STATEMENT OF WORK (SOW)

FOR

ROOFING INDEFINITE-DELIVERY/INDEFINITE-QUANTITY (IDIQ) CONTRACT

A-01. DESCRIPTION OF WORK:

This Roofing contract is a single award Indefinite-Delivery, Indefinite-Quantity (IDIQ) contract to provide a streamlined means to complete roofing repairs and/or repair by replacement of all types of roofing materials and roofing assemblies to include but not limited to; Built-up roofing system (BUR), Styrene-Butadiene-Styrene (SBS) Modified Bitumen, Structural Metal Standing Seam roofing system, Single ply PVC roofing system, and Asphalt shingle roofing. The vast majority of the roofing projects will be at Hill Air Force Base (Hill AFB), with a small quantity of projects at Little Mountain, Utah Test and Training Range (UTTR), Dugway, Carter Creek and other associated sites located in Utah and Pinedale, Wyoming.

All work shall be in conformance with the Statement of Work (SOW) of the contract including, but not limited to: furnishing all materials, labor, tools, equipment, transportation, supervision, management, disposal, and other services to include incidentals necessary for the successful completion of issued task orders in accordance with (IAW) these specifications;

Unified Facilities Criteria (UFC) 3-110-03, manufacturer specifications, Environmental Protection Agency (EPA), Occupational, Safety and Health Administration (OSHA), Utah Division of Air Quality (UDAQ), and all other applicable federal, state, and or local regulations.

The government estimates that the volume of work will be between $3,000K and $4,999,999 during a 5-year performance period. The work will consist primarily of repairs and/or repair by replacement of shingle roofing, metal roofing, built-up roofing, pvc roofing, and modified bitumen roofing systems. Task orders may include installation or repair of heat trace systems, lightning protection systems, gutters, downspouts, curbs, flashings, and other work associated with repairs and replacement of roofing systems.

A-02. QUALIFICATIONS:

All bidders must submit with their proposal the manufacturer’s certification and warranty as described in the general requirements and individual roofing specifications sections.

A-03. PROJECT MANAGEMENT: The following functions are required for contract management for the duration of the contract.

Contractor: The general (prime) contractor awarded this contract is responsible for:

(1) Successful completion of all terms and conditions of this contract.

(2) All employees, subcontractors, suppliers and consultants under his/her employ.

(3) All labor, materials, equipment, supervision, transportation and all else required to successfully complete each Task Order issued.

(4) The contractor shall perform 15 percent or more of the actual construction using prime contractor personnel for each project, office and field supervision will not be included in the 15%.

Local Contractor Office: The contractor shall maintain a local office or warehouse offsite, and at the contractor’s expense, with all the necessary equipment and materials. *The Government will not provide a location on base for the contractor to establish an office or warehouse space.

Personnel: The contractor shall provide qualified personnel to perform all functions necessary to execute this contract, to include but not limited to the following:

Project Manager: The project manager (PM) will be responsible for coordinating all work for each Task Order. This individual(s) shall have direct oversight of all projects and planning activities. The name of this person and any alternates shall be designated in writing to the Contracting Officer (CO) and the 75 CES Project Manager (PM).

Project Foreman: The project foreman will be responsible for the day to day execution of roofing activities in accordance all requirement of this contract and each specific Task Order. This individual shall be proper trained in accordance with manufacturer specifications for the installation and or repairs of the roofing system for the specific Task Order.

Contractor Employee Requirements: The contractor shall ensure that employees and subcontractor employees have the necessary training to perform their functions. Any cost or time associated with obtaining training or certifications shall be at the contractor’s expense. The contractor shall not employ any person who is nor was an employee of the US Government, if the employment of that person would create a real or perceived conflict of interest. Any questions on this issue should be addressed to the

CO.

A-04. GENERAL REQUIREMENTS:

Requirements: Contractors shall abide by all applicable Environmental Protection Agency (EPA), Occupational Safety and Health Administration (OSHA), Utah Division of Air Quality (UDAQ), and Hill Air Force Base (AFB) regulations. All roofing installed shall meet the requirements listed in the Hill AFB Base Design Standards, UFC 3-110-03, the general requirements, and individual roofing types specification included in this contract. These standards are not meant to be exhaustive and if there is a conflict between these standards with other codes, regulations, and specifications included in this contract, the most stringent requirement shall apply at all times. The contractor shall comply with a period of performance (POP) established in the SOW and TOs.

Changes: If the Government requires changes the contractor considers significant enough to add time or money, the contractor must immediately notify the 75 CES PM and CO in writing of its position. Field changes, modifications, or possible expenditures of Government funds must be coordinated through Contracting, Civil Engineering (CE), the Using Agency, and the Contractor. Modifications to Task Orders must be made through PK after the contractor provides a proposal for incorporating the change. Government review of the contractor’s proposal will be required prior to any changes being implemented.

