About this file
This is a Proposal Affidavit form for Williamson County, Texas's Request for Proposal (RFP) 26RFP10 for Flooring Installation and Repair Services, as needed. The affidavit requires all proposing firms to certify compliance with multiple Texas Government Code provisions and represents a standardized certification document applicable to this collaborative procurement opportunity. The contract covers flooring installation and repair services for Williamson County-owned premises on an as-needed basis, with awards subject to adoption by other agencies through piggybacking provisions, except for professional services.
The affidavit mandates certifications regarding boycotts of Israel, engagement with sanctioned countries and organizations, boycotts of energy companies, and boycotts of firearm and ammunition industries. Most boycott-related provisions apply only to companies with ten or more full-time employees with contract values of $100,000 or more paid from public funds, with specific exceptions for entities excluded from federal sanctions regimes. Additional certifications required include verification that the firm has not been debarred from doing business in Texas and confirmation that the proposal was not prepared in collusion with competitors or through price-fixing agreements. Proposers must sign and date the affidavit, providing company name, signatory title, and contact information, with acknowledgment that inaccurate certifications may result in contract termination and payment withholding.
