About this file

This document is Attachment C—a transmittal letter form required for submission in response to 26RFP10, issued by Williamson County, Texas, for flooring installation and repair services on an as-needed basis for county-owned premises. The transmittal letter serves as the primary vehicle for respondents to demonstrate compliance with solicitation requirements and provide essential company information covering the past five fiscal years. Respondents must complete all sections of this form or risk having their submission deemed non-responsive.

The transmittal letter requires respondents to provide comprehensive company details including legal name, business entity type, place of incorporation, locations of major offices and facilities, principal contact person information, Federal Employer Identification Number, and commitment to provide the required services. Additionally, respondents must disclose any bankruptcy filings or criteria met within the past five years, current defaults on loan or financing agreements, any relationships with Williamson County employees or officers, pending sales or acquisitions that could impact operations or company direction, and a detailed list of equipment to be used for service delivery. The form must be signed by an authorized representative legally empowered to bind the respondent to the representations made in both the transmittal letter and the proposal submission. Equipment used is subject to inspection, approval, and potential rejection at the County's sole discretion.