Government Acceptance and Payment: Final payment (NET 30) will be made after final project acceptance is made by the Government and after the contractor submission of all closeout project documentation, i.e. roof warranty when applicable, and an invoice is submitted through Wide Area Work Flow (WAWF). Progress payments (NET 14) may be included in the Task Order award at the Government’s discretion, typically Task orders that exceed 60 days calendar or $150K will be authorized for Progress Payments. Further guidance for Progress Payments will be included in the contract with the applicable contract clauses (FAR 52.232-16 Alternate I & DFARS 252.232-7004).

Coordination and Scheduling: In order for the work to progress smoothly and cause minimal disruptions it will be necessary for the contractor, 75 CES PM and CO to maintain close and active coordination. The contractor shall keep the Government informed at all times of when and what work will be accomplished. All work schedules will be coordinated with the appropriate representatives of the Contracting and Technical Offices and accomplished in such a manner that minimum inconvenience will be caused to the Government. The contractor is required to schedule, at a minimum 24 hours prior, with the 75 CES PM for final/manufacturer inspections.

Asbestos Abatement: The contractor will be required to perform some asbestos abatement in conjunction with roof replacement/repairs performed under this contract. This will primarily be abatement of asbestos containing flashing materials typically associated with built-up/modified roofing systems. Per Hill AFB requirement all asbestos abatement shall be done by an abatement contractor that has been approved by the 75 CES/CEOHA office. Abatement contractor qualification submittal requirements are listed in the 01000 general requirements section, Part 4 Appendices 4.5.

A-05. EXISTING FACILITIES/JOB SITE VERIFICATION:

Dimensions, quantities, elevations, and site conditions shall be verified by the contractor.

Discrepancies between SOW, specifications and existing conditions shall be referred to the 75 CES PM in writing before submitting a proposal, if possible, or before work affected is performed. Failure to make such notifications shall place responsibility upon the contractor to carry out work in satisfactory, workman like manner. Any duplication of work made necessary by failure or neglect on the contractor's part to comply with this requirement shall be completed at their sole expense.

A-06. SUBMITTALS:

The manufacturer certification (Submittal No. 1 as noted in the general requirements section) is a qualification to bid this contract. Submittal No. 1 shall be executed and included with the bid proposal to the Air Force. Failure to provide the required certifications will result in the contractor being declared non-responsive. The contractor is required to provide a manufacturer certification for each type of roof system, materials, and membranes including but not limited to; (a) built-up roofing system (BUR), (b) Styrene-Butadiene-Styrene (SBS) Modified bitumen roofing system, (c) Structural Metal Standing seam roofing system, (d) Asphalt shingle roofing, and (e) single ply PVC roofing.

The manufacturer warranty (Submittal No. 2 as noted in the general requirements section) is a qualification to bid this contract. Submittal No. 2 shall be executed and included with the bid proposal to the Air Force. Failure to provide the required warranty will result in the contractor being declared non-responsive. The contractor is required to provide a manufacturer warranty for each type of roof system, materials, membranes, and services including but not limited to;

(a) built-up roofing system BUR, (b) SBS modified bitumen roofing system, (c) Structural Metal standing seam roofing system, and (d) Asphalt shingle roofing, and single ply PVC roofing.

TO specific submittals of materials may be required and will be specified in the TO Statement of Work. Any time the contractor uses different materials or a different manufacturer that have will require new submittals and approval.

Abatement work plans are required to be submitted, for each task order requiring asbestos abatement. Abatement work plans are to be submitted to the 75 CES/CEOHA office and approved prior to any work being done. All abatement plans shall be written by a UDAQ certified project designer. The contractor shall be responsible for submitting all notifications to UDAQ when required and providing a copy of approved notification to the 75 CES/CEOHA office.

Upon completion of any abatement task order the contractor shall submit copies of all project documentation to the 75 CES/CEOHA office. Documentation shall include but is not limited to daily logs, sign-in sheets, UDAQ notifications, air sampling results, and waste manifests.

With the exception of the waste manifest all documentation must be received before final payment will be made. Waste manifest must be received within 30 days of acceptance by the approved landfill.

The contractor shall be responsible for submitting a waste diversion report at the completion of each Task Order. Waste diversion report shall show a list of materials, weight in tons disposed in landfill, weight in tons that was recycled, and where materials were recycled. *The government will provide a sample waste diversion form after award of the basic contract.

The contractor shall be required to submit the Task Order specific manufacturer warranty prior to submitting for final payment.

A-07. TRAINING:

All personnel performing roofing repairs and/or replacement shall be trained on the proper method of installation for each type of roofing system being installed.

A-08. HEALTH AND SAFETY:

In addition to the detailed requirements of this specification, comply with laws, ordinances, rules, and regulations of federal, state, regional, and local authorities regarding roofing system installation. Submit matters of interpretation of standards to the appropriate administrative agency for resolution before starting work. Where specification requirements and performance documents vary, the most stringent requirement shall apply.

A-09. LANDFILL:

All waste generated shall be the responsibility of the contractor to dispose of. The contractor shall not use any government dumpsters for disposal of contractor generated waste. When possible the contractor shall recycle waste materials. The contractor shall be required to submit a waste diversion report upon completion of each task order, sample report to be provided after award.

