ITEM REFERENCE AND REMARKS ~ NARRATIVE REPORT
Alignment - The left end of Bent 4 is not centered under the transverse joint between Spans 3 and 4. Span 4 only bears on very little of the bearing plate and cap as a result. (Photos 16 through 19) The right end is centered and Channel Units 4, 5, and 6 have near equal bearing; however Channel Units 1 and 2 are very near slipping off of the cap. These Channel Units have settled 1" as evidenced by misalignment of the curb and deck. (Photos 20 and 21) This structure should be closed until Span 4 is adequately supported at Bent 4.
Erosion of Scour, Channel Scour / Erosion - The stream bank is scouring upstream of Span 1 and under Spans 1 and 2 (Photo 22). The streambed is scouring around Bent 3. (Photo 23)
Settlement - Previous inspection reports indicate that Bent 2 had settled slightly. It now appears that Bents 2 and 3 may have settled, but it is difficult to determine whether the bents have settled or other bents were constructed at a higher elevation.
Caps - The cap at Bent 2 is decaying at its core. Sounding, drilling, and probing indicate that it is hollow in its entire length; however, there were no signs of crushing at the time of the inspection. (See Sketch) It also has external decay in areas where checks in the timber are retaining moisture. These decayed areas are only ¼” to ½” in depth at this time. (Photo 24) The bridge is in CRITICAL condition due to the decay of this cap.
Remarks
Deck Slab / Panels / Joists - The deck surface is becoming worn. Aggregate is exposed. (Photos 1 and 2) Some of the deck panels have minor concrete breakage at the corners and edges on the surface. (Photos 1 and 2) Expansion Joints - The deck joints are sealed with what appears to be silicone. The seals no longer bond to the concrete properly in some areas. (Photo 3) Curbs - The curbs have been patched on the right side of Span 1 at the near end and on the left side of Span 5 at the far end. The patch and curb have broken on the right side of Span 1 resulting in the bridge rail post being detached from the bridge. (Photo 4) The curb on the left side of Span 1 has a 6"x8"x2" spall with exposed reinforcing steel on the near end. (Photo 5) Drains and Drainage, Cleanliness - An accumulation of sand, gravel, and vegetation in the gutterlines prevents proper drainage. (Photo 6)
Girders - Span 1, Girder 4 has a 20" diagonal crack at the bearing on Bent 2. (Photo 7) Span 2, Girder 1 has a 4’ to 5’ diagonal crack that originates at the end approximately 8” above the bearing on bent 3 and extends to the bottom of the stringer. (Photo 8) Span 2, Girder 9 has a 12" diagonal crack at the bearing on Bent 2. (Photo 9) Several of the girders are spalling adjacent to steel plates embedded in the bearing area. (Photos 10 and 11) Corrosion has caused incidental, unreinforced grout that had accumulated between the plates and the forms during the casting process to break away.
Bearing Devices - The steel bearing plates are severely corroded (Photos 12 and 13), but continue to perform the function for which they were intended. Span 5, Girder 12 does not bear properly on the cap at Bent 5. The concrete has spalled away from the embedded, steel plate which appears to have been set too deeply into the stringer. The stringer now bears on a 3”x1"x.25” steel plate that was welded to the embedded plate. (Photos 14 and 15) This bearing is inadequate and should be repaired properly.
Waterway Obstructions / Drift - Trees and brush that were cut immediately upstream of Span 2 were left lying in the waterway. (Photos 28 and 29) Remove a tree from the upstream side of the bridge that is leaning heavily toward Span 1. (Photo 30)The channel is littered with fallen trees and debris on both sides of the bridge.
Bridge Railing - The bridge rail on the left side of Spans 3, 4, and 5 and on the right side of Spans 4 and 5 was previously replaced. The splices in the new railing do not all align with a post. The bridge rail posts are not of sufficient size and the spacing is excessive. One of the bridge rail posts on the right side of Span 1 is no longer attached to the bridge as a result of damage to the curb. (Photo 4)
Signing - "Weight Limit 3 Tons" signs have been installed 50' to 60' from Abutments 1 and 6. These signs should be nearer to the bridge. There is a “One Lane Bridge” sign in advance of the bridge at the far end. The "One Lane Bridge" sign is missing from the near end. The “One Lane Bridge” sign at the far end of the bridge has become illegible and needs to be replaced. It is leaning slightly, but remains visible.
Piles - Bent 2, Piles 1 and 2; Bent 3, Pile 2; and Bent 5, Pile 2 are decaying internally. (See Sketches) Bent 5, Pile 2 is also decayed to a depth of 3" at the ground line (Photo 25) and should be replaced. Some piles have checking up to .25" in width. (Photo 26) New piles were driven to replace Bent 4, Piles 1, 2, and 3. Bent 5, Pile 1 has been spliced. (Photo 27)
