channels.
These activities will result in permanent effects to 0.45 acre of perennial river and
0.05 acre of wetlands, including the permanent loss of 0.05 acre of wetlands and
0.04 acre of perennial river, and temporary effects to approximately 3.4 acres of perennial river and 0.53 acre of wetlands. The proposed activities would be conducted in accordance with the Impacts to Waters of the U.S. plans dated May 19, 2025 (Enclosure 2).
We have determined that activities in waters of the U.S. associated with the project are authorized by Nationwide Permit Number (NWP) 14. You must comply with all terms and conditions of the NWP and applicable regional conditions. Enclosed is information about the NWP terms and conditions and the Sacramento District Regional Conditions (Enclosure 3 and 4). In addition, your work must comply with the following special conditions:
1. This Corps permit does not authorize you to take an endangered species, in particular the Southwestern willow flycatcher (Empidonax traillii extimus), Virgin River Chub (Gila seminuda =robusta), or Woundfin (Plagopterus argentissimus). In order to legally take a listed species, you must have separate authorization under the Endangered Species Act (ESA) (e.g., an ESA Section 10 permit, or a Biological Opinion (BO) under ESA Section 7, with "incidental take" provisions with which you must comply). The enclosed U.S. Fish and Wildlife Service (USFWS) BO (Number 2025-0033207, dated 22 December 2025) contains mandatory terms and conditions to implement the reasonable and prudent measures that are associated with "incidental take" that is also specified in the BO (Enclosure 5). Your authorization under this Corps permit is conditional upon your compliance with all of the mandatory terms and conditions associated with "incidental take" of the attached BO, which terms and conditions are incorporated by reference in this permit. Failure to comply with the terms and conditions associated with incidental take of the BO, where a take of the listed species occurs, would constitute an unauthorized take, and it would also constitute non-compliance with your Corps permit. The USFWS is the appropriate authority to determine compliance with the terms and conditions of their BO, and with the ESA.
2. You shall comply with all terms and conditions of the enclosed December 08, 2025 Section 401 Water Quality Certification (Enclosure 6).
3. You are responsible for all work authorized herein and ensuring that all contractors and workers are made aware and adhere to the terms and conditions of this verification. You shall ensure that a copy of the verification and associated drawings are available for quick reference at the project site until all construction activities in waters of the U.S. authorized by this verification are completed.
4. Prior to commencement of construction activities in waters of the U.S.
authorized by this permit/verification, you shall clearly identify the limits of disturbance in the field with highly visible markers (e.g. construction fencing, flagging, silt barriers, etc.). You shall maintain such identification properly until construction is completed and the soils have been stabilized. You are prohibited from any activity (e.g. equipment usage or materials storage) that impacts waters of the U.S. outside of the permit limits as shown on Impacts to Waters of the U.S.
figure, dated 19 May 2025, and prepared by Rosenberg & Associates.
Within 30 days after completion of the authorized work, you must sign the enclosed Compliance Certification and return it to this office (Enclosure 7).
This verification is valid until March 14, 2026, when the existing NWPs are scheduled to be modified, reissued, or revoked. Furthermore, if you commence or are under contract to commence this activity before the date the NWP is modified, reissued, or revoked, you will have 12 months from the date of the modification, reissuance or revocation to complete the activity under the present terms and conditions. Failure to comply with the general and regional conditions of this NWP, or the project-specific special conditions of this authorization, may result in the suspension or revocation of your authorization.