A-010. WORK PROCEDURE:

Perform roofing system repairs and/or repair by replacement in accordance Air Force, Hill AFB, UFC, OSHA, EPA and UDAQ regulations, and as specified herein and in the attached specifications. Personnel shall wear and use protective clothing and equipment (PPE) in accordance with applicable regulations. The contractor shall ensure that appropriate safety precautions are used when working on facilities that are occupied while work is being done.

A. Accessibility of Work Areas:

The Government will rearrange areas to the extent of providing a reasonable, direct, and unobstructed path to the work site. The Contractor shall confine his equipment and employee pattern to these designated areas. Where the building is still occupied during the roofing operations, interference with the functional operation of the building occupants outside of these areas will not be permitted. Where conflicts arise due to Contractor’s operations, the decision of the Contracting Officer or their authorized representative shall be final.

A-011. PROTECTION AND CLEANUP:

Protection: All roofing work shall be performed without damage to adjacent areas, equipment, items, or materials. Should existing equipment, items, or materials become damaged by work done under this contract, the contractor shall restore all areas to original condition at no additional cost to the government. The contractor shall be responsible to ensure roof is water tight at the end of each shift, and shall be responsible for all damage and repairs from water entering facilities during roofing work.

A-012. CONTRACTOR FURNISHED EQUIPMENT/SUPPLIES:

The contractor shall be required to furnish all materials, equipment, and personnel necessary to manage, and perform the roofing work listed in the task order specific statement of work.

If not readily available on site the contractor must supply water and power to the work site as necessary. In most cases, though not always, electrical power and cold water for use will be available within 200ft of work site.

A-013. GOVERNMENT FURNISHED EQUIPMENT/MATERIALS:

Government Furnished Materials (GFM): Government Furnished Materials will not be furnished to the contractor as a general rule. However, the Government reserves the right to provide GFM and/or Government Furnished Equipment (GFE) to be used on any task order.

The contractor shall follow the instructions of the Contracting Officer's representative regarding the disposition of all Government-furnished property not consumed in performance of the task order.

Contractor Transporting: The contractor with his own forces shall transport all GFM and/or GFE, if any described on the task order from the Government’s storage area to the work site indicated in the task order.

Risk and Responsibility: Once accepted, the contractor assumes the risk and responsibility for the loss or damage to any GFM/GFE.

Transportation Cost: It is anticipated that the majority of GFM/GFE will be provided at or adjacent to the project site. In such instances, no separate transportation costs would be incurred or cost. If transportation of GFM/GFE is other than what would normally be encountered, a separate cost would be included in the final negotiated price of the project order.

Location: The location (as well as the type, quantity, and description) of GFM/GFE will be identified in each Task Order when applicable.

A-014. BASE ENTRY:

Conformance with Regulations/Directives: The contractor shall conform to base regulations and directives pertaining to security, safety, traffic, fire, and personnel clearances while on Hill

AFB.

Base Access: The contractor shall be responsible for requesting access to the base for their personnel by completing and submitting AF Form 496 to the contracting officer or their representative at OL:H/PZIOA.

Secure/Restricted Area: The contractor shall be responsible for requesting access to secure and restricted areas by submitting AF Form 2583 for a Controlled Area badge or AF Form 2586 for an Unescorted Entry Authorization Certificate. A background check of each individual requesting entry to a secure or restricted area will also be made before access is authorized. Two forms of picture identification are required each time such areas are entered.

Secured area badges will be issued by 75 CEG Group Security Manager, currently located in Building 593 North.

The Government may require at times that the contractor be under escort during performance of a project. These escorts might be Government employees at times or the contractor may be required to accomplish these escort duties. Controlled areas contain valuable resources and are clearly marked with a “Controlled Area” sign. A warning sign marks restricted work areas.

Restricted work areas contain priority resources and all personnel must obtain specific written permission to enter. Work in some areas of the base may require special briefings for the contractor’s employees (special briefings are required before work is accomplished in the flight line area).

Personnel and equipment entry may be subject to delays due to security processing. The contractor shall not be compensated for reasonable delays in accessing the site. Also, due to the nature of controlled areas, the contractor may be requested to leave the controlled area at any given time. The Contracting Officer may extend the contract performance time when area designations change while the contractor is working.

Processing Time: The above information shall be submitted to the CO a minimum of ten (10) calendar days [fifteen (15) calendar days for access to secure/restricted areas] in advance of when access to the job site will be required.

Unauthorized Activity: The contractor shall inform all personnel working under his/her jurisdiction (including subcontractor and supplier personnel) that access to areas outside of the immediate work area (excluding cafeterias and restroom near the work site, direct haul and access routes, Contracting and Civil Engineering offices, and points of supply and storage) is prohibited. Circulation of said personnel will be limited to official business only. Persons engaged in unauthorized reconnaissance of other contractor or Government activity will be referred to the CO for disposition Infractions involving possible compromise of national security will be turned over to the FBI for disposition, with termination of contract.

