Test kits must maintain a minimum ninety-day shelf-life from receipt and be shipped within seven calendar days of order placement to 1155 South Oakland Avenue, Carbondale, Illinois 62901. The bid submission deadline is March 16, 2026, with bids remaining firm for 180 days from opening, and award will be made to the lowest priced responsive and responsible bidder based on evaluation criteria including responsibility, responsiveness, and price.
Vendors responding to this procurement must submit either a State of Illinois Vendor Disclosure form or an IPG Active Registered Vendor Disclosure form to be considered responsive, and all contractors must comply with prevailing wage requirements established by the Illinois Department of Labor for services performed in Jackson County. The procurement includes preferences for domestically manufactured products under Illinois Procurement Code provisions, with United States-manufactured products preferred unless unavailable in reasonable quantities, significantly more expensive, or of substantially lower quality; vendors may claim applicable preferences such as Domestic Product Preference through designated line item fields. The Illinois Procurement Code provisions may support Business Enterprise Program or Veterans Small Business Program participation through applicable utilization plans. Vendors must demonstrate compliance with multiple state and federal requirements, including the Illinois Human Rights Act, civil rights statutes, environmental protection standards, drug-free workplace standards, and the Illinois Information Technology Accessibility Act, with vendor disclosure forms requiring certifications regarding debarment status, criminal violations, equal employment opportunity policies, and conflicts of interest for contracts exceeding $100,000 in annual value. All resulting contracts will incorporate the Illinois Standard Terms and Conditions and require general commercial liability insurance of $1,000,000 per occurrence and $2,000,000 annual aggregate, auto liability insurance of $1,000,000 per occurrence, and workers' compensation insurance as required by law. The total maximum contract amount will be determined by the State prior to execution, with payment made in accordance with the State Prompt Payment Act and invoices due no later than July 31 following the service year; vendors awarded contracts of $1,000 or more must pay a $15 administrative fee to cover Minority Contractor Opportunity Initiative expenses, which will be deducted from the first payment issued.
