About this file

This is a Summary of Benefits and Coverage (SBC) document for the BlueOptions 03559 PPO health plan offered by Florida Blue, with coverage effective October 1, 2025 through September 30, 2026. The document outlines comprehensive medical, prescription drug, and mental health coverage for individual and family enrollment. The plan features an in-network deductible of $1,000 per person/$3,000 family and an out-of-pocket limit of $3,000 per person/$9,000 family, with separate $200 pharmacy deductible. Covered services include preventive care with no cost-sharing, primary care visits at $20 copay, specialist visits at 20% coinsurance after deductible, and emergency room care. Prescription drug coverage includes generic drugs at $15 retail/$30 mail order, preferred brand drugs at $30 retail/$60 mail order (after $200 pharmacy deductible), and non-preferred brand drugs at $50 retail/$100 mail order (after $200 pharmacy deductible). The plan provides coverage for hospital stays, mental health and substance abuse services at no charge before deductible, maternity care, home health care limited to 60 visits, rehabilitation services limited to 35 visits, and skilled nursing care limited to 60 days. Out-of-network coverage requires 40% coinsurance after applicable deductibles.

The plan excludes dental care, routine eye care, hearing aids, bariatric surgery, acupuncture, habilitation services, infertility treatment, and weight loss programs. Virtual visits are available only through in-network providers for primary care and specialist services at the same copay or coinsurance as in-person visits. The document confirms the plan provides minimum essential coverage and meets minimum value standards. Preventive care services, including age-appropriate screenings and immunizations, are covered without cost-sharing and do not require meeting the deductible. The plan utilizes a provider network with significantly lower cost-sharing for in-network providers; out-of-network providers result in balance billing risk. The SBCID 2994712 identifies this specific plan configuration, and additional plan details are available at www.floridablue.com/plancontracts/group or by calling 1-800-352-2583.

BlueOptions 03559 Coverage Period: 10/01/2025 - 09/30/2026 with Rx $200 Rx Deductible $15/$30/$50 Summary of Benefits and Coverage: What this Plan Covers & What You Pay For Covered Services Coverage for: Individual and/or Family | Plan Type: PPO

SBCID: 2994712

The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately.

This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, www.floridablue.com/plancontracts/group.

For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms see the Glossary. You can view the Glossary at www.floridablue.com/plancontracts/group or call 1-800-352-2583 to request a copy.

Important Questions Answers Why This Matters:

What is the overall deductible?

In-Network: $1,000 Per Person/$3,000 Family. Out-of-Network: $1,000 Per Person/$3,000 Family.

Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.7

Are there services covered before you meet your deductible?

Yes. Preventive care.

This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible. See a list of covered preventive services at www.healthcare.gov/coverage/preventive-care-benefits/.

Are there other deductibles for specific services?

Yes. $200 Pharmacy Deductible; . There are no other specific deductibles.

You must pay all of the costs for these services up to the specific deductible amount before this plan begins to pay for these services.

What is the out-of-pocket limit for this plan?

In-Network: $3,000 Per Person/$9,000 Family. Out-Of-Network: Combined with In-Network.

The out-of-pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they have to meet their own out-of-pocket limits until the overall family out-of-pocket limit has been met.

What is not included in the out-of-pocket limit?

Premium, balance-billed charges, and health care this plan doesn't cover.

Even though you pay these expenses, they don’t count toward the out–of–pocket limit.

Will you pay less if you use a network provider?

Yes. See https://providersearch.floridablue.com/pr ovidersearch/pub/index.htm or call 1- 800-352-2583 for a list of network providers.

This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing). Be aware your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.

Do you need a referral to see a specialist?

No. You can see the specialist you choose without a referral.

All copayment and coinsurance costs shown in this chart are after your deductible has been met, if a deductible applies.

About these Coverage Examples:

This is not a cost estimator. Treatments shown are just examples of how this plan might cover medical care. Your actual costs will be different depending on the actual care you receive, the prices your providers charge, and many other factors. Focus on the cost sharing amounts (deductibles, copayments and coinsurance) and excluded services under the plan. Use this information to compare the portion of costs you might pay under different health plans. Please note these coverage examples are based on self-only coverage.

Note: These numbers assume the patient does not participate in the plan’s wellness program. If you participate in the plan’s wellness program, you may be able to reduce your costs. For more information about the wellness program, please contact: www.floridablue.com.

*Note: This plan has other deductibles for specific services included in this coverage example. See "Are there other deductibles for specific services?” row above.
