COMMERCIAL MAINTENANCE PROGRAM BUSINESS RULES

The purpose of this document is to ensure that all data entries are standard across the region. When closing out maintenance actions for both One Time Maintenance (OM) and Preventative Maintenance

(PM), all applicable data fields must be filled out with the proper information, i.e. Inventory parts, part costs, labor hours, etc. Below are examples of the data entries required for specific routine reports and inspections, along with labor hour accounting and other data collection requirements.

DOCUMENTS

When scanning documents and attaching them to a Maintenance / Service Action those documents must be in PDF Format. When adding Pictures to a Maintenance / Service Action, those pictures also need to be in PDF format. Ensure documents are oriented to allow them to be read without having to readjust after opening.

DAILY BOAT REPORT

Daily Boat reports are the responsibility of the crew that is operating the vessel. If the Contractor has the responsibility for vessel operations, the Contractor must complete the Daily Boat Report for the vessel that is being operated. The Signature blocks for “Inspected By” and “Reviewed By” must also be signed. Once Contractor personnel has completed the Daily Boat Report and has scanned and uploaded the report into the Commercial Maintenance Program, the Report must be turned over to the Port

Operations Officer for his or her review and signature. There is no requirement for the Contractor to collect the document after signature, as the document is being turned over to the customer for situational awareness and for their records. The Contractor is not responsible for tracking daily boat reports completed by US Navy personnel. Daily Boat reports completed by US Navy Personnel are maintained within “Fleet Two” in the Commercial Maintenance Program.

For vessels operated by Contractor Personnel, the data entries shall be as follows:

No discrepancies noted-

“Daily Boat Report Completed by Contractor Personnel, inspection results completed SAT, no discrepancies noted. Completed Daily Boat Report document has been turned over to the Port

Operations Officer.”

New discrepancies noted-

“Daily Boat Report Completed by Contractor Personnel, inspection results completed SAT, discrepancies noted, and OM created. Completed Daily Boat Report document has been turned over to the Port Operations Officer.”

This entry will be used whenever there is a discrepancy noted during the execution of the Weekly

System Test and Material Condition Inspection, but those discrepancies do not force the craft to be placed out of service.

Create the necessary OM(s) and document in notes section of the Daily Boat Report utilizing the OM number and title.

Discrepancies previously noted/no new discrepancies-

“Daily Boat Report Completed by Contractor Personnel, inspection results completed SAT, no new discrepancies noted. Completed Daily Boat Report document has been turned over to the

Port Operations Officer.”

This entry will be used whenever a Daily Boat Report contains a discrepancy that has been previously reported but not yet corrected. When filling out the Daily Boat Report, document any previously reported discrepancies that have not been corrected in the notes section of the document with the OM number and title of open discrepancies.

Discrepancies that take the craft out of service-

“Daily Boat Report Completed by Contractor Personnel, inspection results completed UNSAT, discrepancies noted, and OM created. Completed Daily Boat Report document has been turned over to the Port Operations Officer.”

This entry will be used when there is a discrepancy noted during the execution of the daily boat report and those discrepancies do force the craft to be placed out of service.

Create the necessary OM(s) and document in notes section of the Daily Boat Report utilizing the OM number and title.

When scanning Daily Boat Reports to accompany the Commercial Maintenance Program check, the file name must be in the following format:

Hull Number – Daily Boat Report – Date

“27HS1203 Daily Boat Report 04 Jan 2019”

When a boat is removed from the water and placed on a trailer, the report requirements shift from Daily

Boat Reports to the Weekly System Test and Material Condition Inspection. The same reporting criteria applies, each craft gets an inspection weekly while it is on the trailer and those inspections will be conducted by Contractor Employees.

When a vessel is removed from service, the Daily Boat Report shall be turned off in the Commercial

Maintenance Program and the Weekly Test and Material Condition Inspection shall be turned on. The reverse shall occur when the vessel is placed back into service.

WEEKLY SYSTEM TEST AND MATERIAL CONDITION INSPECTION

The intent of the Weekly System Test and Material Condition Inspection is to show our customer that we are actively reviewing each vessel at least once a week.

Regardless of whether the vessel is waterborne or on a trailer, the Contractor is responsible for conducting Weekly System Test and Material Condition Inspection for ALL vessels assigned to the South

East Regional Port Operations Contract. An exception to this rule, are vessels that the Contractor solely operates and performs daily reports on. The Weekly System Test and Material Condition Inspection will be completed no later than Tuesday of each week.

