About this file

This document is a comprehensive set of environmental assessment reports for a Bureau of Land Management property located at 12691 County Road G in Cortez, Colorado. The reports include an Asbestos Survey and a Lead-Based Paint Inspection, both conducted by Plateau, Inc. on October 27, 2016.

The Asbestos Survey revealed that out of 57 samples collected from various building materials and surfaces, only one sample (sample #32) contained asbestos - a sponge texture in the southwest bedroom with 0.50% Chrysotile. The Lead-Based Paint Inspection found lead-based paint in three specific locations: the tin ceiling in the family room (2.1% lead), the wall system in the family room (1.1% lead), and the den ceiling north of the family room (2.7% lead). Due to these findings, the report recommends lead hazard reduction activities as defined by the EPA and notes that any renovation or construction work will require contractors experienced in handling hazardous materials and following specific safety protocols.

1.0 INTRODUCTION

Plateau, Inc. was retained by the Bureau of Land Management to perform an asbestos survey for the purpose of remodeling of a historic home back to original structure. The home is located at 12691 County Road G, Cortez, Colorado. Mr. Chris Lakin, certified by the EPA and CDPHE, retains certification in accordance with regulatory guidelines and is identified by CDPHE as

Asbestos Inspector #13283. The inspection was conducted in accordance with applicable EPA, OSHA and CDPHE regulations and was performed on October 27, 2016. Those materials affected by assumed to be affected by renovation activities were inspected and sampled in general accordance with AHERA regulations.

2.0 ASBESTOS METHODOLOGY

The purpose of the asbestos inspection was to identify the condition of friable and

Category II non-friable asbestos materials that are present and will be affected by renovation activities. Only accessible materials were inspected and testing in accordance with Colorado Reg.

8, Part B. During the asbestos materials survey, Plateau, Inc. performed the following tasks:

Inspected accessible areas for suspected asbestos materials;

Developed a sampling plan for each material based on the homogeneous material type, friability, accessibility, and material locations;

Collected samples of suspected homogeneous and non-homogeneous materials in accordance of Colorado Reg. 8, Part B and submitted them for laboratory analysis by Polarized Light Microscopy (PLM); and, Documented findings and inspection protocol in accordance with accepted industry standards.

[4]

3.0 ASBESTOS INSPECTION PROCEDURE

Asbestos sampling is conducted by segregating building materials into sampling units call homogeneous areas. A homogeneous area is defined as a material that is uniform in texture and color and appears identical in every other respect. Homogeneous suspect asbestos materials were identified by visually inspecting the following systems:

Surface Systems Thermal Systems Miscellaneous Systems

Based on Mr. Lakin’s inspection, the following types of suspect asbestos-containing materials were identified:

Surface Systems o Wall Systems – Numerous, Multiple Textures

Miscellaneous o Sheet Flooring - Numerous o Mortar o Roofing – Numerous o Roofing Underlayment

3.1 Asbestos Sample Collection

The EPA and CDPHE regulate the minimum number of samples to be collected of each suspect material as outlined on Table 1, Sampling Minimums.

Mr. Lakin collected bulk samples of the suspected friable and non-friable asbestos materials in a random and representative manner as defined by the U.S. Environmental Protection Agency

(EPA) statistical sampling methods. To prevent cross contamination, equipment is washed in between the collection process. The collected samples were packaged in sealed baggies and labeled appropriately.

[5]

3.2 Asbestos Sample Analysis

The bulk samples of suspected asbestos materials were submitted under chain of custody to an independent laboratory for analysis. EMSL Analytical Inc. is accredited by the National

Voluntary Laboratory Accreditation Program (NVLAP) (Lab Code 200828-0). Individual layers of the samples were analyzed by PLM (Polarized Light Microscopy), a bulk sample analysis method established by the National Voluntary Laboratory Accreditation Program (NVLAP), to determine asbestos type and content. Bulk samples were analyzed using EPA Method 600/R-93/116. Unused portions of the samples are archived for 60 days, unless the client requests special handling. By regulation, any single positive asbestos sample classifies the entire homogeneous material as asbestos - containing and additional analysis is not required.

