About this opportunity

Salt Lake County, on behalf of Human Resources, is requesting proposals for pre-employment physical examinations, fitness-for-duty evaluations, tuberculosis testing, and drug and alcohol testing services, with proposals due October 14, 2026 at 9:00 a.m. through the Utah Public Procurement Place (U3P) powered by Bonfire. A pre-proposal WebEx meeting is scheduled for September 23, 2026 at 3:00 p.m., with questions due September 30, 2026.

The scope covers Department of Transportation (DOT) pre-employment physicals, Law Enforcement Officer (LE) pre-employment exams for the Peace Officers Merit Commission, fitness-for-duty evaluations (physical and psychological), tuberculosis testing for existing law enforcement employees, and drug and alcohol testing for specified employee groups. Estimated annual volume includes approximately 150 DOT physicals, 150 law enforcement pre-employment physicals, 1,200 TB tests, 750 drug and alcohol tests for DOT employees, and 420 tests for non-DOT employees. Proposers must demonstrate capacity to complete law enforcement pre-employment exams for hiring classes of 30–40 candidates within seven-day windows. All DOT examinations must be conducted by FMCSA-certified medical examiners. Proposers must hold comprehensive insurance including commercial general liability ($500,000 per occurrence/$1,000,000 aggregate), errors and omissions ($1,000,000 per occurrence/$2,000,000 aggregate), and commercial automobile liability; comply with the Utah Medical Practice Act and HIPAA; and maintain documentation for seven years.

Performance guarantees require 95 percent of law enforcement exam results within 5–7 days, 95 percent of drug and alcohol and TB test results within 2–3 days, and 95 percent of DOT test results within 1 day, with quarterly rebate penalties for non-compliance. The contract carries an initial four-year term with an option for one additional two-year renewal. Evaluation weights the program proposal at 70 percent and the pricing proposal at 30 percent. Scoring preferences apply to proposers offering employee health care benefits or veterans and companywide hiring initiatives. Proposers must submit three separate PDFs: a program proposal not exceeding 25 pages, a pricing proposal via Exhibit C, and optional supplemental attachments. Selection committee meets October 21, 2026, with proposer interviews scheduled for October 28, 2026 as needed.
