Summary During the last ABS annual inspection, ABS Surveyors determined many of the Multi Cable

Transits (MCTs) on board the Nancy Foster do not meet cable packing requirements. It was reported some MCTs may not maintain firetight/watertight integrity, or MCTs may not use the correct firestop putty system. As a result, a number of outstanding technical comments are noted on the ABS Class Certificate.

A Regulatory-approved drawing showing fire zones/boundaries, a key component to the MCT survey, does not appear to exist. It is recommended as a first step that a fire boundary drawing be created and submitted for Regulatory approval. As the fire boundary drawing does not exist, a drawing was created using SOLAS regulations and Drawing NF-085-001 (General Arrangement) as a reference.

During the audit, it was determined 62% of the MCTs do not meet Regulatory and/or OEM requirements when compared to SOLAS Regulations. Due to the overall effort required to correct the found issues, resolution during a single shipyard repair period will be difficult. It is therefore recommended to develop a plan approved by the local ABS surveyors and move forward with resolution of found issues as soon as possible.

Regulatory Requirements ABS Steel Vessels under 90 meters in length Rules, (SVR) discuss requirements for MCTs in various Chapters and Sections. ABS SVR Part 4, Chapter 6, Section 3 (Subsection 5.13.1) provides a good summary:

“Where cables pass through watertight, firetight, or smoke-tight bulkheads or decks, the penetrations are to be made through the use of approved stuffing tubes, transit devices or pourable materials installed in accordance with manufacturer’s installation procedures to maintain the watertight integrity or fire-rating of the bulkheads or decks. These devices or pourable materials are not to damage the cable physically or through chemical action or through heat build-up, and are to be examined and tested as specified in 3-7-1/Table 1 and 4-6-3/5.13.4. Where cable conduit pipe or equivalent is carried through decks or bulkheads, arrangements are to be made to maintain the integrity of the water or gas tightness of the structure.”

For the evaluation of MCTs, it is helpful to break MCT requirements into two parts; MCTs passing through watertight boundaries and MCTs passing through fire-tight boundaries.

Watertight Integrity Requirements Below the Main Deck (Refer to ABS SVR Part 3, Chapter 1, Section 1 sub section 13.1), all MCTs through watertight bulkheads must maintain watertight integrity. On the Nancy Foster, the watertight bulkheads are:

• Frame 12

• Frame 21

• Frame 29

• Frame 46

• Frame 63

• Frame 71

• Frame 83

On the Nancy Foster, the majority of the watertight MCT cable seals consist of Nelson MCT systems or RoxTec MCT systems. In order to maintain watertight integrity, MCT blocks should be in good condition, co-planar, and contain only one cable per block. Blocks should be of the same make and manufacturer. Blocks which do not contain cables should be blank blocks. Compression blocks should be in good condition (i.e. not corroded), and tightened in order to provide a seal. Stay plates should be in good condition and aligned with the MCT frame. Blocks should all be present in order for the MCT to be functional; replacing a subset of blocks with firestop within a frame to make room for multiple cables is not permitted.

Fire Tight Integrity Requirements A drawing showing the Nancy Foster’s fire boundaries could not be located. SOLAS Chapter 2- 2, Part C, Regulation 9, includes fire integrity requirements for bulkheads and decks separating adjacent spaces. Specifically, Tables 9.5 and 9.6 apply to Cargo Ships (NOAA vessels are classified as Cargo Ships by SOLAS). These table govern the requirements for structural fire integrity (Refer to ABS SVR Part 3, Chapter 4, Section 1 sub section 5.1), and thus were used to create a Fire Zone Boundary drawing, NF-085-001. This drawing was created as a reference and is not a Regulatory-approved fire boundary drawing. A regulatory approved drawing should be obtained and used as the final guideline for repairs. In the absence of a fire zone boundary drawing, SOLAS requires the use of the most restrictive interpretation.

It should be noted that ABS SVRs require the fire control plan to specify the locations and type of fire retarding bulkheads however it does not take the place of or fulfill the need for an approved fire zone boundary drawing.