A-015. BASE REGULATIONS: Contractor employees and visitors are subject to the same restrictions as are Government personnel.

Smoking: Smoking is permitted in designated, outside smoking areas only. No smoking will be allowed within 25 feet of any base facility.

Seatbelts: The use of seatbelts by all vehicle operators and passengers is mandatory on Hill AFB and in the state of Utah. On Hill AFB, vehicle operators and passengers caught not wearing their seatbelts will be issued a traffic citation and may lose their on-base driving privileges.

Firearms/Illegal Drugs: No firearms or illegal drugs are allowed to be brought onto Hill AFB.

By accepting a vehicle pass, each person gives their consent to a search of their vehicle while it is entering on or off of Hill AFB.

Speed Limit: The speed limit on base ranges from 15-40 MPH and in parking lots it is 5

MPH.

Parking: Contractor vehicles and equipment will be parked in common parking areas or as otherwise approved in writing by the Facility Manager, 75 CES PM, and/or CO.

Cell Phones: Use of cell phones while driving is prohibited unless equipped with hands free device. On Hill AFB, vehicle operators caught talking on a cell phone while driving will be issued a traffic citation and may lose their on-base driving privileges.

Communications: In accordance with Air Force Instruction (AFI) 33-211, all communications with DoD organizations are subject to communications security (COMSEC) review.

Contractor personnel will be aware telephone communications networks are continually subject to intercept by unfriendly intelligence organizations. The DoD has authorized the military departments to conduct COMSEC monitoring and recording of telephone calls originating from, or terminating at DoD organizations. Therefore, civilian contractor personnel are advised any time they place a call to, or receive a call from a USAF organization, they are subject to COMSEC procedures. The contractor will assume the responsibility for ensuring wide and frequent dissemination of the above information to all employees.

A-016. HILL RANGE ACCESS REQUIREMENTS:

Range Access Criteria: For access to the Hill Range Complex, these criteria must be met:

(1) Access has been granted by the appropriate authority.

(2) The individual has a range user point of contact.

(3) Access is for official Government business only.

(4) The individual possesses valid official photo identification.

(5) The individual complies with all security requirements.

(6) The individual signs a Secrecy (Non-disclosure) Agreement

(7) The individual acknowledges the Range Security and Safety Briefing.

(8) The individual is a United States Citizen.

Visitor Access Procedures: All visitors who require access to Hill Air Force Range facilities must comply with the following:

Utah Test and Training Range (UTTR) Visit Request Form 0-17: Submit the form 0-17 ten (10) working days prior to the visit (15 working days prior for groups of 15 or more) to the CO for each visit for Range Squadron approval. UTTR Form 0-17 will be completed with the following information:

(1) Name and rank

(2) Social Security number

(3) Security clearance

(4) Job title

(5) Date and place of birth

(6) If nationalized, provide naturalization number, port of entry, and date

(7) Organization

(8) Dates of visit

(9) Purpose of visit

(10) Mode of transportation

(11) Entry and exit locations

Property Removal Authorization: All property that is being removed from the range shall be accompanied by a property removal pass. Property that is not accompanied by the appropriate removal authorization will be confiscated and held, pending an investigation of the incident.

Property Removal Procedures: All contractor property and equipment must be clearly marked. An inventory shall be submitted of property and equipment that the contractor will bring on to the Range area. The inventory shall include make, model, and serial number of high value equipment (i.e. power tools, scaffolding, aerial lifts, etc). Hand tools shall be included on the inventory, listed as "tool box with assorted carpentry tools." The inventory will be used to issue Property Removal Passes.

Search, Inspection, and Seizure: All vehicles, personnel, and hand-carried items entering or exiting any range control point, or while on the range, are subject to search and inspection and seizure of any prohibited item at any time. Random, unannounced, and regular searches and inspections will be conducted. These searches and inspections are intended to assure the prohibited articles are not introduced into and Government property is not removed, from the range or its facilities.

Vehicles:

Privately-Owned Vehicles (POV): No POV's will be permitted on Range. The contractor shall transport his personnel from outside the main gate to the work site via their commercial vehicles. The contractor shall only drive on the approved haul route identified in the Vicinity Map as may be included in individual projects.

Commercial Vehicle Authorization: Commercial vehicles may be permitted on Range, provided the driver has been authorized by the appropriate authority to drive on range and that the vehicle is currently registered, licensed, insured, and used by personnel who have been authorized range access.

Rental Vehicles: Rental Vehicles are permitted, provided a current rental agreement is produced and that the rental vehicle is currently registered, licensed, and used by personnel who have been authorized range access to conduct official business.

Range Travel:

Prohibited Travel: Travel by personnel in any area that is outside of their regular assigned duty area is prohibited. Should special assignments require personnel to travel off roads, or into areas where they would not normally be expected to work, prior written permission must be obtained from the Security Administrator and the affected range user.

Sightseeing: Sightseeing is strictly prohibited. Personnel who violate this requirement are subject to disciplinary action, withdrawal of access authorization, or arrest and prosecution and termination of contract.

