About this opportunity
The West Virginia Department of Health and Human Resources, on behalf of Hopemont Hospital in Terra Alta, issued Request for Quotation (RFQ) No. ARFQ HOP1600000001 for the procurement of bakery items to be delivered to the facility on a continuing basis. The solicitation encompasses seven line items: 10,000 loaves of sliced white bread (22-24 oz), 4,000 loaves of sliced whole wheat bread (22-24 oz), 200 loaves of sliced Italian bread (20 oz), 15 loaves of sliced rye bread (22-24 oz), 700 hot dog buns (16 count), 1,800 hamburger buns (16 count), and 400 split top dinner rolls (16 count). All products must be delivered weekly to Hopemont Hospital's Central Receiving at 150 Hopemont Drive, Terra Alta, West Virginia 26764-7728, within six working days of order receipt on an F.O.B. destination basis. The contract is structured as an open-end Agency Master Agreement with a one-year initial term and up to three successive one-year renewal options not to exceed 36 months total. Award will be made to the responsive and responsible vendor submitting the lowest total cost while meeting all specification requirements. Questions regarding the bid must be submitted in writing by July 31, 2015, at 12:00 PM to Shelby Hauser at shelby.a.hauser@wv.gov, with quotations due by August 14, 2015, at 12:00 PM via hand delivery, facsimile, email, or through the WVOasis Vendor Self Serve portal. Prior to award, the successful vendor must be registered with the West Virginia Purchasing Division and may be required to submit performance bonds, labor/material payment bonds, proof of workers' compensation insurance, and applicable licenses and certifications.
The RFQ includes vendor preference opportunities available to West Virginia residents, partnerships or corporations maintaining headquarters in the state for at least four years, veterans (both resident and nonresident), and certified small or women- and minority-owned businesses, with preference awards ranging from 2.5 percent to 5 percent applied as evaluation adjustments, plus an additional 3.5 percent for veteran-owned businesses. Vendors must certify completion of a West Virginia Purchasing Affidavit affirming under penalty of perjury that neither they nor related parties owe debts exceeding one thousand dollars in aggregate to the state or its political subdivisions, nor are in employer default status, with exceptions for those contesting assessments or in compliance with payment plans. The solicitation does not identify any incumbent vendors or current contractors, nor does it specify an explicit contract value or budget range; pricing shall be submitted on a unit cost basis for each line item with all delivery charges included in the bid. All vendors must comply with applicable federal, state, and local laws, including West Virginia's state enrichment law requirements for bakery products, and must maintain sufficient inventory to support the hospital's ongoing operational needs with provisions for quarterly and annual purchase reporting.