4.0 ASBESTOS REGULATORY CRITERIA

According to the Occupational Safety and Health Administration (OSHA), the EPA, and the

Colorado Department of Public Health and Environment (CDPHE), samples with asbestos concentrations greater than 1 percent are classified as asbestos containing and are a regulated material. If a building structure is scheduled for renovation or demolition, samples that contain a

“trace” amount of asbestos (less than 1 percent) must be further analyzed by a more accurate point-count analysis to determine if they surpass the 1 percent threshold, or the materials must be assumed to contain asbestos and be classified as a regulated material.

The EPA and OSHA distinguish between friable and non-friable forms of asbestos materials. Friable materials can be crumbled or reduced to powder by hand pressure when dry. Non-friable materials cannot be crumbled, pulverized, or reduced to powder by hand pressure when dry. Friable materials are more likely to be released into the air, especially if impacted or damaged during normal use, renovation, or demolition of a building. Therefore, the distinction between friable and non-friable asbestos materials is important. The EPA further segregates non-friable asbestos materials into Category I or Category II. Category I non-friable asbestos materials include floor tiles and roofing felts. Removal of these asbestos materials is not required prior to demolition as long as they are in good condition. Category II asbestos materials are all other non-friable asbestos

[6] materials and must be removed prior to normal demolition or renovation.

Whether removed or remaining in a structure during demolition, the confirmed or presumed asbestos materials are subject to EPA National Emission Standards for Hazardous Air

Pollutants (NESHAP) and OSHA regulations. In 40CFR61.145, NESHAP requires that each owner or operator of a demolition activity provide the administrator with written notice of intent. The

CDPHE has implemented the NESHAP program. At the time of this report, a scope of work detailing construction activities has not been provided to Plateau, Inc. Please contact Plateau when plans are developed to obtain a Colorado State demolition permit if necessary.

5.0 ASBESTOS INSPECTION RESULTS

Results of the asbestos analyses for the homogeneous material collected from the structure are summarized below and in Table 2. Table 2 presents Plateau, Inc.’s homogeneous material identifier and laboratory results for asbestos content. Laboratory reports for the samples are presented in Appendix B.

12691 County Road G, Cortez, Colorado

Plateau, Inc. collected fifty-seven (57) samples of suspect building materials from the structure.

PLEASE REFERENCE TABLE 2 – ASBESTOS RESULTS FOR HOMOGENEOUS MATERIALS

All but one sample submitted under chain of custody to EMSL Analytical, Inc. were non-detect for asbestos. The one sample, sample number 32, required point counting analysis according to

Colorado Reg. 8, Part B. and the final result is .50% Chrysotile. Sample 32 is a homogenous area of texturing in the southwest bedroom that includes the wall and ceiling surfaces. The appearance of the material is a sponge texture colored white. This material is not regulated by Colorado Reg. 8, Part B; however, it is regulated by OSHA. Removal of the wall/ceiling systems or striping of the texture will require a contractor with experience in OSHA hazardous materials protocol.

[7]

6.0 CONCLUSIONS AND RECOMMENDATIONS

Asbestos containing materials are regulated by the Colorado Department of Public Health and Environment (CDPHE), the U.S. Environmental Protection Agency (EPA), and the Occupational

Safety and Health Administration (OSHA). Colorado Reg. 8, Part B requires demolition permits for projects that require the removal of any load bearing member. A Colorado State demolition permit application is not included with this report. Please contact Plateau upon final renovation plans so we may determine if a demolition permit application is necessary.

7.0 LIMITATIONS

This report was prepared for the use of assessing the potential asbestos building materials that may be affected during renovation activities of a structure located at 12691 County Road G, Cortez, Colorado. Observations made were not intrusive and limited to readily visible or exposed building material within the structure. Our services were performed with no destruction for sampling; thus, our services are not intended to guarantee or certify the absence of asbestos.

