BCI Background Checks and HIPAA agreements are required for all on-site workers and trades. The construction cost estimate is $2.1 million. Substantial completion is October 1, 2027, with liquidated damages of $2,000 per day for delays beyond fourteen days after that date. A Health Insurance Statement of Compliance Form is required from the awarded contractor prior to Notice to Proceed. Bid bonds of 5% of the bid amount on AIA Document A310-2010 and performance/payment bonds of 100% on AIA Document A312-2010 are required. A mandatory pre-bid meeting is scheduled for September 15, 2026 at 10:30 a.m. in American Fork. Questions are due September 22, 2026. Subcontractor lists with bid amounts of $35,000 or more must be submitted by October 7, 2026 within 24 hours of bid opening. DFCM Addendum #001 (September 3, 2026) and Addendum #002 (September 25, 2026) clarify scope, VFD specifications, and equipment schedules; bidders must acknowledge receipt of both addenda with their bids.
