About this file

This document is a Scope of Work and evaluation criteria exhibit for Salt Lake County, Utah's Human Resources Department solicitation for pre-employment physical examinations and fitness-for-duty services. The County seeks qualified medical service providers to conduct Department of Transportation (DOT) physical exams, Law Enforcement Officer (LE) exams for the Peace Officers Merit Commission, fitness-for-duty evaluations (both physical and mental), tuberculosis testing for existing law enforcement employees, and drug and alcohol testing for specified personnel. The evaluation criteria are organized into three exhibits: Exhibit A addresses functional specifications including FMCSA-licensed medical examiner requirements, examination scheduling capabilities, result turnaround times, location availability, and protocols for handling failed examinations; Exhibit B establishes general and customer service qualifications including company experience, client retention and satisfaction rates, relevant expertise in DOT and LE examinations, and customer support models; and Exhibit C specifies pricing structure by test type per employee.

The functional requirements indicate that proposers must demonstrate capacity to serve large hiring classes of 30-40 law enforcement candidates requiring completion of pre-employment exams within seven-day windows. Detailed reporting and invoicing specifications require inclusion of date of service, service type, cost, employee name, and employee number as unique identifier, with explicit exclusion of Social Security numbers and dates of birth. Proposers must clarify their billing processes, capability for direct billing to county subdivisions, and procedures for random and DOT drug testing file uploads. The evaluation criteria require submission of physician and medical personnel resumes, demonstration of experience with public sector organizations through case studies, and three client references. No specific contract term, renewal options, response deadline, or award date information is provided in this exhibit document.
