About this file

This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in Los Alamos, New Mexico. The form is required as part of the AutoCAD Training solicitation and must be completed and signed by offerors or bidders prior to contract award consideration. The certification applies to primary covered transactions and requires the offeror to attest that neither the company nor any of its principals are currently debarred, suspended, proposed for debarment, or excluded from covered transactions by any federal, state, or local agency.

The offeror must certify that within the three-year period preceding submission, they have not been convicted of fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records; have not violated contract provisions or demonstrated unsatisfactory performance; and have not had public contracts terminated for cause or default. Additionally, the offeror must confirm that they are not current or former County employees and are not immediate family members of County employees or public officials as defined by the certification. The form notes that falsification of any statement constitutes grounds for non-consideration of the proposal or rescission of a contract award, and that certification of any adverse statements may not necessarily preclude consideration but will be thoroughly reviewed by the County.