Sensitive Equipment:

General: Certain equipment is considered sensitive and without written authorization, this equipment is prohibited from use on the Range. Sensitive equipment shall not be brought onto the range without specific written authorization from the 75 CES office located in Building 597, Hill AFB. Personnel who are found to be in possession of sensitive equipment without the appropriate authorization will be challenged and the item confiscated pending an investigation. Personnel in possession of sensitive equipment without authorization, or suspected of improper use of sensitive equipment, are subject to withdrawal of access authorization, or arrest and prosecution.

Sensitive Equipment: The following is a list of equipment that would be considered “sensitive". Provided list may not be all inclusive. NOTE: Use of sensitive equipment will be subject to current Government restrictions

(1) Recording or copying devices of any type.

(2) Electronic equipment that is equipped with a data exchange port which could be used to connect Automated Information System (AIS) equipment. (Excludes items such as pocket computers, wrist watches, or data diaries, not equipped with a data exchange port).

(3) Computers and associated hardware

(4) Cameras and photograph equipment

(5) Binoculars and optical magnifying equipment

(6) Night Vision Devices

Request for Authorization: Sensitive equipment may be authorized for official use on and removal from the range, to obtain sensitive equipment passes, the contractor will submit a letter that identifies the personnel who will operate the equipment. Requests for sensitive equipment passes shall contain:

(1) Name of the responsible person and Social Security Number.

(2) Period of use of the sensitive item.

(3) General purposes of use.

(4) Method of identification.

(5) Identification of item(s) by description, specific serial number, and Government control number.

(6) Copy of any licensing (e.g. FCC).

(k) Prohibited Items: Certain items are prohibited on the range. Personnel who are found to be in possession of prohibited items will be challenged and the item confiscated pending an investigation. Personnel in possession of prohibited items are subject to disciplinary action, withdrawal of access authorization, or arrest and prosecution. The following non- Government owned items are prohibited on the Range, unless an exception is noted:

(1) Alcohol.

(2) Controlled substances including illegal drugs and associated paraphernalia, except for prescription medicine.

(3) Firearms, ammunition, and other dangerous or deadly weapons.

(4) Explosives, incendiary, and explosives or deadly weapons.

(5) Weapons: Mace, stun guns, clubs.

(6) Two-way radios, CB Radios, Cellular phones (Except two-way or CB radios) which are permanently installed in a commercial vehicle, whose frequency has been approved by the installation commander.

(7) Recording or copying devices of any type.

(8) Electronic equipment that is equipped with a data exchange port which could be used to connect Automated Information System (AIS) equipment. (Excludes items such as pocket computers, wrist watches, or data diaries that are not equipped with a data exchange port).

(9) Computers and associated media.

(10) Binoculars, optical magnifying equipment.

(11) Night vision devices.

(12) Recreational vehicles.

(13) Pets, wild or domestic.

(14) Children under 18 years of age.

(15) Other items prohibited by law.

Range Access Gates, Road Access and the UTTR Main Gate: Operational hours for the gates are 0730 -1700, Monday through Thursday.

Materials: The contractor will provide physical security for the materials and work.

A-017. ENVIRONMENTAL PROTECTION:

Solid, Liquid, and Gaseous Contaminants: The contractor shall be responsible for the proper disposal of all solid, liquid and gaseous contaminants including asbestos and lead based paint in accordance with all applicable federal, state, local and Hill AFB regulations, codes, and guidance policies.

Disposal of Refuse: The contractor shall maintain all work sites free from construction debris and trash. The contractor will be required to remove all refuse daily from each job site for work performed under the Abatement contract. All construction debris shall be sorted and recycled, or disposed of in a permitted landfill. No trash or debris shall be disposed of on the base or any other Federal Lands, nor shall it be disposed of in violation of any Municipal, County, or State Ordinances (with the exception of the base landfill and applicable materials as outlined in the Base Facility Design Standards for Hill AFB). The contractor may keep proceeds from all recycled materials. The contractor is responsible for paying all tipping fees associated with off-base landfills.

Covered Chutes/receptacles: All chutes receptacles for refuse and the like, shall be covered or of such design to fully confine the material to prevent the dissemination of dust.

Soil Erosion: The contractor shall provide protective barriers to prevent soil erosion on all required excavations. The contractor will also provide environmental protection to prevent sediment drainage of silts into storm drains.

Safety Data Sheets (SDS): The contractor shall submit an SDS for any chemical or hazardous materials as defined in Federal Acquisition Regulation (FAR) 52.223-3. Hazardous Material Identification and Safety Data for each 75 CES task order at the conclusion of clarification negotiations and prior to award. Each SDS shall be submitted in 3 copies, and the contractor shall retain a copy for on-site use.

Contaminated Soils: Work requested of the contractor may include removal and/or dumping of contaminated soils. Contaminated soils are defined as containing non-hazardous materials such as JP-4/5/8 aircraft fuel, heating fuel oil, asbestos, lead, cadmium, or other materials as identified by the Base Environmental Coordinator. Any contaminated material shall be removed, hauled and disposed of in accordance with all applicable laws, regulations, and policies and shall include submittals of all required permits and disposal certificates.

