About this file
This is a reference questionnaire issued by the Nevada State Purchasing Division, Department of Administration, for solicitation number 08DOA-S3909 regarding presort mail services and discounted postage rates for state mail in the Las Vegas area. The questionnaire is designed to collect client feedback on vendors bidding for this contract opportunity. Vendors are required to submit completed reference questionnaires from current and/or former clients for projects of similar size and scope. The reference questionnaire submission deadline is July 20, 2026, and responses should be directed to Annette Morfin, the single point of contact, at amorfin@admin.nv.gov with the solicitation number included in the subject line. The State requests comprehensive responses to all evaluation questions, with answers marked as "U/K" if unknown or "N/A" if not applicable.
The reference questionnaire employs a standardized rating scale from 0-10 to evaluate vendor performance across multiple criteria, including knowledge and expertise, flexibility regarding scope and timeline changes, quality of delivered materials and products, staff dynamics, schedule adherence, customer service responsiveness, billing accuracy, problem resolution capabilities, business requirement flexibility, and likelihood of future recommendation. Completed questionnaires are designated as confidential business information under Nevada Revised Statutes 333.020(5)(b) and are not subject to public disclosure. In addition to the written questionnaire responses, the State reserves the right to contact references directly for further clarification. References should provide narrative responses regarding their working capacity with the vendor and satisfaction with services, as well as recommendations for future engagement.
