If any non-federal funds have been or will be used for lobbying purposes, the respondent must complete and submit Standard Form LLL "Disclosure of Lobbying Activities" in accordance with its instructions.

The form also mandates that the certification language be included in all sub-award documents at all tiers, including subcontracts, sub-grants, and contracts under grants, loans, and cooperative agreements. Failure to file the required certification can result in civil penalties of not less than $10,000 and not more than $100,000 for each such failure. The form requires the respondent to provide their signature, printed name, title, date, and organization, though these fields are currently blank in this document.