Hazardous Materials: The contractor shall bring to the attention of the Contracting Officer (CO) any previously unknown material suspected of being hazardous with in accordance with federal, state and local laws and regulations which they may encounter during execution of the work period. A determination may be made by the CO as to whether the contractor shall perform tests to determine if the material is hazardous If the CO directs the contractor to perform the tests, and/or if the material is found hazardous and additional protective measures are needed, a contract change may be required, subject to equitable adjustment under the terms of the contract.

(1) Where applicable, the contractor shall use environmentally safe, low Volatile Organic Compound (low-VOC) products, in the course of completion of project.

(2) There will be no on base disposal allowed including use of drains (sanitary, storm, or industrial)

(3) All hazardous material such as sandblast media, chlorinated solvents, paint thinners, and fuels will be labeled and an inventory management system initiated to ensure timely removal and proper disposal.

(4) Fuel storage will be limited to 110 gallons. The base Fire Department and the

Environmental Management office will approve the siting.

(5) Storage of oils, greases, chemicals, or other liquids will require a fully contained facility for spill prevention and security. A minimum of 40 pounds of absorbent material per 44 gallons of chemical/oil/grease will be on hand for spill control.

(6) All spills shall be reported by dialing 911 and giving information as to spill location, type of material and estimated quantity, and if the spill is contained. The contractor will ensure appropriate Personnel Protective Equipment (PPE) is available to take care of spill clean-up and handling of residue.

(7) Spray-painting shall utilize high-transfer efficiency equipment with low volatile organic compound (low-VOC) paints or water-based paints. The VOC content of low- VOC paint shall be 3.5 lbs/gallon or less for primers, topcoats, and specialty coatings.

For clear coats it will be 4.3 lbs or less. If a low-VOC paint is not available for the application, a paint waiver shall be obtained from Environmental Management (EM), by contacting the Contracting Officer's Representative. Uncontrolled spray painting with high-VOC paints is prohibited.

(8) All tanks used for fuel storage must have spill containment for 100% of fuel. Any tanks needed for chemicals, oils, and other liquids must have spill containment for 100% of stored product.

(9) If at any time, an authorized inspector for EM feels that regulatory requirements listed above are not being followed. EM reserves the right to notify the Contracting Officer and recommend corrective action.

(10) Banned Ozone Depleting Substances (ODS): Heating, Ventilating, and Air

Conditioning Equipment which use ChloroFluoroCarbon (CFC) refrigerants are strictly prohibited. This includes but is not limited to R-11, R-12, R-13, R-111, R-112, R- 113, R-114, R-115, R-211, R-212, R-213, R-214, R-215, R-216, and R-217. All

CFC refrigerants must be reclaimed and turned into 75 Civil Engineering refrigerant shop or 309th MXDVACH refrigerant shop. Fire protection system using Halon 1211, 1301, or 2402 are also prohibited. Other substances banned from usage on the work site include carbon tetrachloride, methyl chloroform, and methyl bromide.

Hazardous Waste: The contractor shall be responsible for the proper handling and management of all hazardous waste generated from work performed on Air Force property.

Contractor must, store, document, and transport hazardous waste in accordance with applicable Air Force, State, local, Federal, and Department of Transportation (DOT) regulations. Only Hill AFB authorized individuals are allowed to sign Uniform Hazardous Waste Manifest (UHWM) for Hill AFB hazardous waste, for the purpose of this contract all waste manifests shall be signed by the 75 CES/CEOHA office. Contractor will be held fully liable for any of their activity or non-activity that results in a Notice of Violation (NOV) or any other penalty. Any waste generated, hazardous or non-hazardous by the contractor will be the responsibility of the contractor. If a NOV is issued or regulators are investigating a potential violation, the contractor will assist Hill in defending the investigation.

(1) All drums must be labeled with the correct hazardous drum label. Label must contain the DOT shipping name, United Nations (UN), or Environmental Protection Agency (EPA) waste number, generator information, and accumulation start date. The label must be placed in the upper third of the drum.

(2) All drums used to store hazardous waste must be non-leaking and safe to handle.

Drums that are rusted, dented, or leaking, should be over-packed. Drums and/or over-packs must be purchased by the contractor. Any drums purchased by the contractor must meet DOT specification numbers 5B, 17E, or 17H requirements. The specification numbers are stamped on the bottom of the drum.

(3) Hazardous waste drums must be stored in an area authorized by EM. In general, the area must have a containment system, be secure, and must have signs placed around it that say, “Hazardous Waste No Trespassing".

(4) Hazardous waste drums can only be stored for 90 days after accumulation start date.

Drums will be closed except when adding or removing waste.

Spills: Should any hazardous substance or spills occur on this project, in addition to containing and removing such spill, the contractor shall:

(1) During normal working hours, immediately report all spills in quantities of five (5) gallons or more to the EM office.

