About this opportunity

The Department of Veterans Affairs seeks an independent contractor to conduct annual unannounced surveys at 134 Community Living Centers and 5 Spinal Cord Injury/Disorder Long Term Care Centers using CMS processes and Title 42 CFR Part 483, with proposals due June 30, 2026. The contract is structured as an Indefinite Delivery/Indefinite Quantity vehicle with a 12-month base period and four 12-month option periods, a $24.5 million estimated ceiling, and a $40,000 minimum guarantee per awardee.

Place of performance encompasses VA facilities nationwide across all 50 states and US territories. Contractors must employ surveyors with a minimum of three years' external review experience, including Registered Nurses, Social Workers, Dietitians, and personnel trained in fire safety and NFPA codes. All contractor personnel require Tier 2/Moderate Risk level vetting and mandatory training in VA privacy/HIPAA, cybersecurity, VA Handbook 6500.6, CMS Long Term Care Survey Basic Training, and the HATCh model and Federal tag crosswalk documents.

Deliverables include quarterly survey schedules, survey reports within 10 business days of completion, electronic monthly feedback reports, immediate jeopardy notifications within one hour of identification, a Quality Assurance and Surveillance Plan, Program Work Plan, Communication Plan, and Personnel Training Plan. Monthly and quarterly meetings with Government representatives are required. The NAICS code is 541611, Administrative Management and General Management Consulting Services. Invoices must be submitted electronically through the Tungsten e-invoicing system or VA Box. A Business Associate Agreement governs handling of Protected Health Information under HIPAA. Data breach liquidated damages are $37.50 per affected individual. FAR clause 52.222-90 requires contractors to refrain from racially discriminatory diversity, equity, and inclusion activities.

Evaluation weighs technical approach as most important, followed by past performance, price, and veterans preference factors, with ratings from Excellent to Unacceptable. An amendment posted June 4, 2026, extended the deadline from June 12 to June 30 and added the Past Performance Questionnaire as a required submission element.
