About this file

This is a Site Visit Sign-up Sheet document related to an RFP for Urinalysis Laboratory Equipment issued by the State of Utah Division of Purchasing through the Department of Corrections. The site visit is mandatory for all vendors submitting bids, with each vendor required to have at least one representative present at the Utah State Correctional Facility (USCF) for their proposal to be considered. The site visit is scheduled for October 15th, 2026, beginning promptly at 10:00 a.m. at the Stephen R. Anderson Building located at 1480 N 8000 W, Salt Lake City, UT 84116. Each vendor may designate no more than two individuals to participate in the site visit, which is expected to last approximately three hours. Vendors must complete and submit the site visit sign-up sheet containing participant information (full name as stated on driver's license, date of birth, driver's license number, and issuing state) to klucus@utah.gov by 2:00 p.m. on September 23rd, 2026. Failure to meet this deadline will result in exclusion from consideration.

All site visit participants are subject to a background check conducted by the Utah Department of Corrections to determine eligibility to enter the facility premises, with eligibility determined solely at UDC's discretion. Participants are prohibited from bringing electronic devices including phones, cameras, and smartwatches onto USCF premises but may bring pen/pencil, notepads, tape measures, and other analog tools that pass through standard parcel scanners. The document advises that inmates will be present during the site visit as participants will walk through the medical building during active hours, and UDC assumes no liability for time lost due to unforeseen circumstances such as emergency lockdowns. Any failure to fully disclose required information or comply with UDC staff directions will result in termination of the site visit and vendor disqualification from the solicitation.

USCF Site Visit Sign-up PLEASE READ THE FOLLOWING TERMS BEFORE FILLING OUT THE TABLE FOUND ON PAGE TWO (2)

· Each vendor submitting a bid for this solicitation will be REQUIRED to have at least one (1) representative participate fully in the site visit of the Utah State Correctional Facility (USCF) for their bid to be considered. Each vendor is allowed no more than two (2) individuals to enter USCF.

· The site visit is scheduled to take place on October 15th, 2026. The site visit will begin promptly at 10:00am. Any individuals not present at the meeting point (see below) before 10:00am will not be permitted to participate in the site visit, and will be excluded from consideration for this solicitation. The site visit should last no longer than three (3) hours. Comment by Kevin Lucus: Do you anticipate 2 or 3hrs?

· Vendors will meet inside the front lobby of the Stephen R. Anderson Building, located at 1480 N 8000 W, Salt Lake City, UT 84116.

· All individuals who apply for clearance to participate in the site visit will be subject to a background check to determine eligibility to enter USCF premises. If an individual fails the background check or is determined ineligible to enter USCF, that individual will not be permitted to participate in the site visit. Eligibility to enter USCF is determined at the sole discretion of the Utah Department of Corrections (UDC). Failure to fully disclose any information requested by this document will result in termination of the site visit and the associated vendor’s disqualification from participating in the solicitation.

· Individuals participating in the site visit are advised of the risks associated with entering USCF. Inmates will be present on the premises, as the individuals participating in the site visit will need to walk through the medical building during active hours. UDC will not be held liable for any time lost due to unforeseen circumstances such as emergency lockdown of the prison facility.

· Participants in the site visit will not be permitted to bring any electronic devices onto USCF premises, including phones, cameras, or smartwatches. Leave all such other devices in the car. Failure to follow these rules and any other directions of UDC staff will result in termination of the site visit and the associated vendor’s disqualification from participating in the solicitation. You may bring pen/pencil, notepad, tape measure, or other analog tools that can easily stay on your person and pass through a standard parcel scanner.

· As it is difficult to obtain clearances to enter the prison facilities, all vendors interested in this solicitation must complete the following table and email the completed document to klucus@utah.gov before 2:00pm on September 23rd, 2026. Only one document submission is required per bidder. Failure to submit the document before the deadline will result in exclusion from consideration for this solicitation.
