Attn: Kristin Walker, Civil Engineer

RE: Hazardous Materials Survey Report 25 3rd Avenue Ext., West Haven, Connecticut

Dear Ms. Walker:

Thunderbird Environmental, LLC (Thunderbird) is pleased to submit this hazardous materials survey report to the Natural Resources Conservation Service (NRCS) for the above referenced property located at 25 3rd Avenue Extension in West Haven, Connecticut (herein after referred to as “the Site”). This report presents the findings of our property inspection and summarizes the observed presence of potentially hazardous/regulated substances including: asbestos containing materials (ACM), lead-based paint, heating oil tanks, mercury and polychlorinated biphenyl (PCB) containing materials, pressure treated wood, mold, and other miscellaneous hazardous materials (paint, automotive fluids, household cleaners, etc.). In addition, Thunderbird contracted laboratory analysis of potential ACM and lead-based paint. This report also provides estimated quantities of hazardous/regulated substances, but does not quantify removal/disposal costs.

1.0 Site Description

The Site is comprised of an approximately 0.13-acre residential property located on the southeast side of 3rd Avenue Ext. in West Haven, Connecticut (Figure 1). The site includes an approximately 2,544 square-foot, two-story residence with an unfinished basement. According to the City of West Haven Tax Assessor’s database, the building was constructed in 1920.

The site building is divided into two separate residential apartments including one on the first floor and one on the second floor. The building is constructed with wood framing on a mortared concrete block and stone foundation. The exterior is clad with vinyl siding over wood siding. The basement contained one, recently-installed gas-fired boiler (see photos) and miscellaneous maintenance-related containers as further described in this report. The first floor apartment includes two bedrooms, one bathroom, a kitchen, and a living room. The second floor apartment includes one bedroom, one bathroom, a galley kitchen and a living room.

2.0 Asbestos Survey

Thunderbird completed an asbestos survey of the site building on January 8, 2020. The survey was performed by Martin Brogie, LEP, licensed as an Asbestos Inspector by the Connecticut Department of

Public Health (Certificate Number 275).

The survey consisted of a National Emissions Standards for Hazardous Air Pollutants (NESHAP) assessment that included a thorough inspection of the subject building components/areas in which the location and number of samples collected, using limited destructive testing, provided sufficient detail to estimate the quantity and location of ACM located in the study areas. The federal NESHAP sampling protocols (40 CFR

61.145), utilized for the asbestos assessment of structures, equipment and buildings to be demolished, requires the following:

“…the owner or operator of a demolition or renovation activity and prior to the commencement of the demolition or renovation, thoroughly inspect the affected facility or part of the facility where the demolition or renovation operation will occur for the presence of asbestos…”

The intent of these federal regulations is to require suspect ACM which will be disturbed during the demolition or renovation (including repair), be assessed for asbestos content (defined as greater than 1% asbestos) and, if identified, be removed and disposed of properly. The asbestos survey utilized both statistically random and discrete sampling procedures during the assessment to find safely accessible sampling locations as well as ensuring that as many suspect building materials were sampled as possible.

Samples were collected using a utility knife and other hand tools which were decontaminated in between samples to avoid cross-contamination. Collected samples were placed in plastic sample bags, labeled with a unique identification number, documented in field notes and on the chain-of-custody documents. Upon completion of sampling activities, field renderings of the survey areas were created which displayed the sample locations. Samples were shipped under chain of custody to International Asbestos Testing

Laboratories, Inc. in Mount Laurel New Jersey for asbestos analysis (Polarized Light Microscopy).

Sampling locations are depicted Figures 2A, 2B, 2C, and 2D. Copies of the laboratory analytical reports are provided as Attachment A. Select photos are included as Attachment B. The table below summarizes the sampled materials and the asbestos analysis results.

3. P.C. – Asbestos Content by Point Count Method

Asbestos was detected in two (2) materials sampled throughout the residence including: Dark Brown Linoleum With Black Backer Below Particle Board And Plywood (Bedroom 1) and Brown Floor Tile With Black Backer Below Plywood And Sheetrock (Bedroom 1).

The estimated ACM quantity for the Site is 240 square feet.

No samples were collected from the single, gas-fired boiler as it appeared to be very recently installed.

No suspect ACMs were noted in association with the boiler other than flue cement.

Asbestos abatement will be necessary prior to building demolition.

