About this file

This is a Summary of Benefits and Coverage (SBC) document for the BlueOptions 03160 health insurance plan offered by Florida Blue for the Suwannee County Board of County Commissioners and related constitutional offices in Florida, effective October 1, 2025, through September 30, 2026. The plan is a PPO product compatible with Health Savings Accounts and features a tiered prescription drug structure with $15 copayments for generic drugs, $30 for preferred brand drugs, and $50 for non-preferred brand drugs at retail pharmacies. The plan includes an in-network deductible of $1,650 per person with an out-of-pocket limit of $5,100 per person, while out-of-network deductibles and out-of-pocket limits are double these amounts at $3,300 and $10,200 respectively. Covered services include primary care visits, specialist visits, emergency room care, hospital stays, mental health services, and maternity care, all with cost-sharing obligations after the deductible is met. The plan covers preventive care without cost-sharing and does not require referrals for specialist visits, though virtual visits are only available through in-network providers.

The plan excludes numerous services including acupuncture, bariatric surgery, cosmetic surgery, dental care, hearing aids, infertility treatment, pediatric dental and eye care, and routine adult eye care, though limited chiropractic coverage of 35 visits and non-emergency international care are covered. Home health care is limited to 60 visits, rehabilitation services to 35 visits, skilled nursing care to 60 days, and inpatient rehabilitation to 30 days, with prior authorization potentially required for certain services. The specific premium costs are not included in this document and will be provided separately by Florida Blue. The plan meets both Minimum Essential Coverage and Minimum Value Standards under the Affordable Care Act, and members may participate in a wellness program to potentially reduce out-of-pocket costs beyond the examples provided for sample medical situations.

BlueOptions 03160 Coverage Period: 10/01/2025 - 09/30/2026

HSA Compatible with Rx $15/$30/$50 after In-network Deductible Summary of Benefits and Coverage: What this Plan Covers & What You Pay For Covered Services Coverage for: Individual | Plan Type: PPO

SBCID: 2994669

The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately.

This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, www.floridablue.com/plancontracts/group.

For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms see the Glossary. You can view the Glossary at www.floridablue.com/plancontracts/group or call 1-800-352-2583 to request a copy.

Important Questions Answers Why This Matters:

What is the overall deductible?

In-Network: $1,650 Per Person. Out-of- Network: $3,300 Per Person.

Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the policy, the overall family deductible must be met before the plan begins to pay.7

Are there services covered before you meet your deductible?

Yes. Preventive care.

This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible. See a list of covered preventive services at www.healthcare.gov/coverage/preventive-care-benefits/.

Are there other deductibles for specific services?

No. You don’t have to meet deductibles for specific services.

What is the out-of-pocket limit for this plan?

In-Network: $5,100 Per Person. Out-Of- Network: $10,200 Per Person.

The out-of-pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, the overall family out-of-pocket limit must be met.

What is not included in the out-of-pocket limit?

Premium, balance-billed charges, and health care this plan doesn't cover.

Even though you pay these expenses, they don’t count toward the out–of–pocket limit.

Will you pay less if you use a network provider?

Yes. See https://providersearch.floridablue.com/pr ovidersearch/pub/index.htm or call 1- 800-352-2583 for a list of network providers.

This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing). Be aware your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.

Do you need a referral to see a specialist?

No. You can see the specialist you choose without a referral.

All copayment and coinsurance costs shown in this chart are after your deductible has been met, if a deductible applies.

About these Coverage Examples:

This is not a cost estimator. Treatments shown are just examples of how this plan might cover medical care. Your actual costs will be different depending on the actual care you receive, the prices your providers charge, and many other factors. Focus on the cost sharing amounts (deductibles, copayments and coinsurance) and excluded services under the plan. Use this information to compare the portion of costs you might pay under different health plans. Please note these coverage examples are based on self-only coverage.

Note: These numbers assume the patient does not participate in the plan’s wellness program. If you participate in the plan’s wellness program, you may be able to reduce your costs. For more information about the wellness program, please contact: www.floridablue.com.
