About this file
This is an Affidavit of Compliance form required by the Town of Palm Beach Procurement and Contracts Division for the North Lake Way Living Shoreline construction project in Florida. The affidavit serves as a compliance certification document that bidders must execute and notarize before submitting bids for this living shoreline stabilization and enhancement project along North Lake Way at the Lake Worth Lagoon. The project scope includes construction of a nearshore rock breakwater system, restored mangrove habitat, and stabilized upland slope with native vegetation. A non-mandatory pre-bid meeting is scheduled for Thursday, April 30, 2026, at 9:00 AM EST via Microsoft Teams, with attendance strongly encouraged as the only forum for direct clarification from the project manager and Town staff. Award will be made to the lowest responsive and responsible bidder whose bid meets the stated requirements, with the Town reserving the right to reject any and all bids.
The affidavit requires bidders to attest under penalty of perjury to compliance with Florida's foreign entity laws and anti-human trafficking laws. Specifically, bidders must certify that their entity is not owned or controlled by a foreign government of concern, is not organized under foreign laws with a principal place of business in a foreign country of concern, and does not engage in coercion for labor or services as defined under human trafficking statutes. The affiant must be an adult resident of Florida and authorized to execute the affidavit on behalf of the bidding entity. The document requires notarization by a Florida notary public, with signature, date, entity name, and title fields to be completed prior to bid submission through the Town's Euna Procurement (Bonfire) eProcurement platform.
