About this file

This is a Project Documentation Checklist for the Wastewater System Capacity Improvements Project in the City of St. Helens, Oregon, specifically covering infrastructure construction and replacement within Basins 4 and 6 (Project No. S-679). The project aims to increase system capacity, improve reliability, and address conveyance deficiencies in the sanitary sewer collection system. The checklist establishes submission timelines across multiple project phases, beginning with bid opening requirements including bid bonds, firm offers, bid schedules, acknowledged addenda, and first-tier subcontractor disclosure forms. Documentation requirements continue through project award, with contractors required to submit contractor data certifications, certificates of insurance, performance bonds, and payment bonds (each equal to 100 percent of the awarded contract value) within ten business days of award. Additional pre-construction submissions include proof of St. Helens business licensing, filing of a $30,000 Public Works Bond with BOLI, prevailing wage agreements, designations of superintendent and emergency maintenance supervisors with 24-hour contact information, and project safety plans.

Throughout construction, contractors must maintain weekly certified payroll documentation, submit updated construction schedules as required, provide traffic control plans, and furnish American Iron & Steel certifications monthly. Submittals required prior to the start of construction include materials and equipment lists, product data and shop drawings, and project documentation for residents in the project area. The contract specifies submission of certificates of substantial completion at substantial completion, followed by certificates of final completion after punchlist items are resolved. The checklist explicitly notes that this list represents a minimum baseline and may be expanded at the engineer's direction, with contractors responsible for identifying and preparing all submittals required by the contract documents regardless of whether specifically enumerated in this list.
