DESCRIPTION OF

SHOW:

A brief paragraph stating overall theme of program and where it is being shown.

RESTRICTIONS:

A brief statement discussing the final program restrictions and what the cleared distribution is. (i.e. unrestricted; non-profit sales; web; broadcast; etc.)

USE RIGHTS:

A brief statement of what source material is restricted (i.e., music, historical images, some stock footage, etc). Licenses and release forms to be provided in Attachment A. Detailed information to be provided in Attachment B)

FOOTAGE:

State the type of footage used (original and/or stock) and general restrictions, if any, and term (i.e. for the life of the project, etc.). Also state the format used for capture of all original footage.

STILLS AND

GRAPHICS:

State the type of stills, archival photos, graphics used and general restrictions, if any, type of clearances acquired (i.e. license, permission letter, etc.) and term (i.e. for the life of the project, etc.).

NARRATION:

State names of all narrators and voice actors and restrictions, if any, type of clearances acquired (i.e. license, permission letter, etc.) and term (i.e. for the life of the project, etc.).

MUSIC:

State music used—i.e. original composition, library music, already recorded music and general restrictions, if any, type of clearances acquired (i.e. license, permission letter, etc.) and term (i.e. for the life of the project, etc.).

OTHER TALENT:

State any other talent used, including interviews, and restrictions, if any, type of clearances acquired (i.e. license, permission letter, etc.) and term (i.e. for the life of the project, etc.).

DELIVERED:

List number of hard drives delivered. (Each hard drive delivered should be labeled with the park acronym and media number and include an index of hard drive contents.)

ATTACHMENTS:

A Licenses/Release Forms, and Permission Letters All elements of this program need to be accounted for, such as footage, music, narration, additional talent, and interviews. Attachment A should include all licenses, release forms, and/or permission letters necessary for use of every element.

B Source Material List This is a detailed listing of ALL footage and stills used in the program, listed in timecode order. This list also includes thumbnails, source information and use rights. See the attached sample under Attachment B for the correct table format.

C Credits A listing of credits as they appear in the production.

D Final Scripts A copy of the final script, including program scripts, foreign language, subtitle scripts, and audio description scripts E Sequence Information This section should include the Edit Decision List (EDL) of the completed show.

F Original Footage and Sound Logs Detailed footage and audio logs created during the production phase, including:

Date of filming/recording, location, ID of anyone on screen or speaking, description of scenes including discrete scene numbers, camera(s) used, type of audio recorded, and any other information pertinent to identifying the footage.

G Music Cue Sheet State all music used (i.e. original composition, library music, etc.) and timecode of the selection within the program. Identify music title, artist, and copyright owner, restrictions, if any, and type of license acquired (i.e. license, permission letter, clearances).