The accuracy and reliability of environmental studies are a reflection of the number and type of samples taken and extent of the analyses conducted, and are thus inherently limited to and dependent upon the resources expended. An independent laboratory performed laboratory analysis. Plateau, Inc. is not responsible for the accuracy of data presented by others.

Plateau, Inc. performed the survey in a manner consistent with the level of care and expertise exercised by members of the asbestos inspection and assessment profession. Plateau, Inc. does not imply or guarantee that every suspect asbestos material on or in the building has been identified or sampled. Historically, asbestos has been used extensively in the United States. This inspection is intended to identify those components that are reasonably suspect and hence most likely to be asbestos materials in quantities subject to regulation based on existing industry and regulatory standards. If additional asbestos materials or asbestos-suspect materials are encountered, work should be stopped and an additional asbestos inspection performed.

Thank you for your business. If we can be of further service, please do not hesitate to call us, 970.252.1363.

Sincerely, Plateau, Inc.

1.0) Cover Letter

Prepared for: Bureau of Land Management Montrose, Colorado 81401

Subject Property: 12691 County Road G Cortez, Colorado 81321

Dear Sirs:

Please find the limited lead-based inspection and visual assessment report for the future renovation project located at 12691 County Road G, Cortez, Colorado. The lead-based paint inspection and visual assessment was performed following guidelines set forth by the EPA model for lead-based paint. The purpose of the limited LBP survey was to determine the presence or absence of lead-based paint of suspect surfaces on accessible building materials at the subject property.

Plateau, Inc. is a certified evaluation firm in the State of Colorado and their certification # is 17358. Chris Lakin performed the Inspection and is Certified Lead Inspector/Risk Assessor in the State of Colorado and his certification # is 21988 with an expiration date April 29, 2017. Plateau conducted the visual assessment and lead paint inspection on October 27, 2016.

At the time of this report, the future use of this property has not been determined nor has a renovation scope of work been developed by the BLM. This report is for informational purposes only not allowing hazard assessments for future occupants. The home was originally constructed 1911 according to the Montezuma county assessor records. The home is 100% masonry stone, one and a half stories with a gabled composite shingled roof. Interior materials include painted gypsum wallboard, painted tin ceilings and painted trim components. Exterior materials include painted soffits, trim and wood wallboard.

Chris Lakin performed the inspection utilizing laboratory paint-chip analysis. During the visual assessment Plateau, Inc. observed paint on the interior and exterior components of the property. The survey is a general assessment of existing paint products providing information for what can be anticipated during renovation.

In association with the visual assessment, a limited lead-based paint inspection was performed on components that may be disturbed during a renovation of the property. Positive lead-based paint was found on the tin ceiling in the family room, the wall system in the family room as well as the den ceiling north of the family room. Due to these findings lead hazard reduction activities will be required as defined by the EPA.

If you have any questions please contact us directly at 970.252.1363.

Regards, Christopher Lakin

Lead Inspector / Risk Assessor

2.0) Executive Summary

The Bureau of Land Management has authorized Plateau, Inc. to perform a visual assessment and lead-based paint

(LBP) inspection on a future renovation project located at, 12691 County Road G, Cortez, Colorado 81321.

Plateau, Inc. tested a limited number of painted components according to the specifications described in the protocols for Lead-Based Paint testing in Chapter 7 of the Housing and Urban Development (HUD) Guidelines

(Revised 1997) and all applicable Federal, State, and Local regulations for the inspection of lead based paint.

Plateau’s scope of services involved laboratory paint-chip testing as well as a surface-by-surface visual inspection of the painted surfaces documented to be constructed prior to 1978 and will be potentially disturbed by future renovation activities.