In order to maintain a firetight boundary, MCTs containing a firestop system should be installed in accordance with specific OEM requirements of the system being used. The attending Surveyor is to confirm that the installer is familiar with and has access to the manufacturer’s installation procedures. Common requirements found with firestop systems include: The firestop system must meet the fire rating of the bulkhead in which it is installed; MCT frame depth must be deep enough to facilitate the approved installation of system being used (typically firestop putty systems include a minimum required frame depth); appropriate filler/insulation material must be used;

putty application must be installed above a minimum thickness; minimum distance between the frame and cables must be maintained; no air gaps are permitted; cables should be secure and snug. After installation, all fire-rated cable penetrations are to be visually examined and tested as required.

Procedure Information was gathered for each MCT to fully describe the MCT characteristics. Stuffing tubes or “kickpipes” (transits with a single cable passing through a pipe penetration) were recorded separately and groups in the same location were given a signal serial number. MCT information obtained during the survey includes:

• Serial Number: A designator was assigned to each MCT in the form of 1-17-1.3-V.

o First Number (1) is the deck.

o Second Number (17) is the frame.

o Third number (1.3) is the transverse location; even numbers to port, odd numbers to starboard, “0” for centerline or originating in the main passageway of that deck.

Decimals were used for multiple penetrations in the same frame. Starting with the inboard most and moving outward.

o Fourth character (V) is orientation; “L” for a cable penetration through a longitudinal bulkhead, “T” for a cable penetration through a transverse bulkhead, “V” for vertical cable penetration (through a deck).

• Boundary Rating: Watertight, A-Class, B-Class, C-class, Non-tight, etc.

• MCT dimensions including length, width, and depth

• Spaces on each side of the penetration

• Notes: General observations, deficiencies, concerns, etc. were noted here.

• Repair recommendations: Suggested next steps were noted here.

Results Overall, of the 187 MCTs surveyed, 117 (or 62%) were found not to be in compliance with Regulatory requirements and/or OEM specifications. A summary of all MCT data is found in Appendix I. A summary of all Stuffing Tube data is found in Appendix II

Specific Failure Criteria for Watertight MCTs MCT blocks should be all present, of the same make, co-planar, and tight. Compression blocks and stay plates should be straight and in good condition (i.e not corroded). When cables are removed from an MCT, the associated block should be replaced with blank blocks. Reasons for failure of a watertight MCT can include:

• Compression blocks and/or stay plates do not make good compression on the blocks.

One good indicator of this is if cables can slide in and out within a block.

• Blocks are missing.

• Blocks within a single MCT are of a different make.

• Blocks which once held a cable but the cable is removed without replacing the block (i/e there is a hole in the block).

• Stay plates are bent/warped, indicating different style/size blocks are installed.

• There is a mixture of firestop putty and blocks within an MCT.

• Can see daylight through the MCTs.

Specific Failure Criteria for Firetight MCTs Firestop putty systems have various criteria and specific requirements depending on the system being used and the type of fire zone being penetrated. MCT frames should be deep enough to allow for all components of the firestop putty system. Putty should be installed such that it fills all air gaps and cables are securely held in place. When cables are removed from an MCT, the hole should be filled with insulation (if applicable) and firestop putty. Insulation installation should conform with the OEM’s requirements. ABS class certificates are based on specific testing criteria.

In other words, if the firestop system is being installed in an A-0 Class bulkhead, it may have different installation configurations than an installation in an A-60 Class bulkhead. Reasons for failure of a firetight MCT can include:

• Cable density through a single MCT exceeds 40%.

• Frame depth not sufficient to support firestop system.

• Firestop putty not installed in accordance with ABS approval requirements.

• Firestop putty not appropriate for the type of fire boundary being penetrated.

• Firestop putty system not an ABS approved system.

• Cables removed from the putty system and not filled.

• Can see daylight through the MCTs.

• Firestop system is old and brittle.