(2) After 4:30 p.m. or on weekends, immediately contact the Hill Command Post 777-

3007 and/or the Fire Department 777-3021 or 911.

(3) Forward a written, completed spill report to the Contracting Officer within three (3) working days after the occurrence.

Dust Control: County Dust Permits are required for surface disturbances greater than 1/4 acre, or 100 feet of trenching. Copies of any permits required must be provided to ALC/EMC. If in the opinion of the 75 CES/CEOHA PM/CO there is excessive dust being generated by the construction effort, the contractor shall stop all activity immediately until adequate dust control measures are provided, at no cost to the Government for work at Hill Air Force Base and Hill Air Force Base Ranges.

Preservation of Historical, Archeological, and Cultural Resources: The contractor will take precautions to preserve all existing historical, archeological, and cultural resources as they existed immediately prior to project start. If during project execution, the contractor observes unusual items that might have historical or archeological value, such observations shall be reported as soon as possible to the 75 CES PM/CO.

Protection of Flora and Fauna: The contractor shall perform all work and take such steps required to minimize interference with or disturbances to flora and fauna. The contractor shall not be allowed to alter water flows or otherwise disturb native habitat adjacent to the work areas that in the opinion of the 75 CES PM/CO, are critical to flora and fauna.

Protection of Water Resources: The contractor shall not pollute any streams, rivers, or waterways through direct discharge or by storm water runoff from construction projects/excavation work. The contractor shall comply with all applicable Federal, State, and local laws.

Compliance with Air Regulations: The contractor shall be responsible to ensure all pre construction Air Permits are coordinated through the 75 CES/ PM/CO, and EM to meet EPA Title V requirements.

Recycling Requirements: Recycling efforts should take place whenever feasible.

AF Form 103 Work Clearance Request: Before commencing work on an individual Task Order requiring a disruption in service the 75 CES Project Manager and the contractor shall prepare and coordinate an AF Form 103, Work Clearance Request. The contractor shall notify the Government PM at least 3 weeks prior to the need of the AF Form 103. No service disruption will be permitted until the contractor has an approved AF Form 103 on site.

(1) A copy of the completed AF Form 103 shall be provided to the Government prior to the contractor requesting service disruptions.

(2) Work clearance requests (AF Form 103) shall be submitted a minimum of 21 days prior to the required outage date.

(3) Should the contractor damage any utilities or structures, they shall immediately notify the Base Civil Engineer Service Desk (75 CES/CEOS) currently at (801-777-1856) and the 75 CES PM.

(4) Responsible agencies need to be informed and aware of such outages in advance to make arrangements to accomplish their mission without interruptions.

75 CES/CEOSLU Form 12 Base Civil Engineering Excavation Permit: Before commencing work on an individual DO the contractor and the 75 CES Project Manager and the contractor shall prepare and coordinate the Form 12, Excavation Permit, with drawings indicating where the existing subsurface improvements are in the area and specific excavation and trenching locations widths and depths. The contractor shall notify the Government PM at least 21 days prior to the start of a project when a Form 12 is needed. No excavation will be permitted until the contractor has an approved Form 12 on site.

(1) A copy of the completed Form 12 shall be provided to the Government prior to the contractor requesting utility locates.

(2) Excavation Permit (Form 12) shall be submitted a minimum of 14 days prior to the required date.

(3) The contractor shall be responsible for protection of any underground utilities and structures identified on the Form 12 or by utility locate within five (5) feet of the identified location.

(4) The contractor shall responsible to maintain the utility locates until the contractor no longer requires them to complete the execution of the Task Order.

(5) Should the contractor damage any underground utilities or structures, they shall immediately notify the Base Civil Engineer Service Desk and the 75 CES PM.

(6) If the damaged item was identified on the Form 12 or by the utility locate, the contractor shall repair the damage immediately and return the item to its original working condition at their expense.

(7) If the damaged item was not identified on the Form 12 or by the utility locate, the government will repair it at their expense.

(8) Responsible agencies need to be informed and aware of such requests in advance to make arrangements to accomplish their mission without interruptions.

(9) The Requester needs to complete sections 1 through 10, 12 on the front of the form and 14 on the back of the form.

A-018. GREEN PROCUREMENT: EPA Section 6002 of the Resource Conservation and Recovery Act of 1976 (RCRA) and Executive Order (EO)13101 requires the use of recycled and recovered products identified in the Environment Protection Agency’s Comprehensive Procurement Guidelines. Contractors at Hill AFB shall assist in compliance with this EO through the:

(1) Use of products with post-consumer recycled content

(2) Use of products that are recyclable

(3) Use of low toxicity or non-toxic products

(4) Use and installation of low-maintenance products

The following items are currently listed for Vehicular Products and Miscellaneous in the EPA Guidelines:

(1) Engine Coolants

(2) Re-built vehicular parts

(3) Re-fined lubricating oils

(4) Sorbents

(5) Printing and Writing. Paper used in the office should contain 30% post consumer recycled paper

(6) Other Recyclable Materials. The following items shall be recycled:

(a) metal beverage containers

(b) glass (clear, brown, green)

(c) office paper

(d) newspaper

(e) corrugated fiberboard/cardboard/paperboard

(f) wood

(g) plastics

Contractors may recycle materials not listed above with prior coordination of the CO or COR and the 75 CEG/CEN Qualified Recycling Program (QRP) Manager.

