About this file

This document is an attachment to a Request for Proposals issued by the State of Utah Division of Purchasing on behalf of the Department of Corrections for urinalysis laboratory equipment. The RFP seeks vendors to provide urinalysis laboratory equipment according to the specified scope of work, with an anticipated single contract award to the highest-scoring responsive and responsible vendor. Offerors are required to complete a mandatory site visit signup sheet and submit it to klucus@utah.gov no later than 2:00 PM MST on September 23, 2026. All offerors must attend an in-person site visit on Thursday, October 15, 2026, at the Utah State Correctional Facility in Salt Lake City, beginning at 10:00 AM local time; attendance is mandatory, and offerors not arriving on time will be required to leave. All attendees must successfully pass a background check to gain facility entry. Vendors must adhere to all specifications in the scope of work, supply all required reagents within five days of order receipt, and ensure that testing sensitivity for all drugs meets or exceeds National Institute on Drug Abuse standards. The procurement will process applications in two stages: an initial review of responsiveness, followed by technical proposal evaluation.

The technical evaluation phase uses 800 total possible points across six criteria: training (150 points), service and maintenance (200 points), installation (150 points), work history (50 points), subcontractor compliance (150 points), and software system capabilities (100 points). Offerors must achieve a minimum score of 560 points, or 70 percent of total points possible, to advance to the cost proposal evaluation stage. Proposals scoring below the minimum technical threshold will be deemed non-responsive and eliminated from further consideration. The evaluation will assess vendors' ability to train urinalysis technicians on equipment operation and maintenance, provide ongoing support and maintenance for approximately 600 tests per week, complete installation within a specified timeframe, demonstrate experience in high-volume urinalysis labs in correctional or forensic environments, ensure subcontractor compliance, and offer a robust software system with data export capabilities and network compatibility.

Describe in detail the training that will be needed to initially begin lab operations, including the length of time needed. Also describe any training that can be offered to new UA techs upon request, and any associated costs. (150 pts.)

2. Service/Maintenance - What maintenance steps will be required of UA techs to run the machine, and how frequently will each task be required? Assume an average rate of running ~600 tests per week on a single machine. What aspects of regular maintenance, if any, will be performed by your company/subcontractor? Attach any sample maintenance schedules. (200 pts.)

3. Installation - Describe in detail your plan to complete installation of the machine. What is the maximum number of days that will be required to complete the installation? Include descriptions of any proposed/required modifications to the lab that will be needed to complete installation. What is the maximum number of days required to initially train UA techs to get the lab running again? (150 pts.)

4. Work History - Provide examples of any other high-volume UA labs which use the machine proposed in your RFP, especially those in correctional/forensic environments. When was installation completed, and what is the approximate number of tests run per week? (50 pts.)

5. Subcontractor Compliance - Describe which, if any, parts of the scope of work for this contract that your company plans to utilize subcontractors to complete. What is your company’s plan for ensuring subcontractor compliance with the requirements of the scope of work?