Common Issues Noted The survey clearly demonstrated the need for an ABS/USCG approved fire zone boundary drawing. It was reported this drawing does not exist and it is recommended a fire zone boundary drawing be created and ABS/USCG approval be obtained. The bulkhead fire zone boundaries shown on the approved fire control plan do not follow the SOLAS regulations. The pass/fail criteria used for this survey was based on the reference drawing created (NF-085-001).

During the conversion or at some point in time since NOAA has taken ownership of the ship a number of changes have been made to the ships structure and arrangement. It is apparent that fire integrity was not of concern when these changes were made.

The ship was designed and built with a drop ceiling installed in all accommodation spaces. This drop ceiling has since been removed from all accommodation spaces except the mess deck. Per regulations “continuous "B" class ceilings or linings, in association with the relevant decks or bulkheads, may be accepted as contributing, wholly or in part, to the required insulation and integrity of a division”. When the drop ceiling was removed it changed the fire integrity of the section of bulkhead above it. In turn changing the requirements for the MCT’s in these bulkheads.

Both the 01 and 02 level deck houses have been extended. When these modifications were made appropriate pipe cable and duct penetrations were not installed.

Similarly the hold area was divided into three separate spaces. Due to the size and use of these spaces they are required to be separated by fire rated boundaries. The boundaries all contain large openings for pipe and cable transits. The openings in some cases are simply rough holes cut in the bulkhead with no attempt made at installing a proper transit device. Others have collars but no firestop system is installed.

Throughout the ship, applied firestop systems are not in compliance with OEM installation recommendations and/or are not ABS-approved systems. Such MCTs with firestop material might pass ABS visual inspection or even a soap-and-blow test but are not in compliance with OEM requirements and, more to the point, do not maintain firetight integrity.

There are a number of instances in which multiple cables penetrate bulkheads through something other than an MCT. For example, there are cables that have been run through stiffener relief cuts at the top of bulkheads. These cables all need to be pulled back and run through proper transit devices. The disturbed insulation needs to be repaired to maintain the firetight integrity of the bulkhead.

04 Deck (Pilot House Top) Nine watertight MCTs were surveyed on the 04 Deck and all passed inspection. All are Roxtec cable glands. No corrective action required.

03 Deck (Pilot House) Thirty-seven MCTs were surveyed on the 03 Deck and 36 of them failed inspection.

Four of the 37 MCTs are watertight and located on the bridge wings. One of the MCTs is non-tight (which passed). The remaining 32 are firetight MCTs containing Firestop material. Of the 32 MCTs 30 are vertical and pass through the deck, 2 are horizontal and pass through the bulkheads at the bottom of the stairway. The bridge deck is an A-Class fire boundary per SOLAS, as are the bulkheads at the bottom of the stairwell. Stairways that are open to a space shall be considered as a part of that space. Therefore the firestop system must meet A-Class fire requirements.

The MCT firestop material/system used on the 03 Deck could not be identified, as a result it cannot be confirmed to be an ABS approved material. All of the firetight MCT’s lack sufficient collar depth to properly install any of the approved firestop system on the market.

There are two MCTs that exceed the 40% cable density threshold. In these cases, cables may need to be re-rerouted in order to meet cable density requirements. There are MCTs in close proximity which have space to allow for additional cables or the MCT’s can be modified.

Repair and modernization suggestions for the 03 deck can be found in Appendix 6.

02 Deck Twenty-one MCTs were surveyed on the 02 Deck and 17 failed. There are also 5 instances are cables were passed through stiffener relief cuts or rough holes in bulkheads.

One passing MCT is a Roxtec watertight gland exiting the P-Way to the weather. The others are MCTs that are passing through non firetight bulkheads and are packed in a manner that appears to be flame and smoke tight.

Two of the failing MCTs are vertical and pass through the deck to the 01 level. These spaces above and below are classified as accommodation spaces and not required to be firetight. SOLAS regulations state, “the division is required to be of steel or other equivalent material but is not required to be of "A" class standard. However, where a deck, except an open deck, is penetrated for the passage of electric cables, pipes and vent ducts, such penetrations should be made tight to prevent the passage of flame and smoke.” From a visual inspection it does not appear that the MCT’s are packed in a manner that is flame or smoke tight.