3.0 Lead-Based Paint Testing

The disposal of building materials requires appropriate sampling methods to correctly characterize the proportion and makeup of representative building materials that will be removed during demolition. The Connecticut Department of Energy and Environmental Protection (DEEP) has released a method that states samples for waste characterization are to be representative of the percentage of each building material as it makes up the building. For example, if the building is 50 percent brick, 40 percent concrete, 5 percent wood, and 5 percent metal. The representative 100 gram collected sample would be the same makeup percentage.

Thunderbird staff collected a representative sample of the building at the Site for waste characterization.

The sample was tested for leachable lead via the Toxicity Characteristic Leaching Procedure (TCLP) analysis method as required for waste disposal approval. The sample was a composite of concrete, wood, metal, glass, paint, and other building materials that were in representative portion to their total makeup of the entire building. Based on the TCLP analytical results, leachable lead was not reported in the sample above the laboratory method detection limit of 0.10 milligrams per liter (mg/L) and solid waste debris derived from the demolition of the site building would not be classified as hazardous waste due to lead content.

4.0 Aboveground Storage Tanks (ASTs) and Underground Storage Tanks (USTs)

According to the City of West Haven Tax Assessor’s database, the Site building is heated with natural gas.

A gas-fired boiler was observed in the basement. The boiler appeared to be less than five years old. No fill or vent pipes were observed along the exterior of the Site building.

5.0 Mercury and PCB Containing Materials

As part of the Site inspection on January 8, 2020, Thunderbird observed a thermostat within in the second floor living room. The thermostat has the potential to contain minimal amounts of liquid mercury. There was no other evidence of potential mercury containing material at the Site.

Thunderbird did not observe any potential PCB containing material at the Site.

6.0 Pressure Treated Wood

Pressure treated wood was present at the entrances to both floors of the building. The first floor porch is constructed of pressure treated wood and is approximately 30 s.f. The second story porch is also constructed of pressure treated wood and is approximately 6 s.f.

7.0 Mold

No significant mold was noted in the site building.

8.0 Other Potentially Hazardous Material

Several cans of tile adhesive and paint were observed in the attic.

9.0 Conclusions and Recommendations

Thunderbird Environmental, LLC (Thunderbird) has performed a pre-demolition hazardous materials assessment for the property located at 25 3rd Avenue Extension in West Haven, Connecticut. The assessment included an asbestos survey, lead-based paint sampling, and an evaluation of the presence of other miscellaneous hazardous/regulated materials that may require special disposal or handling prior to, or during demolition.

Based on the findings of the survey, asbestos abatement is required prior to building demolition.

Miscellaneous household/maintenance chemicals and paints should be removed from the residence and disposed and/or recycled prior to demolition. The amount of household/maintenance materials requiring removal is not anticipated to exceed 100 kilograms.

Based on the TCLP analytical results, leachable lead was not reported in the sample above the laboratory method detection limit of 0.10 milligrams per liter (mg/L) and solid waste debris derived from the demolition of the site building would not be classified as hazardous waste due to lead content.

In accordance with Connecticut Department of Energy and Environmental Protection (DEEP) guidelines, disposal of the pressure treated wood should be handled as follows:

• Contractors, utilities, and manufactures should contract directly with a DEEP permitted bulky waste landfill, or send it to an out-of-state wood burner facility.

The identified thermostat should be disposed of in accordance with Connecticut Public Act 12-54.

Dust control measures should be implemented when handling demolition debris.

10.0 Limitations

Suspected ACM observed during the hazardous materials survey were sampled and analyzed for asbestos.

While our inspections are comprehensive, there may be limited areas, such as areas between walls and beneath structures, which were inaccessible and therefore not inspected or tested.

All estimates of materials provided are based on our observations made at the time of our inspection and should be confirmed by abatement contractors.

We appreciate the opportunity to assist NRCS with this project. If you have any questions or comments, please contact us at 860-227-4714.

Sincerely, Darby W. Hittle, L.E.P. Martin Brogie, L.E.P.

Information Pertinent to this Report:

Analysis by US EPA 600 93-116: Determination of Asbestos in Bulk Building Materials by Polarized Light Microscopy (PLM).