Plateau sampled a total of twenty-four (24) surfaces. Twenty-one components were found to contain lead at levels less than the regulatory level of 0.5% by weight. These surfaces are identified in Section 4.0 (Findings). This report represents all field data, observations and findings related to the lead inspection performed in the above referenced property. The results, assessments and findings stated in this report are representative of the conditions observed in the property at the time of inspection.

Lead inspections determine the presence of lead in paint and other possible lead-based and contaminated areas.

This inspection, measures lead in both deteriorated and intact paint surfaces. The procedure involved taking samples from representative surfaces throughout the testing area or room.

3.0) Scope of Inspection

Background Information

The property is located at 12691 County Road G, Cortez, Colorado 81321. The property is vacant currently with future renovation activities that are unknown at the time of this report. No written permission was required to access the property as the owner’s representative provided access at the time of the inspection. The structure is vacant with no visible signs of occupancy, i.e., water and electricity.

Preface

Plateau, Inc. was authorized by Tara Hough with the Bureau of Land Management to perform a lead-based paint inspection on the above referenced property to determine the possible presence of lead-based paint. Only painted walls, ceilings and trim materials comprised the testing combination. The inspection was conducted on

October 27, 2016.

At the time of inspection, a scope of future renovation activities was not provided for review. Plateau based the limited LBP inspection on likely areas of impact for future renovation activities.

Training

All inspectors utilized by Plateau, Inc. have EPA/State licensure and are licensed Lead Risk Assessors, or Inspectors who have passed the “HUD Visual Assessment Course”. Training certificates are included with this report.

Inspection Company

The inspection was performed by an employee of Plateau, Inc. Plateau, Inc. is a Colorado registered Lead

Evaluation Firm No: LEF17358. Company certificates are included with this report.

Methods

Inspections are done using the inspection protocol in Chapter 7 of the Housing of Urban Development (HUD)

Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing (1997 Revision) as well as all other Federal, State and Local guidelines. The methods utilized for lead analysis include careful removal of material, documenting amount and characteristics of material. Material is submitted under chain of custody to an accredited laboratory. Only laboratories recognized under the EPA NLLAP may be used for analyzing samples of paint in target housing or pre-1978 child occupied facilities. Laboratory analysis is more accurate and precise than

XRF, but only if great care is used to collect and analyze the paint-chip sample.

4.0) Findings

12691 County Road G, Cortez, Colorado 81321

This property is currently vacant with pending future renovations. No extrapolations were required because

Chapter 7 Single Family Testing Rules were followed.

Plateau, Inc. tested twenty-four surfaces via laboratory analysis. Twenty-one tested components were found to contain lead at levels below the EPA regulatory level of 0.5% by weight. Three surfaces were found to contain lead at levels above the EPA regulatory level of 0.5% by weight. Please review Table A for location and description.

5.0) Conclusions

When evaluating this report, it is assumed that according to Chapter 7 HUD guidelines, that if one testing combination (i.e. window, door) is positive for lead in an interior or exterior room equivalent, that all other similar testing combinations in those areas are assumed to be positive. The same is true for negative readings. All inaccessible areas are assumed to be positive, even though they were not able to be tested. The positive findings for LBP is the family room and den walls, family room tin ceiling and the den wood ceiling.

Some painted surfaces may contain levels of lead below 0.5% by weight; these components could create lead dust or lead contaminated soil hazards if the paint is turned into dust by abrasion, scraping, sanding or friction. If conditions of intact paint surfaces become destabilized, these conditions will need to be addressed in the future. If any construction or modernization work is done on the premises, this report should be given to the contractors, as well as to any future tenants.

Any stabilization/construction activities that affect the existing paint films (including sanding and demolition) must be initiated by workers who have successfully received proper training and certification in the handling of lead-contaminated materials.

Plateau, Inc. recommends all contractors follow the “Lead Safe Work Practices” as outlined in The Lead Safe

Housing Rule 24 CFR Part 35 as amended June 21, 2004. Interim control measures on friction or impact surfaces, such as windows and doors, may lead to rapid treatment failure. It is the contractor’s responsibility to follow all city, state and federal regulations when performing Lead Hazard Reduction Activities.