The other 15 failed MCTs all transit B-0 rated bulkheads. Because there is no product tested and approved to maintain the fire integrity of a B-Class boundary, all 15 MCTs must be modified to accept an approved firestop system. Installing an approved system not in accordance with OEM requirements does not satisfy regulatory requirements. An approved system that requires 8 inches collar depth for an A-Class boundary, cannot be installed at a lesser collar depth and be considered satisfactory in a B-Class boundary.

Repair and modernization suggestions for the 02 deck can be found in Appendix 6.

01 Deck (Forecastle Deck) Thirty-eight MCTs were surveyed on the 01 Deck and 20 failed. There are also 12 instances were cables are passed through stiffener relief cuts or rough holes in bulkheads.

Three of the passing MCTs are block-type MCT’s, 1 is conduit run from a junction box through the deck. The other 14 penetrate non firetight or C-Class boundaries and appear to be packed well enough to sufficiently prevent the passage of fire and smoke.

Twelve of the failed MCT’s penetrate A-Class boundaries. Of these three have collars with sufficient depth to install approved firestop system in accordance with OEM procedure. The current firestop system needs to be removed and the collars should be repacked. The MCTs are 01-32-1- V, 01-39-1.1-V, 01-39-1.3-V.

The other 17 failed MCTs penetrate A-class and B-Class boundaries and all collars will need to be modified or extended.

The most concerning of these is 01-37-1-V. This MCT is located underneath the emergency switchboard and transits in to Dry Lab. Simply extending the collar is not a viable solution. The modification of this MCT will require a substantial amount of work removing interferences.

A majority of the cables that are run through relief cuts can be pulled back and run through nearby MCTs. There are a few areas that are of greater concern and will require the installation of new

MCTs. The transverse joiner bulkhead located at frame 17.5 is an area of concern. The bulkhead is a B-Class boundary and it is punctured in numerous locations for the passage of cables, none of which are in an approved manner. The bulkhead will need to be repaired and proper transit devices installed. The others are located in SR 01-32-2 and 01-36-2. There are a number of cables and ducting run from SR 01-32-2 through the P-way into 01-36-2. Currently there are no transits installed. There is also plastic hose used as ventilation ducting that need to be replaced with an approved alternative.

The remaining failing MCTs contain firestop applications which do not comply with OEM requirements and will need to be repacked assuming the MCT collar meets depth requirements.

1st Deck (Main Deck) Forty-four MCTs were surveyed on the 1st Deck and 34 failed. There are also 14 instances were cables are passed through stiffener relief cuts or rough holes in bulkheads.

The repair and modification suggestions for the main deck can be found in Appendix 3. Below are the MCTs that will require the greatest level of effort to correct

There are two MCTs 1-29-1.3-T and 1-29-1.5-T, that penetrate the A-Class boundary between the galley and the mess deck. Both collars are open, no firestop system is installed. 1-29-1.3-T has no cables running through it.

There are three MCTs that penetrate the deck of the Wet Lab and transit into the Engine Room, 1-48-0-V, 1-48-1-V, 1-49-1-V. All three are located behind joinery bulkheads and will require a significant amount of interference removal to access them. 1-48-1-V is a stuffing tube type transit with a single large cable, it needs to be inspected to ensure a proper seal is intact. The other two are collars that lack sufficient depth for the installation of an approved firestop system.

MCT 1-40-0-L penetrates the A-Class bulkhead between the ET Shop and Main Deck P-way. This MCT lacks sufficient depth for an approved firestop system to be installed and exceeds the 40% cable density threshold. Because of the amount of cabling that is constantly run in and out of the ET Shop for the expansion of the Ships ability’s, I would recommend pulling the cables back and installing a new EZ-Path type cable transit.