Certifications:

NIST-NVLAP No. 101165-0• NYSDOH-ELAP No. 11021• AIHA-LAP, LLC No. 100188•

Quantification at <0.25% by volume is possible with this method. (PC) Indicates Stratified Point Count Method performed. (PC-Trace) means that asbestos was detected but is not quantifiable under the Point Counting regimen. PC Trace represents a <0.25% amount. Analysis includes all distinct separable layers in accordance with EPA 600 Method. If not reported or otherwise noted, layer is either not present or the client has specifically requested that it not be analyzed (ex. analyze until positive instructions). Small asbestos fibers may be missed by PLM due to resolution limitations of the optical microscope. Therefore, PLM is not consistently reliable in detecting asbestos in non-friable organically bound (NOB) materials. Quantitative transmission electron microscopy (TEM) is currently the only method that can pronounce materials as non-asbestos containing.

Analytical Methodology Alternatives: Your initial request for analysis may not have accounted for recent advances in regulatory requirements or advances in technology that are routinely used in similar situations for other qualified projects. You may have the option to explore additional analysis for further information. Below are a few options, listed as the matrix followed by the appropriate methodology. Also included are links to more information on our website.

Bulk Building Materials that are Non-Friable Organically Bound (NOB) by Gravimetric Reduction techniques employing PLM and TEM: ELAP 198.6 (PLM-NOB), ELAP 198.4 (TEM-NOB)

Disclaimers / Qualifiers:

There may be some samples in this project that have a "NOTE:" associated with a sample result. We use added disclaimers or qualifiers to inform the client about something that requires further explanation. Here is a list with highlighted disclaimers that may be pertinent to this project. For a full explanation of these and other disclaimers, please inquire at customerservice@iatl.com.

1) Note: No mastic provided for analysis.

2) Note: Insufficient mastic provided for analysis.

3) Note: Insufficient material provided for analysis.

4) Note: Insufficient sample provided for QC reanalysis.

5) Note: Different material than indicated on Sample Log / Description.

6) Note: Sample not submitted.

7) Note: Attached to asbestos containing material.

8) Note: Received wet.

9) Note: Possible surface contamination.

10) Note: Not building material. 1% threshold may not apply.

11) Note: Recommend TEM-NOB analysis as per EPA recommendations.

12) Note: Asbestos detected but not quantifiable.

13) Note: Multiple identical samples submitted, only one analyzed.

14) Note: Analyzed by EPA 600/R-93/116. Point Counting detection limit at 0.080%.

15) Note: Analyzed by EPA 600/R-93/116. Point Counting detection limit at 0.125%.

16) Note: This sample contains >10% vermiculite mineral. See Appendix for Recommendations for Vermiculite Analysis.

Recommendations for Vermiculite Analysis:

Several analytical protocols exist for the analysis of asbestos in vermiculite. These analytical approaches vary depending upon the nature of the vermiculite mineral being tested (e.g. un-processed gange, homogeneous exfoliated books of mica, or mixed mineral composites). Please contact your client representative for pricing and turnaround time options available.

iATL recommends initial testing using the EPA 600/R-93/116 method. This method is specifically designed for the analysis of asbestos in bulk building materials. It provides an acceptable starting point for primary screening of vermiculite for possible asbestos.

Results from this testing may be inconclusive. EPA suggests proceeding to a multi-tiered analysis involving wet separation techniques in conjunction with PLM and TEM gravimetric analysis (EPA 600/R-04/004).

For New York State customers, NYSDOH requires disclaimers and qualifiers for various vermiculite containing samples that direct analysis via ELAP198.6 and ELAP198.8 for samples that contain >10% vermiculite mineral where ELAP198.6 may be used to evaluate the asbestos content of the material. However, any test result using ELAP198.6 will be reported with the following disclaimer: “ELAP198.6 method does not remove vermiculite and may underestimate the level of asbestos present in a sample containing >10% vermiculite.”

Further information on this method and other vermiculite and asbestos issues can be found at the following: Agency for Toxic Substances and Disease Registry (ATSDR) www.atsdr.cdc.gov, United States Geological Survey (USGS) www.minerals.usgs.gov/minerals/, US EPA www.epa.gov/asbestos. The USEPA also has an informative brochure "Current Best Practices for Vermiculite Attic Insulation" EPA 747F03001 May 2003, that may assist the health and remediation professional.

The following is a summary of the analytical process outlines in the EPA 600/R-04/004 Method:

1)Analytical Step/Method: Initial Screening by PLM, EPA 600R-93/116 Requirements/Comments: Minimum of 0.1 g of sample. ~0.25% LOQ for most samples.

2)Analytical Step/Method:Wet Separation by PLM Gravimetric Technique, EPA R-04/004 Requirements/Comments: Minimum 50g** of dry sample. Analysis of "Sinks" only.