6.0) Disclosure Responsibility

A copy of this limited report must be provided to new lessees (tenants) and purchasers of this property under Federal Law (24 CFR part 35 and 40 CFT part 745) before they become obligated under a lease or sales contract. The complete report must also be provided to new purchasers and it must be made available to new tenants. Landlords

(lessors) and sellers are also require to distribute an educational pamphlet and include standard warning language in their leases or sales contracts to ensure that parents have the information they need to protect their children from lead-based paint hazards.

7.0) Disclaimers

This is our report of a limited visual survey and lead-based paint analysis. The presence or absence of lead-based paint (LBP) or lead-based paint hazards applies only to the tested or assessed surfaces on the date of the field visit and it should be understood that conditions might change due to deterioration or maintenance. The results and material conditions noted within this report were accurate at the time of the inspection and in no way reflect the conditions at the property after the date of the inspection. Ongoing monitoring by the owner is usually necessary.

Those surfaces that do not contain lead-based paint at or above the federal, state, or local standards may still pose a hazard if disturbed.

The Occupational Safety and Health Administration (OSHA) Lead in Construction Standard states that

“negative” readings (i.e. those below the HUD/EPA definition of what constitutes LBP [1.0 mg/cm2]) do not relieve contractors from performing exposure assessments (personal air monitoring) on their employees per the OSHA Lead Standard, and should not be interpreted as lead free. Although a reading may indicate “negative”, airborne lead concentrations still may exceed the OSHA Action Level or the OSHA Permissible Exposure Limit (PEL) depending on the work activity.

GLOSSARY

Abatement· A measure or set of measures designed to permanently eliminate lead-based paint hazards or llead-based paint Abate ment strategies include the removal of lead-based paint, enclosure, encapsulation, replacement of building components coated with lead-based paint, removal of lead-contaminated dust, and removal of lead-contaminated soil or overlaying of soil with a durable covering such as asphalt (grass and sod are considered interim control measures). All of these strategies require preparation; cleanup, waste disposal, post-abatement clearance testing; recordkeeping; and, if applicable, monitoring. See also Complete Abatement and Intrim Controls.

Accessible surface - Any protruding interior or exterior surface, such as an interior windowsill, that a young child can mouth or chew.

Accreditation - A formal recognition that an organization, such as a laboratory, is competent to carry out specific tasks or types of tests.

Accredited laboratory • A laboratory that has been evaluated and approved by the National Lead Laboratory Accreditation Program (NLLAP) to perform lead measurement or analysis, usually over a specified period of time.

Apron • A trim board that is installed beneath a window sill Area wells ·Corrugated metal or concrete barrier walls installed around a basement window to hold back the earth Attic access· An opening that is placed in the dry walled ceiling of a home providing access to the attic.

Attic Ventilators - In houses, screened openings provided to ventilate an attic space Backing - Frame lumber installed between the wall studs to give additional support for drywall or an interior trim related iitem, such as handrail brackets, cabinets, and towel bars. In this way, items are screwed and mounted into solid wood rather than weak drywall that may allow the item to break loose from the wall. Carpet backing holds the pile fabric in place.

Balusters - Vertical members in a railing used between atop rail and bottom rail or the stair treads. Sometimes referred to as 'pickets' or 'spindles'.

Balustrade· The rail posts and vertical balusters along the edge of a stairway or elevated walkway.

Bare soil - Soil not covered with grass, sod, some other similar vegeta tion, or paving, including the sand in sandboxes.

Barge board • A decorative board covering the projecting ratter (fly rafter) of the gable end. At the cornice, this member is a fascia board.

Base or baseboard • A trim board placed against the wall around the room next to the floor.

Basement window Inserts •The window frame and glass unit that is installed in the window.

Base shoe· Molding used next to the floor on Interior baseboard. Some times called a carpet strip.