All recyclable items shall be stored, shipped, and disposed of in accordance with Federal and Local environmental regulations.

The contractor shall be required to use these recycled or recovered products unless they cannot be procured within a reasonable period of time, at reasonable prices, or the products(s) do not meet technical or performance standards. EPA Guidelines are the minimum requirement. The contractor is encouraged to use other materials and products not listed but commonly used in industry. The contractor shall maintain records and submit an annual report to the CO, along with the annual certification required by FAR 52.223-9 Certification and Estimate of Percentage of Recovered Material Content for EPA Designated Items, upon completion of the basic year and. The contractor shall be knowledgeable of and comply with all applicable Interstate, Federal, State, and Local laws, regulations, and requirements regarding environmental protection. In the event environmental laws/regulations change during the term of this contract, the contractor is required to comply as such laws come into effect. If there is an increase or decrease in cost as a result of the change, the contractor shall inform the CO pursuant to notice requirements and negotiate a modification to the contract.

A-019. UTILITIES SERVICES:

Water and Electricity: In accordance with FAR 52.236-14, Availability and Use of Utility Services, all reasonable amounts of domestic water and electricity will be made available to the contractor by the Government from existing system outlets and supplies. With prior coordination, the contractor shall be given access to water hydrants to obtain water for construction activities.

Restrooms: The contractor will be required to provide portable restroom units for their employees on the job site and must be maintained for the duration of the project.

Temporary Utility Connections: The contractor shall, at his own expense, make all temporary connections and install distribution lines. The contractor shall furnish to the CO a complete system layout drawing showing type of materials to be used and method of installation for all temporary electrical systems. All temporary lines shall be maintained by the contractor in a workmanlike manner satisfactory to the 75 CES PM/CO and shall be removed by the contractor in like manner, prior to final acceptance of the construction/renovation project. The point of connection shall be coordinated with the Government technical representative prior to connection. If additional outlets are needed they will be provided at the contractor's expense.

A-020. WEATHER CONDITIONS:

Weather Hazards: The contractor shall be aware of the hazards likely to arise from weather conditions. Complete weather records and reports may be obtained from any U.S Weather Bureau office.

Contractor shall be responsible for proper application of weather sensitive materials and shall not apply any such materials when weather conditions fall below the minimum recommendations by the material manufacturer.

Contractor shall cover all concrete installed in weather below 32º Fahrenheit with approved insulating blankets.

Contractor is responsible to ensure that in weather below 40º Fahrenheit, water lines and fire protection systems do not freeze.

Contractor shall be responsible for supplying heat to unoccupied interior portions of building under construction at a constant 65º Fahrenheit, plus or minus 5º Fahrenheit. This requirement shall also apply in the event that available heating supplied by the building HVAC system is rendered inadequate by the contractor, during the course of construction to maintain the above temperature of 65º Fahrenheit, plus or minus 5º Fahrenheit.

Temporary heating devices permitted are UL approved electric heaters and/or diesel fueled portable heaters, as long as the portable heaters are located outside the building and away from any fresh air return vents. The contractor shall physically monitor all temporary heating devices at least every four hours. Exception to this requires written approval from the Base Fire Department.

High Winds: When warnings of winds of gale force or stronger are issued, contractor shall take every practicable precaution to minimize damage (including overspray) to persons, work, and adjacent property. These precautions may include removing all stored materials. Tools, and/or equipment from exposed locations and removing or securing any temporary structures/scaffolding.

A-021. FIRE PREVENTION AND PROTECTION:

Hill AFB Fire Regulations: The contractor shall comply strictly with the Hill AFB fire regulations and become thoroughly familiar with and brief employees and subcontractors on the fire safety requirements applicable to this contract per the Fire Prevention Guide for Contractors – Hill AFB.

Welding Permit: No welding, cutting or open flame will be permitted without obtaining a Welding/Cutting/Burning Permit issued by the Base Fire Department. Training class for this permit is currently taught the first Thursday of every month at 0800 at the Building 133 fire offices. This permit shall be requested by the contractor a minimum of one workday prior to required need. A permit must be obtained each time any welding, cutting, or open flame is required.

Clean-Up: The contractor shall at all times maintain good housekeeping practices to reduce the risk of fire damage. All scrap materials, rubbish, and trash shall be removed daily from in and around the work site and shall not be permitted to be scattered on to adjacent properties.

Fire Extinguishers: The contractor shall provide fire extinguishers in accordance with the recommendations of the base fire regulations. Proper type fire extinguisher shall be available at each location where cutting and welding is being performed. When electrical or gas welding and/or cutting is being implemented, interposed shields of incombustible material shall be used to protect against fire damage, due to sparks and hot metal.