The signature blocks for “Inspected By” and “Reviewed by” must also be signed. Once the Contractor has completed the Weekly System Test and Material Condition Inspection and has uploaded the document into the Commercial Maintenance Program, the inspection form must be turned over to the

Port Operations Officer for that site for his or her review and situational awareness. There is no requirement for the Contractor to collect the document after signature, as the document is being turned over for the Navy’s situational awareness and for their records.

The Commercial Maintenance Program data entry for Weekly System Test and Material Condition

Inspection performed by the Contractor will be as follows:

No discrepancies noted-

“Weekly system test and material condition inspection completed SAT, no discrepancies noted.

Completed Weekly System Test and Material Condition Inspection document has been turned over to the Port Operations Officer.”

New discrepancies noted-

“Weekly system test and material condition inspection completed SAT, discrepancies noted, and

OM created. Completed Weekly System Test and Material Condition Inspection document has been turned over to the Port Operations Officer.”

This entry will be used whenever there is a discrepancy noted during the execution of the Weekly

System Test and Material Condition Inspection, but those discrepancies do not force the craft to be placed out of service.

Create the necessary OM(s) and document in notes section of the Weekly System Test and Inspection

Report utilizing the OM number and title.

Discrepancies previously noted/no new discrepancies-

“Weekly system test and material condition inspection Completed SAT, no new discrepancies noted. Completed Weekly System Test and Material Condition Inspection document has been turned over to the Port Operations Officer.”

This entry will be used whenever a Weekly System Test and Material Condition Inspection contains a discrepancy that has been previously reported but not yet corrected. When filling out the Weekly

System Test and Material Condition Inspection, any previously reported discrepancies that have not been corrected must be listed in the notes section of the Document with the “OM” number attached.

Discrepancies that take the craft out of service-

“Weekly System Test and Material Condition Inspection completed UNSAT, Discrepancies Noted, OM created. No parts required. Completed Weekly System Test and Material Condition

Inspection document has been turned over to the Port Operations Officer.”

This entry will be used when there is a discrepancy noted during the execution of the daily boat report and those discrepancies do force the craft to be placed out of service.

Create the necessary OM(s) and document in notes section of the Daily Boat Report utilizing the OM number and title.

Vessel out of service for repairs-

Vessels that are undergoing a routine or emergency overhaul will have maintenance turned off for the duration of overhaul, no reports or inspections will be completed during this time.

When scanning the weekly system test and material condition inspection the file name must be in the following format:

Hull Number – Weekly System Test and Material Condition Inspection – Date

“27HS1203 Weekly System Test and Material Condition Inspection 04 Jan 2019”

MAINTENANCE MATERIAL PROJECTIONS

Preventative maintenance (PM) material will be maintained at a high limit of 120 days. PM material projections will be ordered within the first two weeks of the month. Material should be ordered according to the following example:

RPO’s for PM Material required in January should be submitted within the first two weeks of October.

For calendar-based PM’s, i.e. (semi-annual, annual) order only enough to perform the PM for that period.

One-time (OM) material that has high usage will be considered on a case by case basis for keeping in stock.

HOURLY DRIVEN MAINTENANCE

Hourly driven maintenance must be completed no later than 10 hours after the maintenance has become due. Proper forecasting should ensure that maintenance is identified and scheduled before it alerts. If a Commands mission does not allow for the craft to be turned over for maintenance, that information needs to be noted in the “Ship’s Log” in the Commercial Maintenance Program. It also needs to be noted in the “Systems Log” once the maintenance has been rescheduled.

When completing an hourly driven maintenance PM in conjunction with other hourly driven maintenance, parts used must be listed with the check that called for the part. For example, when completing a 400 hour maintenance the check requires that a 200 and 100 hour check to be completed in conjunction with the 400 hour check. Each check has requirements and parts that are specific to that check. When closing these checks, parts used must be attached to the check that required the part.

SCHEDULED PLANNED MAINTENANCE

Scheduled PM’s must be completed and closed out prior to the end of the calendar week in which they have alerted. If these maintenance checks cannot be completed on schedule and are rescheduled, a reason must be provided for each check in the “Systems Log”.

All checks that must be rescheduled or carried over MUST be approved by the Regional Maintenance

Manager or the Commercial Maintenance Program Manager.

A calendar week starts on Monday and Runs through the following Sunday. Maintenance that alerts on

Saturday or Sunday MUST be completed before close of business Friday of that calendar week.

ONE TIME MAINTENANCE TASKS

OM’s entered into the system will have a default “Due date” that represents the date the action was entered into the system. If an OM’s due date needs to be set to a later period, then prior approval from the Regional Maintenance Manager or the Commercial Maintenance Program Manager is necessary.