It should also be noted at a number of stuffing tubes in the deck of the laundry room are in need of replacement. Furthermore the integrity of the deck should be investigated as it shows signs of severe corrosion.

2nd Deck (Hold Deck) Forty-seven MCTs were surveyed on the 2nd Deck and 10 failed.

Eight of the failed MCTs are watertight and seven have block-type systems in place.

There are three main areas of concern on the hold deck. The spaces between frame 21 and frame 29, the spaces between frame 29 and frame 46, and the conduit through tanks in the Auxilllary Machinery Room (AMS) and Steering Gear Rooms.

The area between frame 21 and 29 is divided into 3 spaces. The boundaries between these three spaces are both required to be A-Class. There are open penetrations in both bulkheads for the passage of cables pipes and ducting. There is no firestop system installed at any penetration in these bulkheads, they are all open.

Similar to the space forward of it the area between frame 29 and frame 46 is divided into three spaces. These spaces are required to be separated by B-Class boundaries. There are open penetrations in all of the bulkheads for the passage of cables pipes and ducting. There is no firestop system installed at any penetration in these bulkheads, they are all open. Furthermore there are cables running through bulkheads with no penetration installed, just a whole cut in the steel portion of the bulkhead.

This is a prime example of why a Regulatory-approved fire boundary plan is needed. In the absence of a fire boundary drawing, SOLAS requires the use of the most restrictive interpretation.

If these spaces were considered one singular space by an approved fire boundary plan then the current configuration would be sufficient. In the absence of the plan all penetrations must be addressed and modified.

The last area of concern on the hold deck is the conduit running between AMS and the steering gear compartments. There are 8 conduit passages that originate at the watertight bulkhead at frame 71 and transverse through the Port and Starboard potable water tanks. The conduit pipe was cut off and left open where it exits the P/W tanks. Conduit is required to be sealed water and gastight at both ends. These conduit pipes need to be capped or cropped out and the tank boundary’s inserted.

MCT 2-83-1.1-T originates in the Stbd Steering Gear compartment and transverses, via steel pipe conduit, through watertight bulkhead at frame 83, Void 2-80-1 and terminates in Passage 2-71-3.

The forward end of the conduit at Frame 80 is packed with putty and is not acceptable. A RoxTec Type R 150 MCT need to be installed. The R 150 will be large enough to accommodate the cable load.

There is a similar situation located in the Port Steering gear compartment. There is steel pipe conduit running from the Port Steering Gear compartment to Store Room 2-71-4. Both ends of the conduit need to be addressed. The aft end has a single stuffing tube with multiple cables running through it and the forward end is poorly packed with putty. Neither of which are acceptable.

Also of note is Stuffing Tube 2-71-2-T. A large stuffing tube installed in watertight bulkhead frame 71 port side just outboard of the potable water tank. The penetration is wide open.

Conclusion and Recommendations During the survey, it was determined a m a j o r i t y of the MCTs do not meet Regulatory and/or OEM requirements when compared to SOLAS and ABS Regulations and existing watertight boundary drawings.

A Regulatory-approved fire boundary drawing does not appear to exist for the Nancy Foster.

In order to conclusively determine/confirm firetight integrity requirements of various bulkheads throughout the ship, and therefore determine which MCTs require what levels of firestop systems, it is recommended as a first step a fire boundary drawing be created and submitted for Regulatory approval. Once complete, the full scope of MCT renewals can be determined. In conjunction with this effort, the Fire Control Plan should be updated.

MCTs with firestop material might pass ABS visual inspection or even a soap-and-blow test but are still not in compliance with OEM/Regulatory requirements. The majority of firetight MCT frames are not deep enough to support the installation of any firestop system for an A or B-Class fire boundary. In such instances, the MCTs will need to be rebuilt or collar extensions fabricated/installed. The full extent of renewals required cannot be determined until a Regulatory-approved fire boundary drawing is completed.

The documents in the appendix have been created to help identify deficiencies and facilitate repairs. They have not been reviewed or approved by any regulatory body. Repair recommendations are made with cost, time and future expansion in mind.
