About this file
This is a Proposer's General Questionnaire template issued by the University of Texas Health Science Center at San Antonio (UTHSCSA) Department of Purchasing for an RFP soliciting qualified proposers to provide revenue cycle services for the Oral Maxillofacial Services (OMS) department. The services encompass both medical and dental coding, billing, and collections operations. UTHSCSA seeks to outsource OMS revenue cycle operations to maximize workflow efficiency, particularly for medical claims processing, as current internal bandwidth and expertise are insufficient. The questionnaire requires proposers to provide detailed information across multiple sections including company profile, service approach, general requirements, service support, quality assurance, and miscellaneous offerings.
Proposers must complete comprehensive sections detailing their business background, financial stability, customer references, key personnel qualifications, work plans with timelines and milestones, reporting capabilities, quality assurance programs, and contingency or disaster recovery plans. The questionnaire mandates submission of at least three customer references from similar service engagements within the past five years, summary resumes of key personnel, disclosure of ownership interests, and clarification of any anticipated difficulties in performance. Proposers must also declare their approach to cloud computing services and identify any additional benefits or unique services offered. UTHSCSA explicitly states there is no guarantee of volume, minimum, or maximum compensation under this RFP or any resulting contract agreement. The questionnaire serves as a critical evaluation tool to assess proposer qualifications, experience, financial viability, and capacity to manage OMS revenue cycle operations effectively.