Examples of this are long lead times for parts or scheduling for overhauls. Proper scheduling and planning will ensure OM’s are completed in a timely manner. Contract requirements require all OM’s to be completed within 30 days of the “Due Date”. Repair parts must be ordered within two days of creating the OM.

When creating a One Time Maintenance Task, the Operation line must contain an accurate, brief description of what the problem is (why the work is required). This is very important in trend analysis.

The “Procedure” data field should contain enough information to accurately explain the procedure to be used to correct the identified discrepancy.

When closing out a “troubleshoot and repair” OM, A detailed description of what was done must be provided in the notes data field.

When creating an OM to repair damage to the Vessel’s Hull or a piece of equipment, attach photos of the damage to the newly created OM.

PERFORMED BY

When closing Maintenance/Service Actions the “Performed by” section must have data in it.

For tasks that are completed by Contractor personnel, include that person’s name in the "Performed By" section. If the task requires more than one person to complete, include all names in the “Notes” section with the supervisor’s name in the “Performed by” section.

Example: Last Name First Initial: Smith T

For Tasks that are completed by sub-contractors, note the sub-contractors company name in the

“performed by” section.

For sites that perform waterborne operations, when closing that evolution, the following information must be included in the notes:

Underway Time (Start time Stop Time).

Work being performed.

Specific vessel hull number (not Local designation).

Number of Personnel on board.

Names of personnel onboard

Example: Requirement to open security barrier system:

0800 – 0900 Opened Security barrier, 30WB1234 3POB, 30WB2345 3POB, evolution time 1 hour. List names of personnel involved; last name first initial.

For the “Actual Job Hour” data field, the total man hours expended to complete the task will be listed.

The time must match the start and stop time listed in the notes.

ACTUAL JOB HOURS

Actual job hours will be captured using the conversion table in enclosure two.

Proper job hour accounting is vital. When closing maintenance actions, the hours listed in the “Actual

Job Hours” field will be total man hours expended to complete the PM or OM task.

Example: If a maintenance task requires two personnel to complete and evolution was one hour long, the actual job hours will be listed as two hours.

RESCHEDULING MAINTENANCE TASKS

From time to time it becomes necessary to reschedule a maintenance task. When this happens, authorization must be obtained from the Contracting Officers Representative (COR) via the Regional

Maintenance Manager or the Commercial Maintenance Program System Manager prior to the task being rescheduled. The task will only be rescheduled from the “Rescheduled Task” tab and the reason for rescheduling must be included.

Maintenance tasks that are turned off because they are “As Required” checks need not be turned on to complete. The task can be completed from the equipment maintenance tab without turning the check

“On”. Following this direction will reduce the number of incorrect entries made in the “Rescheduled

Task” tab.

RUNTIME UPDATE

Vessel/Equipment runtimes must be updated in the Commercial Maintenance Program weekly with the completion of the Weekly System Test and Material Condition Inspection. Run time updates will be completed for all vessels and equipment no later than Tuesday of each week. Run Times must be updated on each individual vessel home page for the Run Time logbook for that vessel to reflect correct information. All in service Harbor Security vessels shall have runtime updates completed on Mondays, Wednesdays and Fridays. If for some reason the vessel hours have not changed, there will be a ships log entry stating that the hours were checked and have not changed since the last recorded update. Vessels that are not mission capable or in an overhaul are exempt from this requirement only during the time the vessel is out of service.

INVENTORY PARTS

When any Maintenance/Service action requires parts, those parts must be attached to the action in the

Commercial Maintenance Program from inventory. Any parts received must be first placed into inventory and then attached to the action. If the notes say that a part was replaced, that part MUST be added to the history, no exceptions.

OTHER PARTS

This section will be used to record the cost of sub-contractors’ services, as well as account for parts that would not be routinely carried in inventory.

SHOP SUPPLIES

When conducting any maintenance task, either PM or OM that requires the use of any material listed on

Enclosure 1, the maintenance person will add “Shop Supplies” to the list of inventory items used. If no other parts were used in the completion of that task “No Parts Required” will be annotated in the notes section. This will apply to ALL tasks completed regardless of number of tasks performed at a given time.

FLUID ACCOUNTING

When completing maintenance tasks that require fluid change, the accounting of the fluid will be in whole quarts/gallons only. EXAMPLE: Any fluid used over 1.5 quarts/gallons will be accounted for as 2 quarts/gallons. Any fluids used over 1 quart/gallon but below 1.5 quart/gallon will be accounted for as 1 quart/gallon. In summary, round up or down when between two full units of measure.

TRAILER MAINTENANCE

As required by our maintenance program Pre and Post use maintenance must be completed on vessel trailers. For sites where the Navy launches or recovers the vessel, that maintenance is the responsibility of the Navy. At sites where the Contractor launches or recovers the vessel, the Contractor is responsible for that maintenance. Maintenance performed by the Navy will not be recorded within the Contractor’s

Fleet 1 Commercial Maintenance Program.

VESSEL STATUS

Sites are responsible for accurately recording the status of each vessel assigned to their respective site.

At a minimum, sites will update the Commercial Maintenance Program status of each vessel daily. This update will include whether the vessel is mission capable, (Green), partially mission capable (Yellow) or not mission capable (Red), with notes for all statuses that are not “Full Mission Capable” (Green). If a vessel falls from fully mission capable status, an email will be generated to the applicable Port

Operations Officer indicating as to why the change in status and an estimated time of repair to bring the vessel to fully mission capable status. The Program Manager, the Regional Maintenance Manager, Contracting Officers Representative, Commercial Maintenance Program Manager, and any other site-specific individuals with a vested interest will also be cc’d on this correspondence. Sites are also required to update the location of each vessel whenever it departs from their respective site.

DAMAGE REPORTING

Any damage to any vessel or piece of equipment under the Contractor’s care must be reported to the

COR. Reporting will happen as follows:

Prior to creating an OM in the Commercial Maintenance Program, pictures of the damage in question must be taken and sent to the Regional Maintenance Manager with an explanation of how it happened (if known).

The Regional Maintenance Manager will report damage to the COR, COR will review picture(s) and determine whether an OM is warranted or whether a “Ship’s Log” entry will suffice. Once a decision has been determined the Regional Maintenance Manager will notify the site in question on how to proceed.

If OM generation is warranted. Upload damage photos to OM documents section.

All damage will be reported on the Contractor’s Monthly COR report for the Month in which the damage occurred.

SYSTEMS AND EQUIPMENT

Systems and Equipment will not be added or changed on any vessel without the Regional

Maintenance Manager’s prior approval.

DANGER TAG OUT REQUIREMENT

Danger tags shall be placed on effected equipment when the maintenance involves safety of personnel or risk of equipment damage. When filling out danger tags, indicate where the tag is physically located as well as the component that is being tagged out. Danger tags shall be uploaded into the Commercial

Maintenance Program and attached to the maintenance action that required the danger tag. Standard nomenclature is vessel hull number (not local id) Danger Tag Sheet and date.

Example: 21UB0101 Danger Tag Sheet 04 January 2019

When creating an OM that requires a danger tag the following must be in the procedure:

1) Install danger tag on the system that maintenance is being performed on to isolate the system.

2) Procedure

3) Remove danger tag when service is complete.

The following is an example of a completed danger tag sheet:

DANGER TAG SHEET

Boat Alteration Records and Liaison Action Record (BOATALT/LAR)

An OM must be created for the installation of any BOATALTS’s or LAR’s. The BOATALT / LAR number must be in the operation data field of the OM. Once the maintenance action is complete the BOATALT plate shall be stamped upon completion of the service action.

Example: Conduct BOATALT 33HS/9A Bilge Ventilation Exhaust Blower Replacement

Example: Port air conditioner has failed, replace in accordance with LAR 33HS-1906

Typically, for LARs the OM will already be in the system before the issuance of the LAR. If this is the case, once the LAR is approved the title of the operation will need to be updated to reflect the appropriate

LAR number.

OVERHAULS

Development of overhaul packages:

Vessels requiring an overhaul will be identified by the Regional Maintenance Manager and the

Contracting Officers Representative (COR) with input from the respective site.

All required work will be related to individual systems and components by creating OM’s in the

Commercial Maintenance Program. Once a vessel is scheduled for overhaul, the respective site shall initiate OM’s that will be done at the depot level and grouped in the assigned overhaul group for that fiscal year and that specific vessel.

Typical progression of OM’s for an overhaul:

Disassemble vessel for overhaul - System: Hull and Superstructure Equipment: Hull

Blast, Prime and Paint - System: Hull and Superstructure Equipment: Hull

Application of Non-Skid - System: Hull and Superstructure Equipment: Hull

Open OM items - System: As applicable Equipment: As applicable

Reassemble vessel - System: Hull and Superstructure Equipment: Hull

Apply local ID on hull/superstructure - System: Hull and Superstructure Equipment: Hull

All outstanding Boat ALTS and LARS should be reviewed when creating an overhaul package and discussed with the Regional Maintenance Manager for concurrence with the COR.
